3 Council Minutes The council of the City of Washington, Iowa, met in Regular Session in the council chambers in the Former Public Library Building, 120 E. Main Street, at 6:00 P.M., Wednesday, November 16, Mayor Sandra Johnson in the Chair. On roll call present: Hagie, Stark, Roth, Shepherd. Absent: Zieglowsky, Wilson-Johnson. Motion by Stark, seconded by Roth, that the agenda for the Regular Session to be held at 6:00 P.M., Wednesday, November 16, 2011 be approved as amended. Mayor Johnson requested that the addition of oath of office for new police officer Brett Johnson be added the agenda. Motion carried unanimously. Consent Agenda: 1. After discussion, motion by Hagie, seconded by Shepherd, to enter into and authorize the Mayor to sign a three year municipal lease agreement with Mid-Iowa Solid Waste Equipment Company for an Aries Pathfinder System and 7 x 14 trailer in the amount of $34,833 per year starting December 1, Motion carried unanimously.

4 Motion by Stark, seconded by Roth, to enter into a grant request to Iowa DOT for the North 4th Avenue siding crossing (north of the main line). Motion carried unanimously. Motion by Stark, seconded by Roth, to approve the Administrative Policy on Employee Utilization of Payroll Direct Deposit. Motion carried unanimously. Motion by Shepherd, seconded by Stark, to approve Restore Purchasing Policy Adopted in Resolution No Motion carried unanimously. Motion by Roth, seconded by Hagie, to Authorize the Administrative Settlement Determinations and Authorizing the Mayor to Sign. Motion carried unanimously. Motion by Roth, seconded by Shepherd, that the council go into closed session per Iowa Code Section 21.5(c) Pending Litigation. Roll call on said motion as follows: Ayes: Hagie, Stark, Roth, Shepherd. Nays: none. Motion carried. Motion by Roth, seconded by Shepherd, that the council return to open session. The finance/personnel committee will meet at 5:00 P.M., Wednesday, November 30 in the conference room in City Hall. 11 11/12/11-11/25/11 STREETS: M/C personnel continued the leaf vac program staying on schedule, compiling 176 loads to date. Crews finished crack sealing the vertical seams on South 9 th Ave from East Washington St/East Polk St and horizontal seams on West 5 th St at North Marion Ave/North Iowa Ave. Yield signs were installed at North 7 th Ave- Eat 9 th St for east/west traffic as passed by council. No Parking signs were either moved or installed in various areas due to amendments of Ordinances by City Council. The street sweeper operated in most areas of the city and will continue weather permitting. Continued with stop and yield sign replacement program. WATER DISTRIBUTION: M/C personnel repaired 2 water main breaks, located at #906 South Ave D where a repair sleeve was placed to fix the leak on a 6 CIP. The other break was located at #608 South 12 th where a section of 4 PVC was installed. Numerous water box repairs were made, including one which required a whole new service installation from the main to the curb box. SEWER COLLECTION: M/C personnel dye tested sewer services to locate their destination of which sewer main they traveled to. STORM SEWER COLLECTION: M/C personnel unplugged some intakes with the rain we had received. MECHANIC/SHOP/CEMETERY: M/C personnel had 4 funeral services at Elm Grove and winterized the cemetery office. Replaced front rotors, pads and all bearings and seals on #302. Serviced both trash pumps, replaced spark plugs in #633 and #634. Replaced broken window in #133(loader), replaced new chain sprockets and drive chain on crack sealer. Serviced snow plow #402 and installed new spreader motor on #301 tailgate spreader. Finished installing #119 spreader, installed new couplers on tamp and started work on #601 spreader. OTHER: M/C personnel completed yard waste and brush pick up each week. Personnel had 13 water shut offs for nonpayment. Crews hauled numerous loads of spoil away from the shop. Responded to 45 One Call Locates. Hauled some rock and sand back to the stock piles. Please note that this report does not include every task M/C personnel performed, but shall be a highlight of our work performed as a department.

