3 Program Participant Gidelines All program participants are reqired to register for the ASCRS Symposim ASOA Congress. Sbmissions to the symposim and congress mst be via the online sbmission process at or All sbmissions are sbject to the ACCME gidelines regarding validation of clinical content. Athors/instrctors/prodcers are responsible for ensring that (1) all recommendations involving clinical medicine presented are based on evidence that is accepted within the profession and (2) all scientific research referred to, reported, or sed conforms to the generally accepted standards of experimental design, data collection, and analysis. Recommendations, treatments, or manners of practicing medicine that do not comply with the above, are known to have risks or dangers that otweigh the benefits, or are known to be ineffective in the treatment of patients shold not be sbmitted or presented. Material in any ASCRS presentation mst not be presented or sbmitted for pblication elsewhere. In making an ASCRS ASOA sbmission, yo are agreeing to the terms of the Copyright Assignment Statement on page 4. Sbmission Process Online sbmission is open from Agst 15 to September 25, Step 1: Lead presenters, instrctors, prodcers, co-instrctors, and faclty mst complete the Faclty CME Review (see page 5). The review mst be completed by September 25, Step 2: All participants presenters, coathors, instrctors, co-instrctors, faclty, prodcers, and coprodcers mst disclose their financial interests. The financial interest disclosre instrctions are on page 5. The disclosre mst be completed by September 25, Step 3: To sbmit a paper, corse, poster, or film, click here: Provide all reqired information. The process will allow yo to review yor sbmission. It is important to se a valid address as all correspondence regarding the accont and/or sbmission will be sent to this address. Step 4: Use yor browser print command to print a copy of yor sbmission for yor records. If yo do not receive a completed sbmission confirmation ID, please contact s at sbmissions@ascrs.org or sbmissions@asoa.org. Incomplete sbmissions will not be processed. 4 Program Participant Gidelines Gidelines for Nonacceptance 1. Sbmissions that are commercially biased will not be accepted. 2. If the athor/instrctor/prodcer did not present his/her schedled presentation the previos year and did not follow the withdrawal gidelines, the sbmission will not be accepted. 3. If the sbmission information has been pblished or presented elsewhere, the sbmission may not be accepted. 4. If the lead athor/instrctor/prodcer and all coathors/co-prodcers/coinstrctors have not provided the Financial Interest Disclosre by September 25, the sbmission will not be accepted. 5. If the lead athor/instrctor/prodcer and all corse co-instrctors/faclty have not sccessflly completed the Faclty CME Review by September 25, the sbmission will not be accepted. 5 Faclty CME Review/Financial Interest Faclty CME Review Program To ensre the highest qality programming and compliance with the Accreditation Concil on Contining Medical Edcation (ACCME) reqirements for CME activities, ASCRS has created a mandatory CME review program for all participants in the ASCRS Symposim and ASOA Congress. The Faclty CME Review Program consists of an online 15-minte presentation and a short post-test. The presentation and test will be available beginning Agst 15, 2013, at Financial Interest Disclosre Reqirements As a sponsor accredited by the ACCME, ASCRS ASOA mst ensre balance, independence, objectivity, and scientific rigor in all its individally or jointly sponsored activities. All individals participating in a sponsored activity mst disclose any financial interest or relationship with the manfactrer(s) of any commercial prodct(s) and/ or provider(s) of commercial services discssed in an edcational presentation or the lack thereof. Financial interest can inclde sch things as grants or research spport, employee, consltant, major stockholder, member of speaker s brea, financial relationships held by spose/partner, etc. The intent of the financial interest disclosre is to provide the Program Committee with information so they can design and implement a balanced, independent, and scientific edcational activity. The Financial Interest Index in the Final Program provides information to attendees so they can make their own jdgment regarding the interest or relationship and the materials presented. 5 Potential participants in ASCRS-sponsored CME programs, inclding coathors/ coprodcers/co-instrctors and faclty, mst provide a complete listing of ALL financial relationships relevant to ophthalmology not jst those related to a specific talk in the financial interest disclosre database. Any relevant relationship that occrred within the previos 12 months shold be reported. Those who have no financial interests to disclose shold indicate none in the database. Disclosre information will be kept on file and sed dring the calendar year in which it was collected for all CME-bearing activities. To sbmit for the Symposim and Congress, yo mst complete the information for this database. All lead presenters mst notify all coathors/ coprodcers/co-instrctors and faclty that they mst enter their financial interest information prior to sbmitting an abstract. 