2 PUBLIC COMMENTS None APPROVAL OF MINUTES November 5, 2014 RESOLUTION Deb Ward WHEREAS, the Prince William Local Human Rights Committee has reviewed the minutes of November 5, 2014; and NOW, THEREFORE, BE IT RESOLVED that the Prince William Local Human Rights Committee does hereby approve the minutes of the November 5, 2014 meeting. Abstained From Vote None Absent From Meeting None 2

3 OLD BUSINESS The new PWC LHRC committee member will be approved at the December 12, 2014 State Human Rights Committee meeting. AUTHORIZED CLOSED SESSION RESOLUTION Deb Ward WHEREAS, the Prince William County Local Human Rights Committee desires to discuss in Closed Session client related matters; and WHEREAS, pursuant to VA Code (A) paragraph 15 for the protection of the privacy of individuals and their records in personal matters not related to public business such discussions may occur in Closed Session; NOW, THEREFORE, BE IT RESOLVED that the Prince William County Local Human Rights Committee does hereby authorize discussion of the aforestated matters in Closed Session. 4 CERTIFICATION OF CLOSED SESSION RESOLUTION Deb Ward WHEREAS, the Prince William County Local Human Rights Committee has this day adjourned into Closed Session in accordance with a formal vote of the Committee, and in accordance with the provisions of the Virginia Freedom of Information Act; and WHEREAS, the Freedom of Information act requires certification that such Closed Session was conducted in conformity with the law; NOW, THEREFORE, BE IT RESOLVED that the Prince William County Local Human Rights Committee does hereby certify that to the best of their knowledge only matters lawfully exempted from open meeting requirements and only such matters for the protection of the privacy of individuals and their records in personal matters not related to public business were heard, discussed or considered. RESOLUTION Becky Spain WHEREAS, the Restrictive Behavior Plan for E.B. has been reviewed and approved for use by the Behavior Management Committee of the Prince William County Community Services; and WHEREAS, the Restrictive Behavior Plan for E.B. has been presented to the Prince William Local Human Rights Committee for approval; NOW, THEREFORE, BE IT RESOLVED that the Prince William Local Human Rights Committee does hereby approve the Restrictive Behavior Plan for E.B. RESOLUTION Becky Spain WHEREAS, the Restrictive Behavior Plan for E.N. has been reviewed and approved for use by the Behavior Management Committee of the Prince William County Community Services; and WHEREAS, the Restrictive Behavior Plan for E.N. has been presented to the Prince William Local Human Rights Committee for approval; and NOW, THEREFORE, BE IT RESOLVED that the Prince William Local Human Rights Committee does hereby approve the use of the Restrictive Behavior Plan for E.N. 7 THE RESTRICTIVE BEHAVIOR PLAN FOR C.P. IS NO LONGER REVIEWABLE BY THE LOCAL HUMAN RIGHTS COMMITTEE This plan is no longer reviewable because C.P. has met his plan objectives. APPROVED THE CONTINUED USE OF RESTRICTIVE BEHAVIOR PLAN FOR R.D. RESOLUTION Becky Spain WHEREAS, the Restrictive Behavior Plan for R.D. has been reviewed and approved for use by the Behavior Management Committee of the Prince William County Community Services; and WHEREAS, the Restrictive Behavior Plan for R.D. has been presented to the Prince William Local Human Rights Committee for approval; NOW, THEREFORE, BE IT RESOLVED that the Prince William Local Human Rights Committee does hereby approve the Restrictive Behavior Plan for R.D. 8 ANNUAL PROGRAM REVIEWS ELITE HEALTHCARE SERVICES Rita Adjetey presented the annual report for Elite Healthcare Services, LLC HUMAN RIGHTS ADVOCATE REPORT Deb Lochart, the Virginia DBHDS Office of Human Rights Director, stated that practice protocols are being developed for electronic devices to include video monitoring and what can and cannot be used. Ms. Lochart and Ms. Burlar saw how it worked and said it is very restricted and used for fragile medical needs only, not for behavior. Susan Rudolph, RN and Tammy Comer were present, and discussed the video monitoring and that regional teams are established to set best practices. Ms. Comer brought a monitor so the committee could see what the equipment looked like. Ms. Lochart asked that the committee allow and monitor the video monitoring at Insight until the practice protocols are in place. The protocol would come to the LHRC to review.
