Members Present: President Daniel R. Claypool, Commissioner Casey R. Kozlowski and Commissioner Peggy A. Provider Amount Unit rate Ashtabula Regional Home Health Services $0 $0 Country Neighbor Program, Inc. $0 $0 Total: $0 WHEREAS, it has been determined to not proceed with the award for the Homemaker and Personal Care component of the RFP in accordance with the caveat outlined in the proposal, which states the County is under no obligation to award a contract/subgrant agreement as a result of this RFP and reserves the right to not select any Applicant; now THEREFORE, BE IT RESOLVED, By the Board of Commissioners of Ashtabula County, Ohio, that the proposals received for the Homemaker and Personal Care for Seniors Programs be rejected and the Homemaker and Personal Care be re-ordered and received on October 26, 2016 at 3:00p.m. RESOLUTION NUMBER APPROVING AMENDMENTS TO HEALTH AND EMPLOYMENT RELATED TRANSPORTATION CONTRACTS WITH CITY TAXICAB & TRANSFER COMPANY, INC., CONTRACT NO. S ERT AND HRT WHEREAS, Patrick Arcaro, Director of the Ashtabula County Department of Job & Family Services has presented the following Contract Amendments for the approval of the Board, to-wit: EMPLOYMENT RELATED: PROVIDER: CITY TAXICAB & TRANSFER CO. Hills Blvd, Broadview Hts., OH Cost: Not to Exceed, $6, Term: begins October 1, 2016 for 3 years; now Agreement as noted above is approved in accordance with the copy now on file in this office. RESOLUTION NUMBER ORDERING REQUEST FOR QUOTES FOR CONSULTANT SERVICES FOR COMMUNITY CORRECTIONS PLAN AND JAIL NEEDS ASSESSMENT IN ASHTABULA COUNTY WHEREAS, Lt. James Kemmerle, Ashtabula County Sheriff s Department has submitted a request to the Board to order a Request for Quotes for consulting services for Community Corrections Plan Update and Jail Needs Assessment in Ashtabula County WHEREAS, this board would agree that a Request for Quotes be issued for consultation services from qualified criminal justice firms which possess the knowledge, skills and experience to update the comprehensive community corrections plan and to conduct a comprehensive jail needs assessment study; now THEREFORE, BE IT RESOLVED, By the Board of Commissioners of Ashtabula County, Ohio, that a quotation opening will be held on the 18 TH day of October, 2016 at 2:00 p.m. at the office of the Ashtabula County Commissioners and the Clerk of the Board is hereby ordered to post notice via the internet at the Ashtabula County Website, RESOLUTION NUMBER AUTHORIZING THE ASHTABULA COUNTY ENGINEER TO PREPARE AND SUBMIT AN APPLICATION TO PARTICIPATE IN THE OHIO PUBLIC WORKS COMMISSION STATE CAPITAL IMPROVEMENT AND/OR LOCAL TRANSPORTATION IMPROVEMENT PROGRAM AND TO EXECUTE CONTRACTS AS REQUIRED

10 WHEREAS, It is the desire of the Board of Ashtabula County Commissioners to submit an application to the Ohio Public Works Commission to participate in Round 31 of the OPWC State Capital Improvement Program; and WHEREAS, Ashtabula County through the Engineer s Office is planning to make capital improvements by replacing the Maple Road Bridge (TH 216 C 0.7) and by resurfacing Windsor Mechanicville Road (CH 9 S-W); and WHEREAS, the infrastructure improvement herein above described is considered to be a priority need for the community and is a qualified project under the OPWC program; and WHEREAS, the total project is $700, with the Ashtabula County Engineer s local share of $350, is to be paid out of MVGT funds; and WHEREAS, It is the desire of this board to designate its President, Daniel R. Claypool as official signatory of the application and related contracts; now THEREFORE, BE IT RESOLVED, By the Board of Commissioners of Ashtabula County, Ohio, that an application for funding from the Ohio Public Works Commission is hereby authorized to be filed; and BE IT FURTHER RESOLVED, That the President of the Board, Daniel R. Claypool, be hereby designated as the official signatory of the Board on the application and any related contracts, and Lisa Hawkins, Clerk of the Board, be instructed to send a certified copy of this resolution to the OPWC and the Ashtabula County Engineer s Office. RESOLUTION NUMBER APPROVING MEMORANDUM OF UNDERSTANDING BY AND BETWEEN THE ASHTABULA COUNTY COMMISSIONERS AND THE ASHTABULA COUNTY 503 CORPORATION FOR ECONOMIC DEVELOPMENT SERVICES WHEREAS, the following Memorandum of Understanding has been presented for the approval of the Board, to-wit: Scope: MOU for development activities that improve and enhance economic development including: Enterprise Zone Tax Incentives, CRA s, CDBG Projects and other economic development projects deemed necessary by both parties Entities: 503 Corporation, Jefferson, OH Cost: Term: $20, annually begins upon signing; now Agreement as noted above is approved in accordance with the copy now on file in this office. VOTE: Yeas: Kozlowski, Carlo Nays: None Abstained: Claypool Absent: None RESOLUTION NUMBER REAPPOINTING MARNA DRUM TO THE NORTHEAST OHIO COMMUNITY ALTERNATIVE PROGRAM (NEOCAP) FACILITY GOVERNING BOARD