6 Credits Contining Medical Edcation Accreditation Statement The American Society of Cataract and Refractive Srgery is accredited by the Accreditation Concil for Contining Medical Edcation to provide contining medical edcation for physicians. ASCRS takes responsibility for the content, qality, and scientific integrity of the CME activity. Objectives Ophthalmologists attending the ASCRS Symposim on Cataract, IOL and Refractive Srgery and the ASOA Congress on Ophthalmic Practice Management will have the opportnity to Smmarize, assess, analyze, and evalate the latest developments for the management of mltiple oclar conditions within the varios ophthalmic specialties Formlate crrent and advanced treatment strategies with oclar conditions, inclding anterior segment and refractive srgery, glacoma, and retinal conditions Identify complicating factors in anterior segment srgery and develop appropriate treatment plans to address the factors pre-, peri- and postoperatively Compare and contrast ophthalmic pharmaceticals and devices Integrate prescribed medical and srgical treatment options into everyday practice Examine external legislative and internal practice isses affecting the field of ophthalmology Designation Statement The American Society of Cataract and Refractive Srgery designated this edcational activity for a maximm of 36 AMA PRA Category 1 Credits. 8 ASCRS Corse Sbmission: Instrctions 8 DEADLINE: SEPTEMBER 25, 2013 ELECTRONIC HANDOUT DEADLINE: APRIL 11, 2014 Limit: 1 corse sbmission per instrctor Corse Presentations: All corses will be 90 mintes. Potential faclty are reminded to design corses that are scientifically sond and nbiased. If the proposed corse has special reqirements, please contact s at corses@ascrs.org. If the proposed corse will be condcted in a langage other than English, an English translation of the title, corse description, and objectives mst also be sbmitted. Peer Review: All corse sbmissions will be peer-reviewed by the ASCRS Program Committee and appropriate clinical committees. Sbmissions will be graded on content and scientific integrity. Sbmissions by non-mds/dos will be reviewed in the context of the presenter s training and edcation. Responsibilities of the Lead Instrctor: Ensre that all participants in the corse complete the Faclty CME Review Program by September 25, 2013 (see page 5). If entering co-instrctors/faclty, confirm the address they sed when sbmitting their financial interest disclosre to be able to inclde them with the electronic sbmission. Prior to sbmitting, notify all co-instrctors and faclty that they mst enter their financial interest information into the financial interest disclosre database. In accordance with the ACCME s pdated reqirements, faclty who have not done so will not be allowed to participate in the corse and will not be listed in the Final Program. See page 5 for details. In sbmitting a corse, the instrctor attests that he/she has reviewed each faclty member s financial interest and determined there is no conflict of interest. Notify all faclty of their inclsion in the corse and (if accepted) of the schedled date and time for the corse. Failre to do so means the corse will be canceled. Prepare or coordinate comprehensive handots to be distribted at the corse or sbmitted to ASCRS for posting online. ASCRS will not accept, photocopy, or distribte handot copies. We will, however, accept electronic handots for posting to or web site. Frther instrctions regarding electronic handots will be sent to lead instrctors of accepted corses. Title The title shold accrately and concisely reflect the sbmission content. For CME prposes, prodct/trade names cannot be sed in the title. Generic descriptors are reqired. Titles with prodct/trade names may be rejected. The title shold not be formlated as a qestion or statement (i.e., shold not inclde a verb). Titles may be edited by ASCRS editorial staff. Limit: 140 characters, inclding spaces Category Choose from the categories on page 7. Corse Description The corse description shold inclde 2 components: (1) the topics to be presented and (2) the format for the presentation; i.e., videos, panel discssion, interactive debate. The corse description will be pblished in the Final Program. Begin yor corse description with the words Corse will.... Limit: 50 words/430 characters Edcational Objective The edcational objective shold be a sccinct statement of what the attendee can expect to learn from the