5 -71 - EB,6/^in/3 和 у 柘 :: 二 He suggested that the title of the subject should therefore be "The challenge to public health of urban development", since that would bring into focus the extent to which ill-housed people, lacking the means to protect their own health, needed assistance He stressed the particular problem of lack of adequate space for recreation in urban areas ^ which led to complex problems of development all over ihe world, He himself favoured the subject for technical discussions; it would provide an opportunity for active participation by health officials from all over the world. Dr PISEK agreed with Dr Watt that urban development seemed to be the problem most common to countries at all stages of development. He was also ready to accept Dr Watt T s wording for the title of that subject. Dr BROTHERSTON agí?eed with previous speakers that all subjects offered excellent pp^sibilities for technical discussions but felt that the third subject listed was: the one on which WHO could make perhaps the greatest single impact on the way of life of.the people of the world. The effects of urbanization on health or ill health were enormous ; also, it was a subject on which health personnel had to learn from the re-thinking of technicians and planners that had taken place in much recent years. It was the sphere in which^ with the.knowledge and experience Г ;v available, WHQ 0 could clo most to improve present circumstances r Professor GONZALEZ TORRES said that cancer did not deserve the highest priority^ at least in developing countries Nutrition did, but there were already many programmes afoot on that subject, which had also been dealt with many times in the

6 EB3;6/Min/5,R.ev.l discussions of the Executive Board and the Assembly, as had the control of the quality of pharmaceutical preparations. He therefore would like to see the problems of urban development selected for the technical discussions at the Twentieth World Health Assembly; many urban populations had not the elements necessary for physical and mental well-beings and those living in semi-urban fringe areas were sometimes even worse off. He was thinking of some places in his own......, ; ' ' '.! ; country and the slums on the outskirts of many cities all over the woi?ld* They lacked sewerage, heating, water supplies and other basic amenities. Professor KAREPA-SMART said that it was apparent that the majority of members of the Executive Board supported the selection of the third subject for the technical discussions to be held at the Twentieth World Health Assembly* Büii, since the discussions of the Board also formed the groundwork for later discussions, it should be noted that smoking as a cause of cancer was going to assume great public health importance in the near future Almost all countries in the world had been led to make some announcements for the guidance of the population on the effects of smoking> and. a frank objective and scientific discussion at world level might help to consolidate international medical opinion on the question 一 he was thinking particularly of the effect it might have on young people.... \ ' :+... Health in its Assembly programme had expressed willingness for the first time to include that question

7 -73 - EB^6/ÍVlin/5 Rev.l Dr KEITA observed that there were two suggestions for amendments to the wording of the third subject in the selection list. Dr Watt's suggestion seemed to be more cpncise. He agreed that the subject was a very important one which must be seen in its widest context, taking into account the problems of slums, sewerage, water supply and space, v/hich were all problems deserving study. Dr VIANNA agreed that the public health problems of urban development would... : -. *...,. -. On ;... " : -. be the most suitable subject for technical discussions, bearing in mind particularly the developing countries. Dr ABDULHAPI hesitated to mention his preference for the fourth subject, the control of the quality of pharmaceutical preparatioris^ since he seemed to be alone in preferring it. But it escaped him why that subject had been passed over so quickly. Health planning and the activities of physicians had become increasingly widespread^ and. wi^h them the consumption of pharmaceutical preparations. illustrate the urgen;t nee(3. for control., he quoted the example of an order To placed in his country for a packet of one thousand micrograms of vitamin B ^ which, upon deliveryj had been found to.9ontain only 250 micrograms Dr SUBANDRIO agreed: with previous speakers that all the suggested subjects were of the utmost importance to all countries of the world. She was sure that delegates to the World Health Assembly would be pleased to ta 长 e part in discussions on any of them. However,the discussions of the Board had revealed some salient points, making it clear that some subjects were of outstanding importance, and especially the challenge to public health of urbanization. She agreed with

