1 1 Life Insurance Corporation of India Divisional Office: Indore Tender Document Ref: DO/Ind/Signage Dated: Last date for submission Pre-bid Meeting Tenders are invited for Supply, Installation, Transport and Commissioning (SITC) of Official Signage boards at different offices/installations under Life Insurance Corporation of India Divisional Office, Indore. The bid would contain two parts: 1. Technical Bid. 2. Financial Bid. The Description of Sign Boards is has been appended below. The tender activity will take place as per the activity plan given below. The terms conditions must be accepted without any precondition and signed as token of acceptance. The application in format A (Technical Bid) and format B (Financial Bid) with necessary documents should be submitted duly signed under seal. All this, as given above should be complied before the closure of bid for its consideration. General Qualification 1. This tender is opened to all Indian Firms/Companies who are eligible to do business in India under relevant laws. However they must have their office or Operating Representative in city of Indore 2. Firms/Companies black-listed by Government/Public Sector Undertakings should not participate. Minimum Eligible Criteria of the Firm 1. The bidder firms must have past EXPERIENCE in the area of work being applied for here. Proof of such experience must be enclosed with TECHNICAL BID. Preference would be given to firm with sound past EXPERIENCE. 2. The firm/company should be of minimum 3 (three) years of standing. 3. The firm/company should be of sound financial standing. 4. The firm/company should be in position to supply/ carry out work orders without delay. It must be in position to comply with the set schedule of work.

THE FIRM/COMPANY SHOULD HAVE PERMANENT OFFICE OR REPRESENTATIVE IN INDORE.IN CASE IT IS NOT SO,FIRM MUST SUBMIT WITH THE TECHNICAL BID AS TO HOW IT WILL PROVIDE SERVICES IN THE AREA OF DIVISIONAL OFFICE INDORE. 6. The firm/company should have registration with the Government and local authorities for undertaking the profession for which it has applied herewith. (Copies of State/ Local Registration License, VAT/TAN number should be enclosed). 7. The firm/company should have Bank Account and PAN number (Copies should be enclosed). Two bid System Offer The offer will be in two parts; Technical and Financial A. The envelope containing Technical bid superscripted with the wording Technical Bid for Supply, Installation, Transport and Commissioning (SITC) of Official Signage boards at different offices/installations under Life Insurance Corporation of India Divisional Office, Indore should not contain any price information and should comprise of following in the below mentioned order. 1. Technical Bid in the prescribed format (Annexure-A) duly signed by authorized person with rubber stamp of the firm. 2. Earnest Money Deposit of Rs.15000/- (Rupees Twenty thousand only) in the form of a demand draft/pay order issued by a scheduled commercial bank favoring LIC of India, payable at INDORE. 3. Tender Fee in form of Demand Draft favoring Life Insurance Corporation of India for Rs.200/- (Rupees Two Hundred only) payable at INDORE. 4. Duly signed Terms & Conditions as token of accepting them.. 5. Addresses and telephone number of Offices and/or contact person at Indore. 6. List of past clients Purchase Orders or Satisfactory Working Certificate specific to the work for which this tender is being submitted. 7. Documentary evidences confirming Registration with Sales Tax, Service Tax TIN No/VAT Etc. 8. An undertaking/ declaration that Tenderer shall honor the warranty commitments

It should contain only financial bid as per format B. It must give all the relevant price information, (both, in figures & words). All quotations are to be inclusive of all applicable taxes. The financial offer should not contradict the technical offer in any manner. The price schedule must be filled in completely without any error, cutting or alterations in rates (both in figures & words). The Financial bid of only those Tenderers who will qualify in Technical bid will be opened. The Technical Bid Envelope and Financial Bid Envelope prepared as above are to be kept in a single sealed envelope super scribed with the wording Tender for Supply, Installation, Transport and Commissioning (SITC) of Official Signage boards at different offices/installations under Life Insurance Corporation of India Divisional Office, Indore No tender documents will be considered unless & until all the documents are properly signed & stamped and all corrections must also be signed by the bidder. Frame covered by 1.2 mm powder coated side panels 26 Gauge GI sheet covering the rear. 5. Edge to Edge lighting and mm drip cap. 6. T5 (15 mm Dia) Tube Lights of Phillips or equivalent instead of ordinary Tube Lights ( 25 mm or 38 mm) 7. Electronic Choke sq mm wiring (ISI standard).

9 9 Terms and Conditions 1. Criterion for Disqualification 1. Failure to provide all or any information/ documents required in the bid proposal or any other information as required by the corporation at any time with or without supporting document. 2. Non- receipt of bid proposal on or before due date and time, 3. Misrepresentation/ wrong information in the bid proposal. 4. Non- payment of EMD. 5. Conditional Bid. 6. Non- payment of tender fees. 7. Use of unfair means. 8. Bids found in unsealed cover, unsigned bids, and any unconfirmed alteration. 2. Acceptance of Bid Corporation reserve the right to accept the lowest or any bid and reject any or all bids with/without assigning any reason. It also reserves the right to accept or prefer any bid or bidder without assigning any reason. 3. EMD refund The EMD deposit of unsuccessful bidder will be returned through NEFT mode without any interest within 30 days of the date of decision. The EMD of the successful bidder will be returned through NEFT without any interest within 30 days of submission of Security Deposit/Bank Guarantee in favor of Corporation or may also be converted in to security deposit 4. Security Deposit The Tenderer whose tender is approved will have to submit security deposit equivalent to 10% of the total order value/contracted value with Indore Divisional Office before placement of order by way of Demand Draft or Bank Guarantee. the event of breach of any condition or the quality / quantity of stores supplied do not confirm to the stipulations contained in the contract, the security deposit shall be forfeited in addition to the Corporation seeking other relief which may be available as per law. Security Deposit will be adjusted in the final payment on successful completion of the work. 5. Forfeiture of the EMD amount EMD amount would be forfeited in case the Tenderer refuses to execute the order either in full or part for any reason whatsoever, if placed at their quoted/agreed rates. In that case further action like blacklisting of firm may also be initiated. If after tender is