11 WHEREAS, the Northeast Ohio Community Alternative Program (NEOCAP) was established by the Common Pleas Judges of Portage, Geauga, Lake, Ashtabula and Trumbull counties in 1997 and serves as the Community Based Corrections Facility (CBCF); and WHEREAS, Ohio Revised Code Section (E) provides that the Community Based Corrections Facility be governed by a Facility Governing Board to which the judicial advisory board of the CBCF shall appoint two-thirds of the members and the boards of county commissioners of the member counties of the CBCF shall appoint the remaining one-third of the members; and WHEREAS, in accordance with the Ohio Revised Code Section (E), the combined Boards of Commissioners from the member counties appoint this member; now THEREFORE, BE IT RESOLVED, By the Board of Commissioners of Ashtabula County, Ohio, hereby reappoints Ms. Marna Drum to serve on the Facility Governing Board for an additional three year term beginning November 9, 2016 and expiring November 8, RESOLUTION NUMBER SETTING THE DATE OF VIEW AND PUBLIC HEARING ON THE VACATION OF A PORTION OF BELDEN ROAD IN ANDOVER TOWNSHIP WHEREAS, the Board of Commissioners has accepted for filing the petition for the proposed vacation of a portion of Belden Road located in Andover Township; and WHEREAS, In accordance with Section of the Ohio Revised Code, the Board of Commissioners shall set the time of view and final hearing in said matter; now THEREFORE, BE IT RESOLVED, By the Board of Commissioners of Ashtabula County, Ohio, that said Board has set the date of October 25, 2016 at 9:00 a.m. as the date and time when it shall view the proposed roadway vacation and the date of November 1, 2016 at 1:30 p.m. in the Conference Room of the Board of County Commissioners, 2 nd Floor, Old Courthouse Building, 25 West Jefferson Street, Jefferson, Ohio; as the date, time and place of the final hearing on said vacation. MOTION: Claypool moved the adoption, Carlo seconded. DISCUSSION: None RESOLUTION NUMBER FINA APPROVING FINANCIAL TRANSACTIONS FOR THE COUNTY WHEREAS, it is declared and determined that all formal actions of the Board of County Commissioners concerning and relating to the adoption of all resolutions that were adopted in this meeting, and that all deliberations of

12 the Board of Commissioners and any of its committees that resulted in such formal action, were open to the public and were in compliance with all legal requirements, including Section of the Ohio Revised Code. Lori Larson: Workers Comp Updates a. 15 Medical Only claims & 3 Lost Time claims Debbie Santiana do the independent medical review b. Varckette refiled lawsuit. The 1 st settlement for the elevator accident was complete about $8,000 to $10,000. He has now refiled the lawsuit. CORSA Updates c. Loss Control Incentive Program- 2 requirements left from the audit. Annual building inspections, 5% of the incentive. The other is 65% or $11,375 value- a documented ongoing building inspection with risk prioritization. CORSA has the Facility Dude, that other counties use. Mark Verzella is very interested in the inventory system they have. CORSA pays 75% of the cost. Need an system for the maintenance staff, will cost $2,480.00, and $ annually after that. It is based on the sq. footage of the county. It s a cloud based system, no licenses required. It can go to all county facilities covered on our statement of values. Lori would pay half thru safety funds, ACDES would pay the other half. It also results in $11,375 if in place by February. There is a webinar scheduled. We have also coordinated demolition of a 15 th St property for Conneaut City. o Pymatuning area added as target area. Met with Landbank to determine next target area to add. Will work on Ashtabula East (Ashtabula township, Kingsville township, N.Kingsville) then Census Tract 12 (Denmark, Monroe, and Pierpont Twp) o Met with Prosecutor, Land bank to write an amendment to MOU s with cities to cover the costs of demos in excess of $25K. Income Surveys o Orwell unable to obtain sufficient surveys in timeframe. o Conneaut survey complete 60.78% LMI compared to 45.47% in HUD data. o Geneva City to conduct survey on census tract 8.02 block group 3 to determine eligibility for Target of Opportunity grant. Next timeline goal is May 31, o No longer accepting applications. o Owner Occupied Rehab goal is 12; 3 complete, 3 under contract, 1 waiting specs, 3 waiting inspection, 2 applications pending o Home Repair goal 23; 18 complete; 1 waiting specs; 2 waiting inspection; 2 applications pending. Having trouble with USDA participating on a couple of these.

In Pierpont, get it back on the tax roles. o Saybrook Twp list partial phase two inspection complete. Saybrook trying to find a party willing to apply for funding. ODSA is not receiving applications for Abandoned Gas Station Cleanup Grant because of concerns about liability. Wrote LGIF grant for Community Corrections Plan and Jail study. Notified to submit MOU s during cure period which ends 10/11/16. Continuing to work with Richmond Township regarding Wargo property. Neighboring property owner willing to donate funds to township for demolition. Prosecutor has filed tax foreclosure. Neighboring property owner provided title search. 17 911 PSAP s/equipment future Technical Committee Standards- meeting to formalize to move forward. He has certain people that should be on the technical committee. The law states who is to be on the committee. There is a technical committee that is outlined in the Ohio Revised Code. Coast Guard- met yesterday. There was a lot of security, stated it was for proficiency and efficiency. This will be a seasonal coast guard station at the end of 2017, could be 4 months or longer. It has already been approved by Washington. They are sending Congressional notifications out. A statement was made when the water gets hard, they will not be there.