12 WWTP report for December 7, 2011 Council Meeting After hour alarm and dog call outs 19 th dog call, Police wanted dog trap set in the park, 2:00 p.m. Jason 20 th dog call, Police wanted dog trap moved to new location, 9:00 a.m. Jason 20 th dog call, dog caught in the trap, 12:00 p.m. Jason 26 th dog call, Safety Center reported a dog to be picked up at 525 South B, 1:30 p.m. Jason WWTP lagoon The lagoon filled to four (4) feet on the 26th. We started pumping contents back for treatment on the 28 th and were finished by 7:15 a.m. on the 30 th. The lagoon is pumped down and ready for the next rain. WWTP October 2011 MOR 0/31 days bypass over lagoon standpipe, million gallons (mg) treated, mg through WWTP, mg in and out of lagoon. Average daily flow mg, maximum daily flow mg, minimum daily flow mg, Sunset Park lift station bypassed for a total of 0.0 hours. NO SSO S out of the head works building for October TSS Removal 85 % required result = 93.2 % Influent TSS monthly average = mg/l Effluent TSS monthly average = 16.2 mg/l CBOD5 Removal 85% required result = 93.0% Influent CBOD5 monthly average = mg/l Effluent CBOD5 monthly average = 8.9 mg/l October 2011 WWTP NPDES effluent permit violations - None to report. Outside WWTP equipment Jason and Danny got the grease and oil changed in all the equipment so it s ready for winter. USEPA written inspection report Copies available at City Hall or the WWTP office for review. The new WWTP and west side gravity sewer will address the USEPA s concern of the all by-passes and SSO s that are occurring in Washington. They recommend the City stay aggressive in dealing with the I/I problems occurring in our sewer collection system. Digester boiler Danny cleaned the boiler flues on the 29 th. Digester recirculation pump Joe Marie replaced some old wires inside the digester control panel on the 29 th. The recirculation pump kept tripping its breaker. WWTP flow meters Jason and Danny fixed a flow meter sensor head. Then calibrated the influent and by-pass meters on the 21 st. Pine trees We cut down seven dead pines at the WWTP. The trees were by the final clarifiers and trickling filter. We decided to cut them down before they landed on the trickling filter media or landed in the clarifier tank. We hauled them to the brush pile when the end loader was here. Underpass lift station building Danny replaced a couple of window panes that had been broken with rock and a bb gun on the 1 st.

13 WWTP resumes normal hours The WWTP will no longer be open from 7:00 a.m. to 10 a.m. on Saturday mornings. We resumed our normal hours (M-F 7-4:00 p.m.) on the 28 th for yard waste drop off. Fred E. Doggett 12/2/2011 9:19 AM

14 To: From: CC: Washington City Council Washington Public Library Illa Earnest, City Clerk Brent Hinson, City Administrator Date: December 2, 2011 Subject: 2 nd November Report to Council After a very eventful November that included adding two new members to the library family (the Director's new little boy, Raymond Michael, and Children's Librarian Jolisa Weidner's new granddaughter, Ivy Belle, both born on the same day!) the library staff is settling in for the winter. Here are a few things we accomplished in November: The Library Board of Trustees had its annual meeting in November and named Rachel Nicola as its new president. I'm looking forward to working with Rachel and the Trustees on a number of projects during this next year, including a strategic longterm plan for the library and continued expansion of our programming. The library filed its initial paperwork for e-rate funding from the federal government. This is a discount program for telecommunications including telephone and Internet services. The library's discount rate will be 70%, and it will take effect July 1, After switching to our new catalog system, we discovered that our patron access catalog computers in the library were not strong enough to run the new software. With the help of the Washington School District's technology staff I contacted the REACT Center in Hiawatha, Iowa. It is a non-profit organization affiliated with Rockwell Collins that takes donated computers from large companies, refurbishes them, and offers them free-of-charge to educational facilities. The library was able to receive six free computers from the Center and they are now up and running. This saved the library and the City well over one-thousand dollars that would have been required to replace the old computers with new towers. Thank you for your support!