corse. Well-written objectives are action oriented and can be observed or measred; i.e., analyze, differentiate, demonstrate, apply. Begin yor objective with the words Attendee will.... Limit: 40 words/250 characters Example Srgical Management of Cataract and Glacoma: Special Emphasis on Cataract, IOL, and IOP Problems in Psedoexfoliation Bradford J. Shingleton, MD Corse Description: Lectre and video-based corse will review the many isses and options associated with cataract srgery in eyes with glacoma. Srgical methods for dealing with cataract, IOL, and IOP complications in psedoexfoliation (PXF) eyes will be highlighted and srgical reslts in over 2000 PXF eyes presented. Edcational Objective: Attendees will be able to identify the mltiple options for srgery in eyes with coexisting cataract and glacoma and will be able to apply varios techniqes and instrmentation in dealing with specific problems common to psedoexfoliation eyes. Handots If yor corse is accepted, yo will be reqired to sbmit an electronic version of yor handot no later than April 11, Handots will be available online. Instrctions and information abot the format for corse handots will be inclded in the presentation acceptance letter if yor sbmission is accepted. The handots mst be received by the deadline or the instrctor assmes responsibility for providing copies on site. 9 ASCRS Paper Sbmission: Instrctions DEADLINE: SEPTEMBER 25, 2013 Limit: 2 paper sbmissions per athor Becase of repetition and time limitations, only 1 sbmission may be accepted. Please sbmit in order of yor preference of acceptance. Paper Presentations: All accepted papers will be limited to 5-minte presentations. Within each session, papers will be clstered into discssion grops, typically 3 to 5 related papers. After each clster, there will be a panel discssion. Peer Review: All paper sbmissions will be peer-reviewed by the ASCRS Program Committee and appropriate clinical committees. Abstracts will be graded on content and scientific integrity. Sbmissions by non-mds/dos will be reviewed in the context of the presenter s training and edcation. Responsibilities of the Lead Athor Prior to sbmitting, the lead athor mst complete the Faclty CME Review and enter their financial interest information into the financial interest disclosre database. Lead athors mst notify coathors to enter their financial interest information into the financial interest disclosre database. Those who have not sbmitted by September 25, 2013, cannot be inclded on the abstract. In sbmitting a paper, the lead athor attests that he/she has reviewed each coathor s financial interest and determined there is no conflict of interest. Strctred Abstract Please read the following instrctions careflly before completing the sbmission: Choose the paper category from the list on page 7. Please ensre yo have listed all contribting athors on the abstract and that the contribting athors have sbmitted crrent financial interest disclosres in the sbmission database before the September 25 deadline. Prpose: Indicate the qestion that the stdy answers or the hypothesis it tests. Do not inclde names or affiliations of athors. Do not inclde sponsorships, grants, etc. Limit: 50 words/350 characters inclding spaces and pnctation Methods: Describe the stdy design, indicating randomization, masking, and whether the data collection was retrospective or prospective. Identify the patients, inclding selection procedres, inclsion criteria, and nmbers. Indicate the intervention procedres and the otcome measres. Limit: 100 words/700 characters inclding spaces and pnctation Reslts: Present the otcomes and measrements. Data shold inclde the level of statistical significance. 10 ASCRS Paper Sbmission: Instrctions Sample Strctred Abstract Prospective Comparison of 1 Accommodating and 2 Mltifocal IOLs: Visal Acity, Refractive Otcome, and Contrast Sensitivity at Postoperative Year 1 Robert Edward T. Ang, MD Prpose: To compare the long-term refractive and visal otcomes among patients implanted bilaterally with 1 of 3 types of presbyopia-correcting intraoclar lenses (IOLs) dring cataract srgery. Methods: This is a prospective, randomized stdy of 73 cataract patients with mean age of 65 years followed for 1 year after bilateral implantation of the Crystalens AO accommodating IOL (Basch & Lomb), the ReSTOR 13 mltifocal IOL (Alcon), or the Tecnis Mltifocal IOL (Abbott Medical Optics) dring phacoemlsification. The main otcome measres were manifest refraction spherical eqivalent (MRSE), high- and low-contrast ncorrected distance (UDVA, 6 m), intermediate (UIVA, 67 cm), and near visal acity (UNVA, 40 cm), and contrast sensitivity. Reslts: At postoperative year 1, the mean MRSE was D for Crystalens, D for ReSTOR, and D for Tecnis. Mean binoclar HCUDVA was 20/20 in the 3 grops. Mean binoclar HCUIVA was 20/20 in the Crystalens and 20/25 in the ReSTOR and Tecnis grops. Mean binoclar HCUNVA was 20/25 in the Crystalens and 20/20 in the ReSTOR and Tecnis grops. Binoclar contrast sensitivity with glare showed Crystalens was better than ReSTOR at 6 cpd (P ) and 12 cpd (P ) and better than Tecnis at 12 cpd (P ). Conclsion: Uncorrected distance vision was similar among the grops. Uncorrected intermediate vision was better with Crystalens than with the mltifocal lenses. Uncorrected near vision was better with the mltifocal lenses. Contrast sensitivity was better with Crystalens than with both mltifocal lenses. 