8 *EB36/Min/3 Rev.l Dï» Abdulhadi that the question of the quality control of pharmaceutical preparations must not be lost sight of, A similar subject had been suggested for recent Health Assemblies; she believed that the Indian delegation had first put it forward in But perhaps the time was not yet quite ripe for its discussion. If not, the resolutions that had been adopted on the subject over the last three years must be borne in mind, and provision should be made for its discussion soon. Meanwhile, she associated herself with those in favour of the third subject, and in particular with Dr Watt's wording, which made it clear that not only housing but the totality of urbanization should be discussed. In that connexion she proposed a further amendment to replace the words "urban development" by the single word "urbanization" 9 since the latter had perhaps a broader meaning, covering all aspects of the process. Many contributory problems, including sewerage, water supply, and even prostitution had been mentioned; industrialization and. the setting up of factories should be taken into account as aspects of urbanization that made planning difficult, together with the problem of people arriving in cities as refugees or as migrants from rural areàs and causing unplanned growth, with its accompanying health problems Professor MUNTENDAM said that he had changed his mind since the previous meeting, and wished to associate himself with those in favour of technical discussions on the challenge to public health of urbanization, as that was the most multidisciplinary subject.

10 EB^6/Mín/^ Rev.l The DIRECTOR-GENERAL said that Dr Carlos Luis Gonzalez had been born in Venezuela, and was forty-eight years of age. at the Central University of Venezuela in 1938c He had received his medical degree He had devoted all his professional career to public health, health unit in Venezuela,; HG had been successively Chief Medical Officer of the Director of Training Courses for Health Officers at the Ministry of Health; Director of Public Health of Venezuela, and Minister of Health. He had represented Venezuela nt. a number of international health conferences including several World Health Assemblies He had also been a member of the Executive Board of the World Health Organisation. Between 19^6 and 19 斗 Dr Gonzalez had taken the degrees of Master and Doctor in Public Health at the Johns Hopkins School of Public Health. His career in international health had developed between 195 斗 and i960 at the Pan American Sanitary Bureau/V/HO Regional Office for the Americas where he was head of the Division of Public Health and later Assistant Director of the Pan American Sanitary Bureau/ Deputy Regional Director of the WHO Regional Office for the Americas In recent years, Dr Gonzalez had been consultant to WHO for the Study Group on the Integration of Mass Campaigns against Specific Diseases into General Health Services к He had also served as a consultant for the malaria pre-eradication programmes д particularly in relation to the organization of the minimum health services required for the support of a malaria eradication programme. In that connexion, he had served as a consultant to the Expert Committee on Malaria in 1962, He had published over thirty papers on various subjects related to public health. He was a member of the American Public Health Association and had been one of its vice-presidents ; he was also a member of the American Society of Tropical

11 Medicine and Hygiene, the American Academy of Political and Social Science and the Venezuelan Society of Public Health. He was at present Professor of Preventive.:":. ;.: J- i w ,. -. : :.;.:!-... 12 EB36/ÍVIin/5 Rev.l REPORT ÓF TIÎS UNICEF/WHO JOINT COMMITTEE ON HEALTH POLICY, FOURTEENTH SESSION: 1 Item of the Agenda (Document ЕВЗб/б) (continued from the second meeting, section 6) The CHAIRMAN reminded the members of the Executive Board that a drafting group, under the chairmanship of Professor Muntendam, had been asked to draft a resolution on the report of the UNICEF/WHO Joint Committee on Health Policy, and invited the chairman of the drafting group to introduce the draft resolution. Professor MUNTENDAM said that the draft resolution drawi up by the group under his chairmanship was. intended to inform the Executive Board of UNICEF at its next meeting in June of the statements made during the. consideration of the Joint Committee's report^ so that the reactions of UNICEF could be known at the next session of the Executive Board of WHO* At the invitation of the Chairman, Dr QUIRÓS, Rapporteur, read the following draft resolution: The Executive Board, :. BY THE DIRECTOR-GENERAL AS AT 30 APRIL I965: Item 4o2 of the Agenda (Document EB36/7) Mr SIEGEL, Assistant Director-General, introducing the report contained in document EB,6/7, said that the Director-General was following the customary procedure of reporting to the Board on the allotments issued under the regular budget, the Expanded Programme of Technical Assistance and the Voluntary Fund for Health Promotion*. Document EB)6/7, which gave the situation as at 30 April I965, would provide the Board with information regarding the way in whioh the Organization 1 s work was being carried out under the Appropriation Resolution for the current fiscal year, The Secretariat would be pleased to provide any further information required Dr KEITA asked why, under the section of the document dealing with Africa, no allotment was listed for the malaria pre-reradication project envisaged in 1965 for Guinea. Mr SIEGEL replied that the document listed only the allotments issued between January and J>0 April 1965 and did not refer to those which might subsequently be issued on the basis of planned programmes He drew attention to page 5 杯 of the document, where three allotments were listed for Guinea.. '...,.- : : ;..; - Dr KEITA said that he : had in fact seen the allotments listed on page but had :.presiomed that they referred to existing projects. What he wished to know was whether or not an allotment had been foreseen for the malaria pre-eradication project in Guinea, which had just been established.

