1 On TUESDAY, SEPTEMBER 25, 2018, the CONSTRUCTION COMMITTEE of the Greater Orlando Aviation Authority met in Conference Room Apollo/Gemini/Taurus, 5855 Cargo Road, Orlando International Airport, Orlando, Florida. As of January 16, 2013, lobbying any Aviation Authority Staff who are members of any committee responsible for ranking Proposals, Letters of Interest, Statements of Qualifications or Bids and thereafter forwarding those recommendations to the Board and/or Board Members is prohibited from the time that a Request for Proposals, Request for Letters of Interests, Request for Qualifications or Request for Bids is released to the time that the Board makes an award. As adopted by the Board on September 19, 2012, lobbyists are now required to sign-in at the Aviation Authority offices prior to any meetings with Staff or Board members. In the event a lobbyist meets with or otherwise communicates with Staff or a Board member at a location other than the Aviation Authority offices, the lobbyist shall file a Notice of Lobbying (Form 4) detailing each instance of lobbying to the Aviation Authority within 7 calendar days of such lobbying. As of January 16, 2013, lobbyists will also provide a notice to the Aviation Authority when meeting with the Mayor of the City of Orlando or the Mayor of Orange County at their offices. CHANGE ORDER AGENDA 1. The following Change Orders were presented for consideration: A. Change Order BP VARIOUS REVISIONS. This change order to the contract with Hensel Phelps Construction Company for Airside 4 Improvements Program - FIS/CBP Renovation and Wing Expansion is to: 1. (a) Provide all labor, materials and equipment to temporarily relocate power and data for four each Global Entry kiosks to the temporary demising wall in the center of the FIS queue space and relocate a CBP temporary core switch and MDF equipment from CBP Communications Room AS04 HUB to AS04 HUB (Reference Contractor's CE Nos. 638 and 809, and RFI No. 1189, Owner Directed). ADD: 314,286.00; Calibration) and, (b) Reduce Allowance K (TSA and CBP Equipment Move with by (52,379.00), from $2, to $0.00, to offset the cost associated with Item No. la above. DEDUCT: ($2,379.00). NET AMOUNT FOR ITEM NO. 1: ADD: $11,907.00; 2. Provide all materials to furnish two each security level supply air grilles (Type J) and one each security level return air grille (Type K) for Room AS04 HUB (Reference Contractor's CE No. 719 and ASI No. 082). ADD: $2,883.00; 3. Provide all labor, materials and equipment to provide power and data poles for four each temporary CBP podiums in front of the processing booths near the CBP Command Center. (Reference Contractor's CE No. 761-R1, Owner Requested). ADD: $3,013.00; 4. Provide all materials to install approximately 108 SY of carpet planking in Rooms AS04 HUB A and AS04 HUB B. (Reference Contractor's CE No. 771 and RFI No. 980). ADD: $3,359.00; 5. Provide all labor, materials and equipment to install six each Cat6 data cables from Room AS04 HUB to AS04 HUB , and AS04 HUB to AS04 HUB (Reference Contractor's CE No. 790 and RFI

Provide all labor, materials and equipment to install three each cameras and three each microphones in Room AS04 HUB , and two each remote door release switches to Restrooms AS04 HUB and AS04 HUB (Reference Contractor's CE Nos. 748 and 836, and RFI Nos and 1101). ADD: $10,529.00; and, 9.Provide all labor, materials and equipment to fabricate and install 24 each Cat6 patch cords for modular furniture installed in Rooms AS04 HUB , AS04 HUB , and AS04 HUB (Reference Contractor's CE No. 835 and RFI No. 1350). ADD: $1, Upon motion of Mr. Patterson, second by Ms. Schneider, vote carried to approve Change Order BP , for a total amount of $41, and no time extension, with funding from Passenger Facility Charges, General Airport Revenue Bonds, and Aviation Authority Funds. B. Change Order BP VARIOUS REVISIONS. This change order to the contract with Hensel Phelps Construction Company for Airside 4 Improvements Program - FIS/CBP Renovation and Wing Expansion is to: 1. (a) Provide all labor, materials and equipment to install CMU wall in AS04 HUB D. This work includes 487 blocks of CMU, associated reinforcement and additional framing at new rooms. (Reference Contractor's CE No. 496 and RFI No. 1002). ADD: $12,961.00; and, (b) Credit for