3 VI. REPORTS OF COMMITTEES B. EDUCATION MS. EVE LYNN WESTBROOKS DR. UNA H. R. KEENON 1. RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO GOANIMATE INC. WHEREAS, the will contract with GoAnimate, Inc. to provide a safe online learning environment for students to reinforce the Ohio Learning Standards through animation for the school year, at a cost of $6,300.00; therefore, be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $6,300 for said services from Title I RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO KATIE NOVAK (UDL 101) WHEREAS, the will contract with Katie Novak to provide an online professional development course focused on the Universal Design for Learning (UDL), and WHEREAS, all participants will be equipped with the skills to incorporate UDL for creating integrated, sustainable action plans for long term implementation; recruit and engage students in the UDL experience by creating a shared culture of mastery oriented feedback; align the multi-tiered systems of support with the UDL framework; address issues of social justice for students of all variability and; create lessons that will effectively differentiate instruction while strengthening core instruction, and WHEREAS, fifty teachers and administrators will participate in the introductory course through the Canvas Online Learning System, at a cost not to exceed $6,000.00; therefore, be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $6, for said services from Title I

RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO KATIE NOVAK (UDL 201) WHEREAS, the will contract with Katie Novak to provide an advanced online professional development course focused on the Universal Design for Learning (UDL), and WHEREAS, all participants will be equipped with the advanced skills to incorporate UDL for creating integrated, sustainable action plans for long term implementation; recruit and engage students in the UDL experience by creating a shared culture of mastery oriented feedback; align the multi-tiered systems of support with the UDL framework; address issues of social justice for students of all variability and; create lessons that will effectively differentiate instruction while strengthening core instruction, lead UDL peer training; and WHEREAS, seventy five teachers and administrators will participate in the advanced course through the Canvas Online Learning System, at a cost not to exceed $9,000.00; therefore, be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $9, for said services from Title I RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO VISIONARY LEADERS INSTITUTE WHEREAS, the will contract with Visionary Leaders Institute to assist the students and enhance test readiness by reviewing and refreshing the technological skills of students in preparation for the next generation of online assessments, and WHEREAS, the tools for effective use of technology, assessment navigation, online tool usage and practicing tools in an online testing environment to help students achieve academic success, and 4

5 WHEREAS, the Visionary Leadership Institute will work with teachers at Caledonia Elementary School, Chambers Elementary School, Mayfair Elementary School, Superior Elementary School and Heritage Middle School at a cost of $17,000.00; therefore, be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $17, for said services from Title I RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO CUYAHOGA COMMUNITY COLLEGE PUBLIC SAFETY TRAINING CENTER WHEREAS, the will contract with Cuyahoga Community College Public Safety Training Center to provide fifty (50) hours of 911 telecommunication training for Shaw High School Criminal Justice and Fire Technology students during the school year at a cost of $4,500.00; therefore, be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $4, for said services from the Career Tech/General Fund RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO UNITED CEREBRAL PALSY ASSOCIATION OF GREATER CLEVELAND, INC. WHEREAS, this Agreement is made between United Cerebral Palsy Association of Greater Cleveland, INC (UCPA) and to provide district wide Individualized Education Program services for the purpose of providing occupational therapy in the public school setting to students with a wide range of abilities from April 10, 2016 to July 31, 2018; and WHEREAS, The District shall pay UCPA Eighty Dollars ($80.00) per hour of OTR, and Sixty Dollars ($60.00) per hour of COTA service; and mileage of fifty-five ($0.55) per mile between District sites; therefore, be it RESOLVED, that the accepts the April 10, through July 31, 2018 United Cerebral Palsy Association of Greater Cleveland, Inc., Therapy Agreement and authorizes the Superintendent or her representative to enter into said agreement with United Cerebral Palsy Association of Greater Cleveland Inc., and further be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $100, for said services from the IDEA Fund