14 EB36/Min/3 Rev.l Mr SIEGEL explained further that any programmes approved after JO April 1965 would be covered by an allotment which would be issued subsequently Dr KEITA said that he was still not satisfied with the reply but would consult Mr Siegel privately on the matter later In the absence of any further comment, and at the request of the CHAIRMAN, Dr HAPPI, Rapporteur, read out the following draft resolution: The Executive Board NOTES the report of the Director-General on the allotments issued under the regular budget, the Expanded Programme of Technical Assistance and the Voluntary Fund for Health Promotion, as at JO April 1965* Decision: The draft resolution was adopted. 1 CURRENCY OF PAYMENT OF CONTRIBUTIONS: (Document ЕВЗб/ll) Supplementary Item 1 of the Agenda Mr SIEGEL, Assistant Director-General, introducing document EBJ6/11, said that there had not been sufficient time to submit a fuller. report to the Board on the question of the currency of payment of contributions. He drew attention to Annex 1 ñ of the document, which contained a copy of a letter received from *the delegation of the United Arab Republic to the Eighteenth World Health Assembly concerning the possibility of making contributions in national currencies; and to Annex 2, which contained relevant extracts from the minutes of the discussion on the matter in the 1 Resolution EB36.R20.

15 一 8l - -EB36/Min/3 Rev.l Committee on Administration,: Finance and Legal Matters e Paragraph : 3 on page 1 of document Евзб/и referred to previous resolutions which had been adopted by the Health Assembly and the Executive Board in that connexion, and which could be found in the Handbook of Resolutions and Decisions, He pointed out that the acceptance of a wide variety of currencies for the payment of contributions would create difficulties for the Organization in maintaining efficient operations That had been proved by experience gained from the Expanded Programme of Technical Assistance, contributions to which were made in many different currencies. The Director-General therefore proposed, if the Board so desired, to submit a more comprehensive report on the subject to its thirty-seventh session in January Such a report would also contain a reference to the Financial Regulations of the Organization, by which the Director-General was governed in dealing with the question of currency of payment of contribution. Dr WATT agreed that, since the; matter was so complex, it required further '.. :: 1. :.,.:!! : :....:... consideration before a fair decision could be reached. There was one aspect, however> which he wished to ask the Director-General to include in his study and that was the cost that would be incurred in terms of administration and personnel. He wished to know what exactly was involved and how such additional costs would be met.,::.: У Professor MACUCH recalled that the question of currency of payment of contributions had already been discussed on several occasions and that the Committee on Administration, Finance and Legal Matters at the Eighteenth World

