2 SIXTH AMENDMENT TO AGREEMEN T NO. C-1202S0 OF CITY OF LOS ANGELES BETWEEN THE CITY OF LOS ANGELES AND LAMP INC., DBA LAMP COMMUNITY RELATING TO THE HOUSING OPPORTUNITIES FOR PERSONS WITH AIDS (HOPWA) PROGRAM EMERGENCY AND TRANSI1IONAL HOUSING AND SUPPORTIVE SERVICES IN HOUSING THIS SIXTH AMENDMENT to Agreement Number C is made and entered into by and between the City of Los Angeles, a municipal corporaiion, hereinafter referred to as the City and Lamp Inc., dba Lamp Community, a California non-profit corporation, hereinafter referred to as the Contractor. WITNESSETH WHEREAS, the City has contracted with Lamp Inc., dba Lamp Community wherein the Contractor shall provide Emergency and Transitional Housing and Supportive Services in Housing under the HOPWA Program to persons living with HIVAIDS arid their families, said Agreement is C of City Contract, executed on April 4, 2012, which together with all amendments thereto shall hereinafter be referred to as the Agreement; and WHEREAS, Section 707 of the Agreement provides for amendments to the Agreement, and WHEREAS, this Amendment is necessary ana proper to continue this Program and to ensure uninterrupted services; and WHEREAS, the City and the Contractor are desirous of amending the Agreement as authorized by the action of the Los Angeles City Council and Mayor (refer to Council File Number , authorized by City Council on March 27, 2015 and May 5, 2015, and approved by the Mayor cn April 2, 2015 and May 8, 2015, respectively), which authorized the General Manager of the Housing and Community Investment Department to prepare arid execute an amendment to the Agieement for the purpose of: a) extending the term of the Agreement for an additional six (6) months for a new ending date of September 30, 2015; b) adding additional funds in the amount of Two Hundred Ninety- Nine Thousand Six Hundred Seventy-Seven Dollars ($299,677) based on the 41st Program Year Housing and Community Budget Consolidated Plan for the period April 1, 2015 through March 31, 2016, for a new total amount of One Million Nine Hundred Eighty-Two Thousand Four Hundred Sixteen Dollars ($1,982,416L Catalog of Federal Domestic Assistance (CFDA) ; and b) making other changes as are required in connection with the foregoing, all as detailed elsewhere in this Amendment. -1 -
3 NOW THEREFORE, the City and Contractor agree that the Agreement be amended as follows: AMENDMENT 1. Amend Section 102, Representative of the Parties and Seivice Notices, by replacing Interim General Manager with General Manager. 2. Amend Section 201, Time of Performance, to delete end March 31, 2015 and replace it with end September 30, Amend Section 202.A.1, Supportive Services in Emergency, 1 ransitional, and Permanent Housing, subsection 3 Client Service Level by deleting subsections a and b, and replacing it with the following: a. Minimum number of unduplicated clients to be served. 90 1) 30 clients will be in the Contractor s EmergencyTransitional program 2) 15 clients will be clients who reside in the Contractor s permanent housing facility(ies); and 3) 45 clients referred from the Department of Health Services (DHS) with HIVAIDS will be provided HOPWA resource navigation services through the LAMP HOPWA pilot program with DHS. b Average cost per person served: $2, Amend Section 202.B.3, under Client Service Level for Emergency and Transitional, by deleting subsections a and b, and replacing it with the following: a. Number of unduplicated clients to be served: 30 b. Average cost per person served: $2,243,13 5. Amend Section 204.A., under General for Emergency and Transitional Housing, by deleting subsection 1 and replacing it with the following: 1. The Contractor shall provide Emergency and Transitional Housing Services as follows: Emergency Housing Beds: 0 Unduplicaied Emergency Housing Clients: 0 Transitional Housing Beds: 1b Unduplicated Transitional Housing Clients: 30-2-
4 6 Amend Section 205.1, under General for Supportive Services in Housing - Permanent Housing, by deleting the first sentence and replacing with the following: The Contractor will provide supportive services to 15 eligible clients in tne HOPWA Supportive Services in Housing - Permanent Hous'ng. 7 Amend Section A, under General for Supportive Services in Housing - Permanent Housing, by removing the first sentence in subsection A.8. and replacing it with the following: The Contractor will dedicate one full time staff person at the level ot MSW or equivalent with at least three (3) years of experience to provide supportive services to 45 HIV positive clients who are ieferrea through the County Department of Health Services (DHS) Flexible Housing Subsidy Pool. 8. Amend Section 205.2, under General for Supportive Services in Housing - Emergency and Transitional, by deleting the first sentence and replacing with the following: The Contractor will provide supportive services to 30 eligible clients in the IIOPWA Supportive Services in Housing - Emergency ana Transitional Housing program, 9. Amend Section 301, Compensation and Method of Payment, subsection A, by deleting and replacing with the following: The City shall pay to the Contractor as compensation for complete and satisfactory performance of the terms of this Agreement ari amount not to exceed One Million Nine Hundred Eighty-Two Thousard Four Hundred Sixteen Dollars ($1,982,416). This amendment adds Two Hundred Ninety-Nine Thousand Six Hundred Seventy- Seven Dollars ($299,677) Tor a new total of One Million Nine Hundred Eighty-Two Thousaric Four Hundred Sixteen Dollars ($1,982,416).