10 10 submitted, Tenderer decides to withdraw it, it will not be allowed. However if Tenderer insists the EMD will be forfeited. 6. Product Specifications 1. Tenderer should quote rates taking in to account only the pre-specified OEM. If tender is submitted quoting rate taking in to account other OEM than pre-specified one, it would be summarily rejected. 2. Tendering firm if given order should provide WARRENTY CARD issued by prespecified OEM 3M Pangraphics for the material used. Apart from that, tendering firm must also submit an undertaking on a stamp paper of Rs that will be provide warranty is irrespective of the OEM commitments. 3. If after the supply or installation or commissioning it is discovered that the signage supplied do not conform to the specifications such supplies will be rejected at suppliers cost who will have to replace it with supplies exactly according to the specifications. In case of non-compliance with this condition Corporation will take actions as it would deep fit. 7. Price Composition 1. Cost of the signage as per the specifications and the estimated job. 2. Cost of the installation as per the list of locations provided herewith. 3. All types of taxes quoted separately. 4. Warranty exclusive of the OEM warranty on the components used for three years from the date of installation. All of the above except the taxes will be taken to decide the lowest bidder. Once quoted the bid value will not be allowed to change for any reason whosoever. 8. Delivery Installation and Commissioning The Signage should be installed at the locations given herewith as per the pre-defined specifications within six weeks of placing the order. No issue such as road permit for transportation, transportation or any other type of damage etc will be of any concern to the Corporation. Any delay in carrying out the order will attract penalty as defined under Liquidation damages of the tender. Pre-Installation and Post-Installation inspection may be carried by the LIC officials. The supplier must satisfy the inspection team with all the relevant documents etc. 9. Liquidation damage In case of delayed installation of signage after the stipulated period, Tenderer shall be liable to pay liquidated damages at a percentage of the delayed order value subject to minimum Rs and maximum 3% of the delayed order value, as detailed below:

11 11 1% for the first week. for the second week. 3% for the third week and above. For calculating the delay as above part of the week will be considered as full week. Delay in the delivery obligations may also attract any or all of the following actions in addition to the above: 1. Forfeiture of Bank Guarantee/ Security deposit. 2. Imposition of penalty. 3. Termination of contract. 4. Blacklisting of Firm. However the in case FORCE MAJEURE is considered for invocation, a notice to corporation should be sent within two days of such eventuality. However request of the contractor, if given for extension of the time and accepted by the corporation, Liquidation Damage penalties will not be imposed. 10. Payments No interim payment would be made. Full and final payment shall be made after successful installation and commissioning of all the Signage at the locations given herewith after deduction of TDS as per the rules. 11. Termination of Contract The Corporation reserves the right to terminate/cancel the whole or part of the agreement at their sole discretion after giving a notice of 15 days. If the contract is terminated by the service provider, it would be liable for penalty which would be equivalent to 10% of the unfinished part of the total work. 12. Disputes In the event of any dispute, the decision of Senior Divisional Manager LIC of India Divisional Office would be final and binding. However in case there is need for further arbitration, it would be done under the provisions of Arbitration and Conciliation Act The venue of arbitration will be Indore. Only courts with jurisdiction of Indore would be contacted in case of legal referrals of Disputes. Notwithstanding the above the decision of LIC of India shall be final and binding on the Tenderer. 13. Validity of the Contract All Tenderer will submit the bids only if they agree to all terms and conditions mentioned in the tender document and subsequent modification if any. Any other requests for change in the specification, terms and conditions of the tender document will not be accepted.

12 The Tenderer must sign on each page of the terms and conditions enclosed herewith as confirmation of their acceptance. 14. Pre-bid Meeting 12 Clarification/Modification after the Pre-bid Meeting will become part of this tender and will be applicable to all the participants and also Corporation. 14. General Terms and Conditions 1. The Tenderer shall leave the site clean and hand over the site in proper manner. 2. The Tenderer shall make their own arrangement for watch and ward of his materials and Corporation will not be responsible their safety in any manner. 3. The work executed should be approved by the Corporation and Tenderer shall rectify any bad workmanship pointed out at any stage and remove from the site all the rejected materials immediately. 4. The Tenderer shall be responsible for application of labor laws, compensation for injury and accidents to the persons, whether employed by them or by their sub-contractors 5. The Tenderer shall ensure that no damage is caused to the Corporation s property/decorative structure/fittings while carrying out the work. The Tenderer will have to rectify/make good such damages at their cost. 6. The Tenderer shall ensure that the workman employed by them for the execution of work are suitably covered under Workmen s Compensation Act, ESI and other legislative enactments applicable for such work and the corporation shall not be liable for payment of any compensation etc in this regard.
