THEREFORE BE IT RESOLVED, that after review, the Fulton County Commissioners hereby approves the minutes of Regular Session held July 30, 2015 as presented; and BE IT FURTHER RESOLVED, by the Board of County Commissioners, County of Fulton, State of Ohio that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code. 6 R E S O L U T I O N In the Matter of Accepting Recommendation of ) Office County Commissioners Loss Control Coordinator to Approve ) Fulton County, Ohio Workers Compensation Claim ) August 11, 2015 The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 11, 2013, at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the following resolution: WHEREAS, the Fulton County Loss Control Coordinator, Dawn Hogrefe, has submitted notice of a new Workers Compensation Claim being filed for: NAME DEPARTMENT DATE OF INJURY STATUS Kenneth Brink Senior Center 7/16/15 med only/no lost time WHEREAS, the Loss Control Coordinator recommends approval of said claim; and THEREFORE BE IT RESOLVED, that the Fulton County Board of Commissioners accepts the recommendation of the Loss Control Coordinator, thereby approving the Worker s Compensation claim as listed hereinabove for Kenneth Brink; and BE IT FURTHER RESOLVED, that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code. For Rock ) Fulton County, Ohio Salt Ice Control ) August 4, 2015 The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 4, 2015, at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the following resolution: WHEREAS, on July 9, 2015, the Board of Commissioners held the bid opening for rock salt for ice control for the Season on behalf of the County Engineer; and WHEREAS, on July 14, 2015, the Commissioners awarded the bid to the lowest and best bidder, Morton Salt, Inc. of Chicago, IL, in the amount of $54.83/ton for delivery by truck, $54.00/ton hauled by County and a contract has been successfully negotiated; and THEREFORE BE IT RESOLVED, that the Fulton County Board of Commissioners hereby enter into Contract with Morton Salt, Inc. for rock salt for ice control on behalf of the County Engineer pursuant to the terms and conditions contained therein; and BE IT FURTHER RESOLVED, that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code. 8 R E S O L U T I O N In the Matter of Resolution to Approve ) Office of County Commissioners Oversight of Contract between ) Fulton County, Ohio The Fulton County Department of Jobs & ) August 4, 2015 Family Services and Adriel, Inc. ) The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 4, 2015, at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the following resolution: WHEREAS, a contract has been successfully negotiated on behalf of Fulton County Department of Jobs & Family Services and Adriel, Inc. for the procurement of goods and services; and THEREFORE, BE IT RESOLVED, that the Fulton County Board of Commissioners approve oversight of Contract between the Fulton County Department of Jobs & Family Services and Adriel, Inc. for the procurement of goods or services for use by The Department Jobs & Family Services in the administration of the Visitation Program, pursuant to the terms and conditions as set forth therein; and BE IT FURTHER RESOLVED, that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code. 9 R E S O L U T I O N In the Matter of Resolution to Approve Subsidy ) Office of County Commissioners Grant Agreement for Community-Based ) Fulton County, Ohio Corrections Programs ) August 4, 2015 The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 4, 2015, at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the following resolution: WHEREAS, Fulton County has made application to the Ohio Department of Rehabilitation and Corrections (ODRC) for funds made available under the Community Corrections Act Grant, and has submitted a proposal for the use of these funds; and WHEREAS, pursuant to that application Fulton County has been awarded the sum of $29, for the Fiscal Year beginning July 1, 2015 and ending June 30, 2017 and it is now necessary to execute the grant agreement associated therewith; and THEREFORE BE IT RESOLVED, that the Fulton County Board of Commissioners will enter into Grant Agreement for the Community-Based Corrections Program with the State of Ohio, Department of Rehabilitation and Correction, Division of Parole and Community Services, Bureau of Community Sanctions, pursuant to the terms and conditions contained therein and BE IT FURTHER RESOLVED, that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code. 10 R E S O L U T I O N In the Matter of Resolution Approving FY14 ) Office of County Commissioners SRE Storage Building Site Selection & Design Report ) Fulton County, Ohio ) August 4, 2015 The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 4, 2015, at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the resolution herein. WHEREAS, in accordance with the conditions of the FY14 Grant Agreement previously awarded and entered into by the Board on September 22, 2014, Delta Airport Consultants has prepared the Site Selection and Design Report for the Snow Removal Equipment Building (AIP # ) and submitted to the Board for their review and approval; and WHEREAS, after review, the Board finds the report to be satisfactory. THEREFORE BE IT RESOLVED, the Fulton County Commissioners hereby approves the SRE Storage Building Site Selection & Design Report as presented and authorizes the Report to be submitted to the Federal Aviation Administration; and BE IT FURTHER RESOLVED, by the Board of County Commissioners, County of Fulton, State of Ohio that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code. 11 RESOLUTION In the Matter of Resolution to Proclaim ) Office of County Commissioners August 2015 as Child Support Awareness ) Fulton County, Ohio Month ) August 3, 2015 The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 3, 2015 at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the following resolution: WHEREAS, to improve the well-being of Ohio s families and to assist them with realizing their greatest successes, children need positive emotional and financial support; and WHEREAS, children grow and thrive with the commitment of both parents to provide for their emotional and physical needs, as well as opportunities for enriching their minds; and WHEREAS, Ohio s Child Support Program will continue to reach out and share information by collaborating and building partnerships with people and organizations to support children and strengthen families; and WHEREAS, through the leadership of the Fulton County Child Support Enforcement Agency, we recognize the importance of educating our parents about services that are available through Ohio s Child Support Program; and WHEREAS, the theme of this year s Child Support Awareness Month is Support is Key. We applaud parents who are committed to the success of their children; therefore during Child Support Awareness Month, we acknowledge and celebrate parents who provide positive emotional and financial foundations for their children; and NOW, THEREFORE, BE IT RESOLVED THAT WE, the Fulton County Board of Commissioners, do hereby designate, in conjunction with the State of Ohio, August 2015 as CHILD SUPPORT AWARENESS MONTH. 17 RESOLUTION In the Matter of Resolution to Establishing Hearing ) Office of County Commissioners On Objection to Type III Expedited Annexation ) Fulton County, Ohio Petition as filed by Quiring Delta Real Estate 1, Inc. ) August 4, 2015 The Board of County Commissioners of Fulton County, Ohio met in regular session pursuant to notice, on August 4, 2015, at 152 South Fulton Street, Wauseon, Ohio, with the following members present: Commissioner moved for the adoption of the following resolution: WHEREAS, a petition for Type III-Expedited annexation has been filed by Quiring Delta Real Estate 1, Inc., pursuant to ORC and , requesting the realproperty identified in said petition be annexed to the Village of Delta; and WHEREAS, on this the 4 th day of August, the Board of Commissioners has been given notice of a filing of objection from the York Township Board of Trustees to the proposed annexation and it is now necessary to schedule the hearing on said objection; and THEREFORE, BE IT RESOLVED, that the Board of County Commissioners of Fulton County hereby establish August 6, 2015 at 10:00 am as the date and time of the hearing on the objection as filed by the York Board of Township Trustees, said hearing to be held at the hearing room of the County Commissioners of Fulton County, Ohio; and BE IT FURTHER RESOLVED that it is found and determined that all formal actions of this Board of County Commissioners, County of Fulton, State of Ohio concerning the adoption of this resolution were adopted in an open meeting of this Board of County Commissioners, and that all deliberations of this Board of County Commissioners and of any of its committees that resulted in such formal action, were in meetings open to the public in compliance with all legal requirements including Section of the Ohio Revised Code.
