1 Madhya Pradesh Rural Roads Development Authority (An Agency of Panchayat & Rural Development Department, Govt. of MP) Block 2, 5 th Floor, Paryavas Bhavan, Bhopal No /22/D-12/ FA/NABL /2018 Bhopal, Dt. 05 / 03/2018 Invitation for Expression of Interest for Empanelment of NABL Labs MP Rural Road Development Authority, an Agency under Panchayat and Rural Development Department, government of Madhya Pradesh intends to empanel NABL accredited labs (in Mechanical /Civil works) from Madhya Pradesh State only for random quality assurance assignment of various projects in the State of Madhya Pradesh. Interested agencies from Madhya Pradesh may submit proposal at MPRRDA Head Office Prayawas Bhawan Bhopal on prescribed form with required documents as listed in para 4 of detailed EOI upto 3.00 PM on Detailed EOI notice can be downloaded from department's website : Cost of EOI Document is Rs. 5000/- which should be submitted in the form of DD in the name of CGM, MPRRDA, Bhopal along with the proposal. A pre proposal conference will be held at MPRRDA Bhopal meeting hall on at 3.00 PM to discuss the scope of work and answer quires that might be raised by the participant firms at that stage. 2 Madhya Pradesh Rural Roads Development Authority (An Agency of Panchayat & Rural Development Department, Govt. of MP) Block 2, 5 th Floor, Paryavas Bhavan, Bhopal No /22/D-12/ FA/NABL/2018 Bhopal, Dt. 05 / 03/2018 Details of Invitation for Expression of Interest for Empanelment of NABL Labs MP Rural Road Development Authority, an agency of Government of Madhya Pradesh Panchayat & Rural Development, intends to empanel NABL accredited Lab (in Mechanical /Civil works) from Madhya Pradesh State only for third party quality assurance of its various projects in the State of Madhya Pradesh on the following terms and conditions upto 3.00 PM on :- 1- Qualification Requirement. 1. The Lab should be accredited from NABL (National Accreditation Board for Testing and Calibration Laboratories). After accreditation, the Lab should have at least on surveillance audit from NABL and the accreditation should be live at the time of application. 2. Accredited Lab should be based in Madhya Pradesh. 3. Firm should have NABL accreditation for, testing of any three of the following material:- Soils / GSB Aggregates Bitumen Bituminous Mix Cement Cement concrete Steel Building Material (Details of various tests are given in Annexure-I ) 4. Documents to be submitted (i) Firm s profile in the enclosed format (Annexure-II). (ii) Copy of accreditation certificate issued by NABL. 2

3 (iii) Surveillance audit certificate. (iv) List of key personnel available with the proposer / Lab (Location) (v) List of equipments. (vi) Audited profit and loss account & Balance sheet of last three years. (vii) Affidavit, duly notarized stating that Information/documents given with the proposal are true and correct to the best of my knowledge and belief. If any information is found to be incorrect, subsequently, even after empanelment, MPRRDA may forfeit SD & blacklist from further working in the department. No close relative is working in MPRRDA. 5. Financial Turnover Firm should have minimum annual financial turnover of Rs Lakhs in any one year during last 3 years from testing fee only. 2- Other Terms and Conditions:- (i) (ii) (iii) (iv) (v) (vi) After processing of application, the firm will be given date for inspection of Lab. Officers nominated by MPRRDA will inspect and verify the Lab. The inspection will be carried out in accordance with the format attached (Annexure-III). After selection for empanelment, the firm will be informed for submission of security deposit and execution of the Agreement. Within 15 days of the communication, the firm will have to execute Agreement along with the security deposit of Rs. 2,00,000/- in the form of BG or FDR valid for 24 months drawn in favor of CGM, MPRRDA, Bhopal. Firm will have to work on the approved rate list issued, from time to time, by the Chief Architect cum Chief Engineer (RDD), PWD, Bhopal. Payment to the firm will be made by Head Office, MPRRDA directly on receipt of test reports. Initially, the empanelment will be done for two years which can be extended further depending upon satisfactory performance. The security deposit (SD) will be released on successful completion of contract period. SD will be forfeited if empanelment of the Lab is cancelled due to poor performance / noncompliance of the terms and conditions of the Agreement. 3

4 (vii) Lab may be black listed if found indulging in corrupt practices and passing the samples which in the subsequent test found to be not as per specifications. The firm will be given due opportunity of hearing before any adverse order is passed. (viii) Procedure for collection of samples, testing, submission of reports and payment to the firm will be as outlined in Annexure IV. 11 Annexure-V MADHYA PRADESH RURAL ROAD DEVELOPMENT AUTHORITY, 5th Floor, Block-II, Paryavas Bhawan, Arera Hills, Bhopal (M.P.) AGREEMENT This AGREEMENT is made on this day of, 20 between the Chief Executive Officer, Madhya Pradesh Rural Road Development Authority, 5 th Floor, Block-II, Paryavas Bhawan, Arera Hills, Bhopal (M.P.), Madhya Pradesh on behalf of Madhya Pradesh, Rural Road Development Authority, Bhopal (M.P.) (hereinafter referred to as the Client ) which expression shall where the context so admits, includes his successors in office and assigns on the one part, and (hereinafter called the Lab ) which expression shall where the context so admits, includes his successors in office and assigns on the other part. WHEREAS (a) The Client intends to carry out Testing in respect of Road & Bridge works (hereinafter called the Testing ); (b) The Lab, having represented to the Client that they have the required professional skills, personnel and technical resources, have agreed to provide the Services on the terms and conditions set forth in the Agreement; (c) The Client has received funds from Govt. of India under Pradhan Mantri Gram Sadak Yojna (hereinafter called PMGSY) for construction of road and bridges in the State of Madhya Pradesh. The works and services shall be subject to the guidelines of the schemes. NOW THEREFORE the parties hereto hereby agree as follows: - The following documents attached hereto shall be deemed to form an integral part of this agreement and interpreted in the following order : 1. Acceptance letter. 2. EOI. 3. Terms of Reference and annexure I to V Continued. 11

12 The mutual rights and obligations of the Client and the lab shall be set forth in the agreement; in particular: (a) The Lab shall carry out the services in accordance with the provisions of the agreement; and (b) The Client shall make payments to the Lab in accordance with the provisions of the agreement.