16 ЕВЗб/Mln/) Rev.l Health Assembly had been unable to take a decision on the matter, which had been raised toy the Polish delegation since it was not on the agenda It was a matter that was of concern not only to Poland but to a number of countries where the national currency was not acceptable for payment of contributions He therefore wished to express appreciation to the Director-General for placing the item on the agenda of the current session of the Board since, although certain measures had already been taken by the Board and the Health Assembly to alleviate the position of those countries whose currencies were not acceptable, the optimal solution had still not been found. In view of the importance of the subject, he therefore wished to submit for the Board 1 s consideration the following draft resolution: The Executive Board, Taking into account that the steady increase in the regular budget of the Organization creates a considerable burden, particularly for certain Member States; Recalling its resolutions V/HA2»58 and WHA14.52 and resolutions EB28.R28 and EBJ1.R11 concerning payment of contributions in currencies other than United States dollars, Swiss francs or pounds sterling; Bearing in mind that certain agencies of the United Nations accept a portion and even the whole of contributions in local currencies of the Member States, REQUESTS the Director-General: (1) to ask the governments of Member States to communicate to him their views on the whole problem of payment of contributions in currencies other than United States dollars, Swiss francs or pounds sterling; (2) to continue to study this problem with a view to extending possibilities of payment of contributions in local currencies other than those now authorized, taking into accoxmt replies of governments; and (3) to report the results of such a study to the thirty-seventh session of the Executive Board and afterwards to the Nineteenth World Health Assembly.

17 -83 - EB36/Min/5'.RQv:.l Dr HAPPI asked whether, in fact, the only currencies accepted by the Organization were United States dollars, Swiss francs and pounds sterling, as stated in the. i.. Л "Г: : :. :. 卞 Л '!. ;. :.. :, document before the Board The CHAIRMAN considered that, in view of the complexity of. the question, and. pf its importance to the: majority of countries, the Director-General should be given the opportunity to study the matter further and to report back to the thirty-seventh session of.;the Board with all re 1 evant toaekground material * If he; had understood, aright, that was also the intention of Professor Macuch in proposing, his draft resolution. Dr QUIROS agreed with the view of the Chairman : the Director-General was the person most qualified to give an opinion on the matter. / Professor GERIC said that, although he fully, appreciated the difficulties which would arise for the Organization if payments were made in national currencies, the difficulties of the countries in question should also be borne in mind. At the :. _ " 1 ". : Board's previous session, he had suggested that the problem might be partially resolved if some of the equipment needed for WHO projects in countries experiencing currency.difficulties could be purchased in. those countries. The Secretariat should be requested to. propose some mutually acceptable solution to the problem. He was of the opinion that,the matter should not be discussed at that stage by the Board and supported the proposal that it should be deferred until the Board's session, when fuller documentation would be available. thirty-seventh

18 ЕВ5б/М1п/з Rev.l Dr SUBANDRIO said that it was her understanding that Professor Macuch had, in submitting his draft resolution, made a concrete proposal that the Director-General should ask governments to communicate their views to him on the whole problem of payment of contributions and that the Board should then re-examine the matter at its thirty-seventh session. While in the European Region the Organization f s work was mainly confined to such fields as education and training and the award of fellowships, in the other regions WHO maintained large offices and appointed representatives, consultants and advisers, all of whom had to be paid in United States dollars, pounds sterling or Swiss francs. It might be possible for a part of WHO'S expenses in respect of salaries at such offices to be contributed by the government concerned in local currency The work involved would only be a simple matter of converting dollars into loacl currency and should not entail the establishment of any additional administrative machinery. Her suggestion, of course, applied only to those countries where the Organization incurred expenses in local currency. The CHAIRMAN observed that the consensus of opinion seemed to be that the matter should be referred to the thirty-seventh session of the Board. Mr TARCICI said that he well understood that the payment of contributions in local currencies could give rise to difficulties for the Organization - but they could be overcome, whereas the difficulties experienced by certain countries in paying their full contribution were virtually unsurmountable. However, he had full confidence in the Organization 1 s administration to approach the problem with an understanding of

19 -85 - EB^6/ÍVlin/5 Rev.l those difficulties, and he trusted that the study to be.undertaken and submitted to the Board at its thirty-seventh session would' be carried out in the same spirit, of comprehension.. j... Mr BRAVO-CARO, alternate to Dr Martínez, said that the question of the currency in which contributions should be paid had been debated at length by the United Nations committee established to deal with the matter. WHO might obtain some useful information from the discussions held in that committee and. the conclusions reached might well serve as a: guide in. finding a solution to the - problem. :...'.;... *..; ; : ; Mr SIEGEL said that, if Professor Macuch wished his draft resolution to be considered at the current session of the Board, the Secretariat would like first to point out certain inaccuracies in-. s ; ome of the ref erences. For example, the suggestion that other agencies in the United Nations system accepted a portion, or even the whole, of contributions in local currencies was incorrect. To his knowledge, no international organization making statutory assessments accepted payment in local currencies, which were accepted only for voluntary contributions such as those made to the Expanded Programme of Technical Assistance or the United Nations Special Fund But local currency was also accepted for contributions to the WHO Voluntary Furid for Health Promotion: as he had already stated in the Committee on Administration 9 Finance and Legal Matters, he did not recall any instance where the Director-General had failed to accept a payment in a national currency for a contribution to the Volmtary Fund for Health Promotion,

