Receipt at the cash counter on 3 rd Floor, West Wing, Yogakshema, J B Marg, Mumbai or in the form of Demand Draft favoring LIC of India are invited for re-filling of tenders Tender quotation should be either TYPE WRITTEN or MUST BE IN INK and should NOT be with any pre-conditions The interested vendors fulfilling the following conditions may apply in two-bid - 1. Should have satisfactorily completed similar jobs in any PSU, PSU Bank, Public Sector Insurance Companies, Public Sector Financial Institutions during FY , , Attach certified copies of work orders clearly specifying nature of work, value of the job awarded. 2. Vendor/agency must have its office/branch and service centre in Mumbai also and must be in existence for the last 5 years Attach proof 3. Vendor/agency should be a licensed agency in accordance with Maharashtra Fire Prevention and Life Safety Measures Act,2006 at the time of applying and during the contract period Attach copy of valid license 4. Vendor/agency must have an average annual turnover of Rs.50 lacs from works of similar nature.attach copies certified by Chartered Accountant 5. Vendor/agency must have a valid PAN, Service Tax Regn No Attach copies 6. Vendor/agency must have a team of competent/qualified technical personnel 7. The Vendor/agency should submit its offer letter along with refundable EMD of Rs.5,000/- in the form of DD drawn in f/o LIC of India payable at Mumbai. 8. The offer letter shall remain valid for a period of 90 days from the date of opening of technical bids 9. Relevant information/copies of certificates, work orders should be annexed with tender documents. Non-receipt of any information/certificates with the tender may entail rejection of the tender. 10. 2 General 1. Delivery & Completion:- Re-filling of fire-extinguishers as per ISI specifications should start within 10 days from the date of awarding the order and should be completed in a phased manner in lots of 40 to 45 each Each lot should be reinstalled within 15 days from the date of taking up the lot, failing which penalty of 0.5% per week of the total contracted refilling cost of the lot will be charged for the delay subject to maximum of 5%. When fire extinguishers are taken for re-filling, the vendor shall provide temporary replacement at no extra cost whatsoever till the fire extinguishers are brought back and installed 2. Refilling work would be carried out in consultation with Chief Security Officer, Deputy Officer of LIC Random testing of the quality of MAP powder at the time of refilling will be carried out by Chief Security Officer, Deputy Officer of LIC. 3. The testing should be carried out with pressure prescribed as per specification of ISI Standard. One or two fire extinguishers of each category should be tested to ensure satisfactory compliance. Cost of testing will be borne by the Vendor. 4. After awarding of order, in case of unsatisfactory services and/or failure to complete the job in time, LIC of India reserves the right to terminate the contract black-list the agency and forfeit the EMD, Security Deposit. 5. The EMD of unsuccessful bidders will be refunded within a period of 45 days from the date of issuing work order to the Successful bidder. However, in case the successful bidder backs out/withdraws before/after the work order is issued, then, his/her EMD will be forfeited and the Vendor/agency will be black-listed. 6. LIC reserves the right to cancel one or all tenders without assigning any reasons whatsoever 7. Envelope-1 should be super-scribed Tender for re-filling of fire extinguishers at LIC of India, Yogakshema, Mumbai and shall contain the following: Envelope-2 Envelope-3 The envelope for Technical bid, Envelope-2 shall be super-scribed Technical bid for re-filling of fire extinguishers at LIC of India, Yogakshema and shall contain the following: Information as per the proforma Mandatory Technical Requirement along with copies of required documents. Tender document duly stamped and signed on each page as token of acceptance of all terms & conditions laid down Offer letter EMD of Rs.5,000/- in the form of DD favoring LIC of India payable at Mumbai The envelope for Financial bid, Envelope-3 shall be super-scribed Financial bid for re-filling of fire extinguishers at LIC of India, Yogakshema and shall contain the following: Rates for re-filling of fire extinguishers should be excluding VAT,Service Tax and in the format given in the tender document. Submission of Financial Bid in any format other than the format provided in the tender document will not be accepted and the bid will be rejected. 8. The rates quoted in the Financial Bid shall be inclusive of Transportation Charges, Technicians Fees, TA/DA any other expenses whatsoever. The successful bidder shall have to deposit a sum equivalent to 10% of the Contract Value as Security Deposit, which in the event of not carrying out the contracted services as per the terms and conditions of the tender, shall be forfeited. Security Deposit in the form of DD fvg. LIC of India, payable at Mumbai shall be deposited within 5 days of awarding the order. Security Deposit without interest will be refunded at the time of final settlement after satisfactory completion of work. 10. Payment to the vendor/agency shall be made after satisfactory completion of the work. 11. Disputes, if any, shall be considered to be within the jurisdiction of Courts in Mumbai 12. The vendor will carry out the re-filling of existing fire extinguishers which shall include:- a) Replacement of defective spare parts like discharge tube, nozzle, horn water, washer, plunger etc at the time of re-filling as per approved rates subject to prior approval of LIC b) Repair of fire extinguishers, if required but subject to prior approval of LIC c) Refilling charges quoted shall include re-filling of gas cartridges and hydro-testing of the concerned fire extinguishers d) Hydro testing certificate of fire extinguishers will be submitted along with bill e) Replacement of small spares like rings/washers to be done at no extra cost 13.