6 7. RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO DR. ANDREW JONES WHEREAS, Dr. Andrew Jones will provide eighth grade students with instruction in arithmetic and algebraic structure, designed to enable students to justify each equation that they write with an axiom definition or a theorem; and WHEREAS, students will learn how to follow theorems and algorithms, and will receive instruction relative to statements, sets, natural numbers, whole numbers integers, rational numbers, and linear equations; and WHEREAS, this program will take place from April 17, 2017 to May 26, 2017; therefore, be it RESOLVED, that this Board authorizes the Treasurer to encumber and pay an amount not to exceed $1, from Title I Fund RESOLUTION TO AUTHORIZE AN AGREEMENT WITH THE CASE WESTERN RESERVE UNIVERSITY FRANCES PAYNE BOLTON SCHOOL OF NURSING WHEREAS, Case Western Reserve University on behalf of its Frances Payne Bolton School of Nursing agree to cooperate in the provision of clinical experiences for undergraduate and/or graduate students enrolled in the Frances Payne Bolton School of Nursing; and WHEREAS, the shall agree on the terms of the use of preceptors for the use of any precepted learning experiences; and WHEREAS, the University and School District agree to the terms as outlined in the agreement (see attachment); therefore, be it RESOLVED, that this of the authorizes the Superintendent or her designee to enter into agreement with Case Western Reserve University on behalf of its Frances Payne Bolton School of Nursing for said services for a period of five (5) years beginning March 10,

RESOLUTION TO ADOPT THE EAST CLEVELAND PUBLIC LIBRARY 2017 TAX BUDGET WHEREAS, Ohio Revised Code Section requires the East Cleveland Public Library Board of Library Trustees to submit its Tax Budget (attached) to the East Cleveland on or before May 31, 2017; therefore, be it RESOLVED, that the following Tax Budget indicating the income needed by the East Cleveland Public Library for the year 2017 is $2,297, of which, approximately $1,390, would be received from the Public Library Fund of the State of Ohio, be submitted to the East Cleveland for submission to the County of Cuyahoga on or before July 20, RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO EDGE DOCUMENT SOLUTIONS, LLC FOR A SERVICE AGREEMENT FOR AN UNLIMITED USER ULTIMATE EDGE SOFTWARE LICENSE AND TWO HEWLETT PACKARD PRINTERS, SN USGNN07889 AND CNGXV75545 WITH SERVICE PROVIDED BY HANK S SALES AND SERVICE INC. WHEREAS, the East Cleveland City School district will contract with Edge Document Solutions, LLC to provide an annual service agreement for an unlimited user Ultimate Edge software license for the East Cleveland City School district commencing July 1, 2017 through June 30, 2018 at a cost of $1, and two Hewlett-Packard printers at a cost of $ each; WHEREAS the Printer Maintenance Agreement covers technical support, emergency printer support and maintenance; therefore be it RESOLVED, that the East Cleveland hereby authorizes the Treasurer encumber and pay an amount not to exceed $2, for said services from the General Fund 001. (See Exhibit #2) 7

RESOLUTION TO AUTHORIZE A CONTRACT WITH AND PAYMENT TO ACCOUNTEMPS WHEREAS, the will contract with Accountemps to provide temporary staffing services for the District, and WHEREAS, the temporary staffing services provided by Accountemps will be for a term of at least 150 work days; and effective May 16, 2017 WHEREAS, Accountemps will provide as-needed personnel at a rate equal to hourly pay, tax burden, workers compensation, unemployment costs, social security, and state disability insurance or other payroll charges; therefore, be it RESOLVED, that this Board authorizes the Treasurer to pay all costs associated with Accountemps to be paid from the General Fund

RESOLUTION TO APPROVE THE PROCEDURE FOR PERIODIC REVIEW OF BOARD MINUTES PERTAINING TO THE COURT SETTLEMENT EAST CLEVELAND CITY SCHOOL DISTRICT BOARD OF EDUCATION PROCEDURE FOR PERIODIC REVIEW OF BOARD MINUTES At least twice in a fiscal year, the Treasurer or designee shall conduct a periodic review of the Board s meeting minutes to ensure accuracy as compared to audio recordings. The review shall be conducted by listening to the recording of at least one regular and at least one special meeting and verifying that the written minutes are accurate as compared to the audio recording. Review of the [date] regular / special Board meeting (circle one) In order for the minutes to be considered accurate, all of the following must be checked off: Minutes must accurately set forth the place, date and time of the meeting. Minutes must accurately set forth a list of Board members who are present or absent. 9

10 Minutes must accurately record all decisions made by the Board, including all votes of each Board member. Minutes must record all motions, including but not limited to motions to amend the minutes, regardless of whether those motions are seconded and/or approved. Minutes must include a synopsis or description of any discussion of matters the Board is considering. The synopsis or description need not be a verbatim transcript, but must include sufficient facts and information to permit an individual who was not present at the meeting to understand and appreciate the rationale behind the Board s decision. Minutes must include a list of all documents or exhibits used by the Board during the meeting. Minutes must state the proper basis for executive session, under O.R.C (G), as described below: 1. To consider one or more, as applicable, of the check marked items with respect to a public employee or official: a. Appointment; b. Employment; c. Dismissal; d. Discipline; e. Promotion; f.