Wall Type C/G framing not installed. DEDUCT: ($3,630.00). NET AMOUNT FOR ITEM NO. 1: ADD: $9,331.00; 2. Provide all labor, materials and equipment to patch and paint wall at AS04 HUB This work includes 4 yards of concrete grout, and painting of 34 SF of wall. (Reference Contractor's CE No. 537 and RFI No. 1105). ADD: $902.00; 3. Provide all labor, materials and equipment to re-route three 2-inch fire lines. This cost includes 84 LF of Schedule 40 2-inch pipe, 21 LF of Schedule 10 4-inch pipe, joints, fittings, and caps. (Reference Contractor's CE No. 593, Owner Directive). ADD: $7,286.00; 4. Provide all labor, materials and equipment to provide access control to Passenger Elevator P-55, and Fire Alarm/Elevator programming for fire recall at Passenger Elevators P-54, P-55, and P-56. This cost includes two card readers, one DIB, and 450 LF of 3/4-inch EMT conduit. (Reference Contractor's CE No. 667 and PR No. 109). ADD: $6,175.00; 5. Provide all labor, materials and equipment to procure inch Low Loss 50 OHM Coaxial Cables. This cost includes purchase of 100 piece lot. (Reference Contractor's CE No. 804, Owner Directive). ADD: $437.00; 6. Provide all labor, materials and equipment to relocate paper towel dispensers and associated power receptacles in AS04 HUB and AS04 HUB This cost includes 40 LF of 3/4-inch EMT conduit, and 150 LF of #10THHN solid wire. (Reference Contractor's CE No. 840 and RFI No. 1187). ADD: $560.00; and, 7. Provide all labor, materials and equipment to replace overhead brace rails in Rooms AS04 HUB , AS04 HUB , AS04 HUB and AS04 HUB , with full height pilasters. In addition, one fire alarm smoke detector was relocated in AS04 HUB This cost includes new pilasters, drywall work, and painting. (Reference Contractor's CE No. 244 and PR No. 086). ADD: $5, Upon motion of Mr. Hunt, second by Ms. Schneider, vote carried to approve Change Order BP , for a total amount of $30, and no time extension, with funding from Passenger Facility Charges, General Airport Revenue Bonds, and Aviation Authority Funds.

[A handout was presented, which included a revised Coversheet.] This change order to the contract with Hensel Phelps Construction Company for Airside 4 Improvements Program FIS/CBP Renovation and Wing Expansion is to: 1. (a) Provide all labor, materials and equipment to install a 2-hour GWB 4-feet-4-inches high to deck above existing masonry {Rooms 3542 east wall, 3563 west wall, and 3584 east wall, total of 89 ft.). Fill cavity with mineral wool blanket in accordance with detail and notes. {Reference Contractor's CE No. 220 and PR No. 80). ADD: $10,005.52; and, (b) Provide all labor, materials and equipment to provide a credit for not having to demolish the existing slab and add new footings/concrete pour back. (Reference Contractor's CE No. 220 and PR No. 80). DEDUCT: ($34,128.52). NET AMOUNT FOR ITEM NO. 1: DEDUCT: {$24, ); 2. Provide all labor, materials and equipment to provide a fully functional microphone system by installing cabling from jetway door to gate counter, and 4 temporary microphones for Gates 90, 92, 94 and 96. (Reference Contractor's CE No. 750 and RFI No. 1225). ADD: $4,784.00; and, 3. Provide all labor, materials and equipment to add an electrical circuit for Sign W2-C at the Gate 90 vestibule. ADD: $ Upon motion of Mr. Patterson, second by Mr. Hunt, vote carried to approve Change Order BP , for a total credit amount of ($18,911.00) and no time extension, with funding credited to Passenger Facility Charges, General Airport Revenue Bonds, and Aviation Authority Funds. D. Change Order BP INTERIM QUANTITY ADJUSTMENTS (BASE BID). [A handout was presented, which included a revised Coversheet and Supporting Documentation. ] This change order to the contract with The Middlesex Corporation for Loop Road Resurfacing and Related Work is to provide all labor, material, and equipment to perform the work described in the following Base Bid quantity adjustments: (a) Increase Pay Item (Milling Existing Asphalt Pavement inches) by 781 SY, from 30,028 SY to 30,809 SY at the contract unit price of $1.50/SY. ADD: $1,171.50; (b) Decrease Pay Item (Milling Existing Asphalt Pavement 1.5-inches) by 312 SY, from 1,402 SY to 1,090 SY at the contract unit price of $1.50/SY. DEDUCT: ($468.00); (c) Increase Pay Item (Delineator, Flexible Tubular) by 