20 EB36/Min/3 Rev.l As far as the regular budget was concerned, however, for which there were statutory assessments, it would clearly be a very difficult procedure indeed, and one which would make the Organization 1 s operation virtually impossible, if payment of contributions were to be accepted in the local currency of each Member State. The governing provisions in that connexion were to be found in Financial Regulation 5,5, on page 7 斗 of Basic Documents, which read: Annual contributions and advances to the Working Capital Fund shall be assessed in US dollars, and shall be paid in either US dollars or Swiss francs; provided that payment of the whole or part of these contributions may be made in such other currency or currencies as the Director-General, in consultation with the Board, shall have determined. With reference to the question raised by Dr Happi, he said that, in accordance with a resolution 1 adopted by the Board at its thirty-first session, eight local currencies were now accepted by the Organization, since it had been found possible despite an additional "burden in terms of cost and time, to accept the currencies of those countries where regional offices were located. Another principle followed by the United Nations and adopted by V/HO was that of fair treatment for all Member States so that, if the Organization agreed to accept payment in the local currency of one Member State, then all other Member States would also be given the opportunity of paying a portion of their contributions in that local currency» That principle, which had been carefully applied since the Organization's inception, was apparently now being queried - although not expressly so - in the letter dated 11 May 1965 from the delegation of the United Arab Republic (Annex 1 to document ЕВЗб/ll) It might therefore be necessary to consider reverting to the original arrangement, whereby contributions were accepted only in United States dollars or Swiss francs 1 Resolution EB31-RH.

Professor KAREFA-SMART asked whether the phrase " teaching and laboratory equipment" was intended to include essential reference books. She was very much in favour of voluntary funds, but there was a tendency to neglect communicable disease control in favour of medical research: from the figures she had quoted it would be seen that the contributions received for the Special Account for Medical Research had been six times those for the Malaria Eradication Special Account. In his opening address to the Health Assembly the Director-General had said that the campaign against communicable diseases was the Organization 1 s first concern. It was important to bring home its importance to donors of voluntary contributions. Paragraph», of the report stated that, while no programmes had been projected under the Special Account for Smallpox Eradication, the Special Account for Accelerated Assistance to Newly Independent and Emerging States, and thé Special Account for Assistance to the Democratic Republic of the Congo, there continued to be a need for increased assistance from the Organization for those purposes. She had been encouraged to note during a plenary meeting of the Health Assembly the statement by the delegate of the United States of America to the effect that President Johnson was initiating a fund for smallpox eradication The DIRSCT'OR-GENEML pointed out that the figure of $ for contributions received for the Special Account for Medical Research was not related in any- way to the proposal for the creation of a world health research centre. It would be seen from Annex 1 of the report that grants under that special account had been made in

Such contributions were a good investment in fields of research directly related to the control of communicable diseases. Dr SUBANDRIO said that she had quite understood that the figure she had quoted was not related to the proposed world health research centre It was nevertheless a research account, and it did seem to her that research was becoming the prima donna among health activities / Dr QUIROS said that the idea of establishing world health foundations was a very good one provided that sufficient interest and enthusiasm were shown in them. In his country a private foundation existed that was willing to offer help to Tenderdeveloped countries, particularly in the public health field. While the proposal should be. supported, care should be taken in setting up such foundations to ensure that they worked successfully* / Professor GERIC thanked the Director-General for the efforts he had made towards the strengthening of the Voluntary Fund for Health Promotion and the setting up of further national world health foundations, which he hoped would meet with success.
