2 CONSENT AGENDA 1) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, amending Article VII, Section 7.11 of the Personnel Rules and Regulations of the City of Deerfield Beach relating to the Family Medical Leave Act incorporating the National Defense Authorization Act, provisions of certain family members of military personnel 2) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, approving the request to rename S.W. 8 th Court (from Martin Luther King Boulevard, S.W. 3 rd Avenue to Dixie Highway) to Madison Court. 3) Resolution / - A Resolution of the City of Deerfield Beach, Florida, approving the request to rename S.E. 3 rd Avenue to Pruitt Avenue. 4) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, approving the request to rename N.W. 3 rd Court (from N.W. 4 th Avenue to N.W. 3 rd Avenue/MLK, Jr. Avenue) to Coretha Brown Court. 5) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach authorizing and supporting the grant application for funding from the Edward J. Byrne Memorial Justice Assistance Grant (JAG) Program FY Local Solicitation. 6) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach approving an amendment to an agreement between the City of Deerfield Beach, the Areawide Council on Aging of Broward County. 7) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, authorizing a Request for Proposal to the Areawide Council on Aging of Broward County for Supportive Services at N.E. 3 CONSENT AGENDA CONTINUED 8) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, authorizing the Mayor to enter into Modification Number Three to Subgrant Agreement Number 09HM between the Florida Division of Emergency Management and the City of Deerfield Beach, extending the performance period of the agreement. 9) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach authorizing the proper City officials to approve the expenditure of Community Development Block Grant (CDBG) Funds in an amount not to exceed $9, to make housing rehabilitation repairs to the home of Jeanne Bernard. (Funds from Community Development Block Grant Funds) 10) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, providing for the rejection of all proposal responses to the /20 Sporting Officials and Referees (RFP # /20) since two (2) of the responses were not responsive to the RFP and the third proposer would not agree to the requirements of the RFP; and providing for an effective date. 11) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, granting a waiver, as authorized in Section 54-1 of the City Code, with respect to the restriction on re-employment compensation to Linda King, Acting Director of Human Resources. 12) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, authorizing the City Manager to execute an application with the State of Florida Department of Transportation for the temporary closing/special use of Hillsboro Boulevard for the Annual Fourth of July Celebration. 3

6 QUASI-JUDICIAL PUBLIC HEARINGS 19) P.H. -101: SIGN CASE NO. 694 Applicant: METRO TRAFFIC SCHOOL, by Anthony Lopez Proposal: Seeking the following SIGN VARIANCES from the provisions of the Deerfield Beach Land Development Code: 1.) from Section (c)(3)(a) in order to allow a window identification sign which is 11.5 square feet in size rather than the maximum of three (3) square feet allowed by Code; AND 2.) from Section (c)(3)(b) in order to allow a window advertising sign which is square feet in size rather than the maximum ten (10) square feet allowed by Code. Location: The signs are located at 3300 W. Hillsboro Boulevard. ACTION: Motion on decision of Commission 20) P.H. -102: SIGN CASE NO. 695 Applicant: BLACK ROSE TATTOO SHOP, by Robert J. McCullock Sr. Proposal: Seeking a SIGN VARIANCE from the provisions of Section (c)(3)(b) in order to allow two (2) window advertising signs, each of which is 32 square feet in size, and each of which contains letters which are eight (8) inches in height. The Code allows only 1 window advertising sign which is not in excess of ten (10) square feet in size, with a maximum height of letters of four (4) inches. Location: The signs are located at 3300 W. Hillsboro Boulevard. ACTION: Motion on decision of Commission 21) P.H. -103: SIGN CASE NO. 696 Applicant: ZION LUTHERAN CHURCH, by Frank Strey Proposal: Seeking a SIGN VARIANCE from the provisions of Section (b) of the Deerfield Beach Land Development Code in order to allow a monument sign located five (5) feet from the property line to be 40 square feet in size and nine (9) feet in height rather than 15 square feet in size and five (5) feet in height as allowed by Code. Location: The sign will be located at 959 S.E. 6 th Avenue. ACTION: Motion on decision of Commission GENERAL ITEMS 22) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, authorizing the Mayor to enter into a Local Agency Program Agreement with the Florida Department of Transportation for Financial Management No , SW 10 th Street Linear Park Improvements City Project No P. (Funds from the Transportation Enhancement Program) (Tabled from May 18, ) 6

7 GENERAL ITEMS CONTINUED 23) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, Florida, relating to the provision of Fire Rescue Services, Facilities and Programs in the City of Deerfield Beach, Florida; establishing the estimated rates for Fire Rescue Assessments for the Fiscal Year beginning October 1, ; directing the preparation of an assessment roll; providing for a vacancy adjustment procedure; authorizing a public hearing and directing the provision of notice thereof; and providing an effective date. 24) Resolution / - A Resolution of the City Commission of the City of Deerfield Beach, approving an agreement between the Florida East Coast Railway and the Florida Department of Transportation entitled Railroad Reimbursement Agreement Grade Crossing and Crossing Devices Municipal to include N.E. 2 nd Street. ADMINISTRATIVE ACTIONS AND CITY MANAGER COMMENTS COMMENTS BY MAYOR AND COMMISSIONERS PUBLIC TO BE HEARD Persons addressing the Commission during this segment may not discuss items that are on the agenda. A three (3) minute time limit will be imposed on those addressing the Commission. The Commission shall take the subject under advisement and refer to administration unless a majority of the Commission wishes to discuss the matter. ADJOURNMENT FUTURE MEETINGS Regular Meeting June 15, Regular Meeting August 3, Written requests to address the Commission must be submitted to City Manager one week (Tuesday by 2:00 PM) prior to commission meeting. Any person wishing to appeal any decision made by the City Commission with respect to any matter considered at such meetings or hearings will need a record of the proceedings, and for such purposes may need to ensure that a verbatim record of the proceedings is made, which record includes the testimony and the evidence upon which the appeal is made. The above notice is required by State Law (F.S ). Anyone desiring a verbatim transcript shall have the responsibility, at his/her own expense, to arrange for the presence of a certified court reporter at the hearing. 7