125 EA, from 52 EA to 177 EA at the contract unit price of $65.00/EA. ADD: $8,125.00; (d) Increase Pay Item (Thermoplastic, Standard, Yellow, Solid, 18- inches for Diagonal or Chevron) by 238 LF, from 540 LF to 778 LF at the contract unit price of S3.50/LF. DEDUCT: ($1,540.00); (b) Increase Pay Item (New 45-foot Concrete Pole, 10-foot Arm and Type A LED Luminaire) by 1 EA, from 20 EA to 21 EA at the contract unit price of $4,500.00/EA. ADD: $4, ; (c) Decrease Pay Item (Relocate 45- foot Concrete Pole, 15-foot Arm and Replace Luminaire with New Type A LED) by 1 EA, from 5 EA to 4 EA at the contract unit price of $ 3, /EA. ADD: $1, Upon motion of Ms. Schneider, second by Mr. Hunt, vote carried to approve Change Order BP , for a total amount of $ and no time extension, with funding from Capital Expenditure Fund, Improvement and Development Fund, and General Airport Revenue Bonds. G. Change Order BP VARIOUS REVISIONS. This change order to the contract with Carr & Collier, Inc. for South Cell Lot and Utilities is to: 1. Provide for all labor, equipment and materials to modify an existing traffic-rated manhole lid, provide additional asphalt milling and the required survey time to layout the north portion of Vertical Drive and Diagonal Way intersection in accordance with DSI NO. 03 instructions. ADD: $15,561.04; 2. Provide all labor, equipment and materials to revise the proposed grades along Diagonal Way and Vertical Drive. The Pay Item quantities are revised as follows: (a) Increase Pay Item Line 9, Embankment, by 65 CY, from 23,500 CY to 23,565 CY at the contract unit price of $21.00/CY. ADD: $1,365.00; (b) Increase Pay Item Line 10, 12-inch Type B Stabilization LBR 40, by 86 SY, from 12,800 SY to 12,886 SY at the contract unit price of $9.00/SY. ADD: $774.00; (c) Increase Pay Item Line 12, 8-inch Lime rock Base Material LBR 100, by 932 SY, from 4,400 SY to 5, 332 SY at the contract unit price of $17.25/SY. ADD: $16,077.00; (d) Increase pay item Line 14, 1.5-inch Type SP12.5 Traffic Level A, by 162 TN, from 680 TN to 842 TN at the contract unit price of $140.00/Ton. ADD: $22,680.00; (e) Increase Pay Item Line 27, Type F Curb and Gutter, by 260 LF, from 2,500 LF to 2,760 LF at the contract unit price of $20.00/LF. ADD: $1, NET AMOUNT FOR ITEM NO. 2: ADD: $47,372.00; and, 3. Additional portion of cost above the contract Pay Item 14 for added 1.5-inch Type 12.5 Traffic-Rated Level A Asphalt, by 162 TN at the contract unit price of $ 15.00/TN. ADD: $2,

6 Page 6 of 22 Mr. Patterson asked if this was the final change order. Mr. Brown replied no, that there is still some minor signage work to be completed. Upon motion of Mr. Hunt, second by Ms. Schneider, vote carried to approve Change Order BP , for a total amount of $65, and no time extension, with funding from Capital Expenditure Fund and Customer Facility Charges. H. Change Order V INCREASE DOOR SIZE. This change order to the contract with R.L. Burns, Inc. for Landside Level 2, A-Side Bag Make-Up Door Replacement (Design/Build) is to provide all labor, materials and equipment to increase the new door size to a 4-feet wide opening, from the existing 3-feet wide opening (3 each) at the A-Side Bag Make-up doors. Contractor shall install new 4-feet wide metal fire-rated doors and door frames, in accordance with the drawings and specifications, dated August 14, ADD: $1, Mr. Patterson asked if there was additional work on this project, replied no. Mr. Graham Upon motion of Mr. Patterson, second by Mr. Hunt, vote carried to approve Change Order V , for a total amount of $1, and no time extension, with funding from Capital Expenditure Fund. I. Construction Change Directive BP HURRICANE IRMA PREPARATION/EMERGENCY RESPONSE. This construction change directive to the contract with PCL Construction Services, Inc. for Ticket Lobby Program Main Ticket Lobby Modifications is to pay all costs allowable under the Contract Documents for the preparation, first 48 hours emergency response, and permanent recovery repairs and damage associated with Hurricane Irma. This Construction Change Directive shall resolve all claims arising out of or associated with Hurricane Irma. (Reference Contractor's PCL CRX Nos. 332, 332.1, 332.2, and 332.4). ADD: $177, Mr. Patterson stated that the Contractor has already been paid for this emergency