2 PUBLIC COMMENTS None APPROVAL OF MINUTES March 4, 2015 April 1, 2015 No Quorum RESOLUTION WHEREAS, the Prince William Local Human Rights Committee has reviewed the minutes of March 4, 2015; and Rights Committee does hereby approve the minutes of the March 4, 2015 meeting. 3 RESOLUTION RATIFY THE ACTIONS FOR THE LOCAL HUMAN RIGHTS COMMITTEE MEETING ON APRIL 1, 2015 WHEREAS, due to lack of a quorum of the full Committee, presentations were heard at the meeting on April 1, 2015; and WHEREAS, ratification of the actions on April 1, 2015 is required by a quorum of the full membership of the Prince William Local Human Rights Committee; and WHEREAS, the following actions were taken by the full membership of the Prince William Local Human Rights Committee on May 6, 2015: APPROVED MINUTES OF MARCH 4, 2015 MEETING APPROVED THE ACTIONS OF THE MEETING ON APRIL 1, 2015 FOR WHICH WAS THERE WAS NO QUORUM APPROVED CONTINUED USE OF RESTRICTIVE BEHAVIOR PLAN FOR A.A. APPROVED CONTINUED USE OF RESTRICTIVE BEHAVIOR PLAN FOR M.A. APPROVED USE OF THE RESTRICTIVE BEHAVIOR PLAN FOR FOR T.M. APPROVED CONTINUED USE OF RESTRICTIVE BEHAVIOR PLAN FOR R.A. APPROVED CONTINUED USE OF RESTRICTIVE BEHAVIOR PLAN NOW, THEREFORE, BE IT RESOLVED that the Prince William Local Human Rights Committee does hereby ratify the actions of the April 1, 2015 meeting, as listed above. 3
4 OLD BUSINESS None NEW BUSINESS Request for Affiliation Transfer Softz Touch Home Health Care David Nsimbo presented and requested affiliation transfer to PWC LHRC Request for Affiliation Review HR P&P, Behavior Management, House Rules AdvoServe Gregory Harrison, Michele Torrence and Mayfte Dusseau presented and requested affiliation with the PWC LHRC. The committee approved the request. RESOLUTION Deb Ward City Healthcare Victoria Onyemezikeya and Kevin Onwidise presented and requested affiliation with the PWC LHRC. The committee approved the request. RESOLUTION Deb Ward Juanita James 4
5 ANNUAL PROGRAM REVIEWS FAMILY INSIGHT Jonathan Sutton and Nicholas Ptaceck presented the annual report for Family Insight. REM Jerry DeRosier and Holly Jarrett presented the annual report for REM Virginia. An individual was present requesting Next Friend status to a REM Virginia consumer. The individual was interviewed by the committee and approved for Next Friend status. RESOLUTION
6 AUTHORIZED CLOSED SESSION RESOLUTION Juanita James WHEREAS, the Prince William County Local Human Rights Committee desires to discuss in Closed Session client related matters; and WHEREAS, pursuant to VA Code (A) paragraph 15 for the protection of the privacy of individuals and their records in personal matters not related to public business such discussions may occur in Closed Session; NOW, THEREFORE, BE IT RESOLVED that the Prince William County Local Human Rights Committee does hereby authorize discussion of the aforestated matters in Closed Session. 6
7 CERTIFICATION OF CLOSED SESSION RESOLUTION WHEREAS, the Prince William County Local Human Rights Committee has this day adjourned into Closed Session in accordance with a formal vote of the Committee, and in accordance with the provisions of the Virginia Freedom of Information Act; and WHEREAS, the Freedom of Information act requires certification that such Closed Session was conducted in conformity with the law; NOW, THEREFORE, BE IT RESOLVED that the Prince William County Local Human Rights Committee does hereby certify that to the best of their knowledge only matters lawfully exempted from open meeting requirements and only such matters for the protection of the privacy of individuals and their records in personal matters not related to public business were heard, discussed or considered. 7

17 HUMAN RIGHTS ADVOCATE REPORT Sarah Burlar reported the following: The SHRC (State Human Rights Committee) disbanded NOVA the regional LHRC on 4/24/15. Affiliates disbursed to other LHRCs.