12 ASCRS Poster Sbmission: Instrctions Sample Strctred Abstract Otcomes of IOL Exchanges at Wake Forest Over 6-Year Period Michelle D. Patel, MD Contribting Athor: Pal J. Dickinson, MD Prpose: To report characteristics of intraoclar lens (IOL) exchanges performed over a 6 year period at Wake Forest University and to frther analyze the IOL exchange reslts within sbsets to look for differences in otcomes and complications between grops with different characteristics. Methods: Retrospective chart review of patients ndergoing IOL exchange at Wake Forest between 2005 and Primary otcomes were visal acity and complications. For visal otcome evalation, the following were exclded: patients with 3 months follow p and patients who were not refracted. 124 eyes of 120 patients were inclded. Patients were divided into the following grops: Grop 1 Refractive IOL Exchange (1a 5 Intact capsle, 1b 5 Open capsle), Grop 2 Lens Malposition (2a 5 No PPV reqired, 2b 5 PPV reqired), Grop 3 Lens Malposition casing strctral damage (3a 5 Uveitis-glacoma-hyphema syndrome and/or vitreos hemorrhage, 3b 5 corneal edema). Reslts: For best corrected visal acity, 56.7% improved, 32.8% were nchanged, and 10.4% worsened. There was a 19.4% complication rate (24/124 eyes), with the top 3 being cystoid maclar edema, re-dislocation, and hyphema. There was no statistically significant difference between the 3 sbgrops in terms of worsened visal acity or complications. No patients in grop 1a had worsened visal acity. There was a trend toward statistical significance between the complication rate between grops 1a and 1b (p ). There was no statistically significant difference between grops 2a and 2b (p ) or between 3a and 3b (p ). Conclsion: Most patients had improved/nchanged visal acity. There was a trend toward statistical significance for increased complications for refractive IOL exchanges with an open posterior capsle. It is best to avoid opening the posterior capsle in patients who may reqire a refractive IOL exchange. ASOA welcomes corse sbmissions that promote this goal and meet the edcational needs of attendees, as described in the Practice Management Tracks. Presenting at the Congress provides national recognition and exposre as well as the opportnity to share concerns and ideas with colleages. Extra photographs will be arbitrarily eliminated. Qality of Entries Images that have obvios defects (e.g., poor focs, color imbalance, improper exposre, or significant artifacts) will not be accepted for jdging. If masking redces the artifacts, it is recommended that it be sed. Recent work is encoraged. 18 Ophthalmic Photographers Society Exhibit Sbmission Format An (20.0 cm cm) or (28.0 cm cm) format is encoraged. Photographs in other sizes will be accepted if they can be monted on an (28.0 cm x 35.5 cm) or (40.5 cm cm) display board and still maintain an aesthetic appearance. Mltiple photographic entries are acceptable in the following size combinations only: Two cm cm Two-for cm cm Two-six cm cm Photomontages (mltiple photographs affixed to each other) will not be accepted. A montage mst be re-photographed/digitized, printed, and sbmitted as a single photograph. Monting Monted photographs will not be accepted. Photographs accepted for display will be monted at ASCRS expense. Monting will be determined by ASCRS. All photographs will be trimmed slightly. Identification Make sre yo label yor sbmissions careflly. (Please, no labels on the backs of prints). Please do not se correction flid. Photographs shold be identified on the back, top-center, with a felt-tipped pen. Please print legibly and label the back, top-center, of each photograph with the following information exactly as it appears on the sbmission. Example Clinical photographs mst be titled with diagnosis. The terms normal, probable, or nknown are acceptable. Entries will not be jdged on the basis of an accrate diagnosis. Monochromatic photographs mst be labeled with the wavelength sed. Mltiple photograph entries shold indicate the intended order. Gross specimen photographs shold inclde final magnification with identification or contain a metric scale. Photomicrographs mst contain information on lighting or type of micrography (polarized, phase contrast, scanning electron, etc.) and the final photograph magnification.