preparation/response work through the Emergency Purchase Authorization process. This construction change directive adds this scope and cost into the BP project. Upon motion of Mr. Patterson, second by Mr. Hunt, vote carried to approve Construction Change Directive BP , for a total amount of $177, and no time extension, with funding from Capital Expenditure Fund (ZC-00241) to be reimbursed by Insurance and FEMA to the extent eligible. CONSTRUCTION MANAGEMENT AT RISK (CM@R) GUARANTEED MAXIMUM PRICE (GMP) BUDGET, BUYOUT, AND CONTINGENCY MANAGEMENT REQUESTS AGENDA 2. The following GMP Budget, Buyout, and Contingency Management requests (related to the South Terminal C, Phase One Project) were presented for consideration: A. Subcontractor Award for BP-S00142 (GMP No. 6-S) with Professional Service Industries, Inc, for 3rd Party Waterproofing Inspection. Mr. Guirges presented the memorandum, requesting approval for Hensel Phelps Construction (HPC) to issue a Subcontractor Award on BP-S00142, South Terminal C, Phase 1 Early Foundations (GMP No. 6-S), at the Orlando International Airport, in the amount of $4,750.00, to Professional Service Industries, Inc., for 3rd Party Waterproofing Inspection. Hensel Phelps Construction is requesting to issue a subcontract award to Professional Service Industries, Inc. for third-party waterproofing inspection services for the Adcor swell seal water stop and associated work within BP-S142 (GMP-65) in the not-to-exceed amount of $4,

7 Page 7 of 22 Hensel Phelps Construction direct negotiated with Professional Services, Industries, Inc. The Subcontractor terms and conditions do not apply to the Owner's contract with the CM. Professional Service Industries, Inc. is a qualified third-party inspection services company certified to inspect Grace Construction Products. In awarding this third-party inspection through Professional Service Industries, Inc., the manufacturer warranty for the overall foundation waterproofing system will be provided as the Adcor swell seal water stop is considered an accessory product for the waterproofing system by Grace Construction Products. The Adcor swell seal water stop is installed by the concrete subcontractor, not the waterproofing subcontractor. Due to how this scope of work was purchased there is a scope gap in the inspection process to provide the manufacture warranty of the waterproofing system. The funding for this scope of work will come from GMP No. 6-S line item Unbought Scope US03 Waterproofing for a not to exceed amount of $4,750. The effect of this subcontract modification for this project is as follows: HPC to issue subcontract award to Professional Service Industries, Inc. in the not-to-exceed amount of: GMP line item(s) value: Deduct US03 Waterproofing: Remaining GMP line item(s) value: $ 4, $ $ (4,750.00) $ Hensel Phelps Construction is committed to 33% MWBE and 5% LDB for Construction Management Services and 20% MWBE and 4% LDB participation for Construction Services. The subcontractor identified and committed to participate on BP- S00142 (GMP No. 6-S) for this subcontract award is Professional Service Industries, Inc. (Non-MWBE/LDB/VBE). There is no small business participation associate with the proposed award. Ms. Schneider asked for clarification of the handwritten statement on the GMP BBC Form, which reads, "The subcontractor terms and conditions do not apply to the owner's contract with the CM". Legal Counsel stated that some of the Terms and Conditions detailed in the supporting documentation are not standard for the Aviation Authority and do not apply. Upon motion of Ms. Schneider, second by Mr. Hunt, vote carried to approve Hensel Phelps Construction (HPC) to issue a Subcontractor Award on BP-S00142, South Terminal C, Phase 1 Early Foundations (GMP No. 6-S), for the total not-to-exceed amount of $4,750.00, with funding from General Airport Revenue Bonds and Passenger Facility Charges. SOUTH TERMINAL C, PHASE 1 REQUEST FOR RECOMMENDATION OF APPROVAL OF AN ADDENDUM TO THE ARCHITECT OF RECORD SERVICES FOR THE SOUTH TERMINAL C, PHASE 1 AGREEMENT WITH HNTB CORPORATION FOR DESIGN SERVICES FOR WILEY DRIVE INTERSECTION REALIGNMENT & IMPROVEMENTS FOR W-S00110, SOUTH TERMINAL C, PHASE 1 - ARCHITECT OF RECORD (AND MAJOR SUBCONSULTANTS), AT THE ORLANDO INTERNATIONAL AIRPORT. 3.
