1 UNICEF/2015/South Sudan/Rich South Sudan Humanitarian Situation Report 24 APRIL 7 MAY 2015: SOUTH SUDAN SITREP #58 SITUATION IN NUMBERS Highlights The security situation continues to deteriorate in Unity and Upper Nile States, with humanitarian actors forced to reduce their field presence in several areas. UNICEF has scaled up health, WASH, child protection and gender-based violence prevention services in response to influxes into Bentiu and Malakal PoCs. Following the final planned release of 282 boys and 1 girl associated with the Cobra Faction in Labrab, the total number of released children is 1,757, including 5 girls. Around 70 per cent of released children have now returned to their families and are being followed up by the National Disarmament, Demobilization and Reintegration Commission and partners to monitor the children s reintegration, identify signs of distress and provide support to these children and their families to access broader reintegration services. The March round of the Food Security and Nutrition Monitoring System (FSNMS) assessment showed global acute malnutrition (GAM) rates above the 15 per cent emergency threshold in the three conflict affected states as well as the two high burden states of Warrap and Northern Bahr el Ghazal. 2 Situation Overview & Humanitarian Needs Since the beginning of the conflict, more than 2 million people have fled their homes, including over 1.5 million people displaced inside South Sudan. According to UNHCR, there has been a sharp increase in new South Sudanese arrivals in Ethiopia, increasing from some 1,000 people a month in the first quarter of the year to more than 4,000 refugees registered in April. The refugees are reportedly fleeing increased fighting and food insecurity in Jonglei and Upper Nile states. UNHCR is currently registering more than 10,000 new arrivals at various entry points in the Gambella region. According to the UN Mission in South Sudan (UNMISS), as of 20 April, the estimated number of civilians seeking safety in six Protection of Civilians (PoC) sites located on UNMISS bases is 119,725 including 52,908 in Bentiu, 34,420 in Juba UN House, 28,717 in Malakal, 2,374 in Bor, 944 in Melut and 362 in Wau. With on-going displacement, over 31,000 people are now reported in Malakal PoC. The security situation continues to deteriorate in Unity State, with the SPLA reported to be retaking areas from the SPLA-iO. This has increased displacement, including to Bentiu PoC, with over 600 new arrivals recorded over the last week. Since late April, humanitarian actors have been restricted to Bentiu PoC and the Rubkona airstrip. UNICEF has prepositioned sufficient supplies in Bentiu to support the on-going influx and sustain the response over the coming months. In Upper Nile, humanitarian space has been significantly reduced due to the resumption of fighting. Humanitarian actors have been forced to reduce their presence in several parts of the State. Tensions are also affecting movements of staff outside of the PoC, with staff fearing being targeted. Following the recent fighting in Fashoda County and Malakal town, over 6,000 new IDPs have sought shelter in Malakal PoC, where UNICEF and partners have scaled up the response. Humanitarian leadership and coordination UNICEF actively participates in the Humanitarian Country Team and the Inter Cluster Working Group, which lead strategic and cross-sectoral coordination of humanitarian programmes. UNICEF leads the WASH, Nutrition and Education clusters as well as the Child Protection sub-cluster. Within the Health cluster, UNICEF provides leadership on vaccination, communication and social mobilization. UNICEF also supports the core supply pipelines for the Education, WASH and Nutrition clusters, providing essential humanitarian supplies to all partners. While continuing to focus on the delivery of life-saving interventions, UNICEF is also investing in providing access to education and a protective environment for affected children. Humanitarian Strategy: Rapid Response Mechanism (RRM) As part of the interagency rapid response teams, UNICEF continues to expand activities in remote locations together with WFP and partners. More than 742,000 people, including over 156,000 children under 5, have been reached by the 47 UNICEF and partner RRMs to date. In 2015, 12 RRM missions in northern Jonglei and northern Unity States have reached close to 190,000 people, including 39,400 children under five. A RRM monitoring mission is currently underway in Wai, Ayod County. UNICEF recently completed a mission to Kuernyang, Jonglei State, reaching 12,206 people including 2,511 children under 5. The main objective of the mission was to respond to displaced populations and host communities following the fighting in Fangak County. The mission was conducted jointly with the partners on the ground, Nile Hope, CADA and CMA. During the mission, 2,357 children were screened for malnutrition with 48 cases of severe acute malnutrition (SAM) detected, as well as 324 cases of moderate acute malnutrition (MAM). Severely and moderately acutely malnourished children were admitted for treatment. Health interventions included the vaccination of 5,630 children against measles and 5,858 children against polio, while 494 pregnant women were vaccinated against tetanus. Twentyfive separated and eight unaccompanied children were identified and registered for family tracing and reunification. Summary Analysis of Programme Response HEALTH: New arrivals continue to be vaccinated upon entry to Bentiu PoC, with 741 children under 15 vaccinated against polio and 572 against measles during the reporting period. Routine immunization also continues in the PoC and no cases of measles or acute flaccid paralysis (AFP) have been identified in the past two weeks. Under five mortality rates have decreased from 17 in week 17 to 3 in week 18, and remain below the emergency threshold. 2

3 In response to the measles outbreak in Upper Nile, a vaccination campaign has started in Maban. Preparations for a measles campaign in Malakal PoC are ongoing, targeting 4,000 newly arrived children 6 months to 15 years. However, similar campaigns in Fashoda and Malakal counties have been stalled due to insecurity. The response to the circulating vaccine derived polio virus (cvdpv2) outbreak, confirmed in October 2014, continues as per outbreak response plan with implementation of short interval additional dose (SIAD) campaigns targeting children under 15 in the three conflict affected states. The three consecutive rounds are being implemented in a staggered manner depending on access, partner presence, processing of funds, and availability of air assets for vaccine distribution. Round one started 2 December 2014 and 1,123,558 children were reached in 28 of 32 counties whereas four counties (Rubkona, Old Fangak, Khorfulus, and Longechuk) have not been reached due to access constraints. The second round is on-going with 476,959 children vaccinated to date in 17 counties and the third round completed in 12 counties reaching 426,565 children. Due to intensified conflict and worsening security situation planned implementation in several counties (Ayod, Leer, Koch, and Mayendit) has been put on hold. In addition to provision of vaccines, logistics and cold chain, UNICEF together with MoH and partners is undertaking community and household social mobilization. The second round of Polio National Immunization Days (NIDs) which took place March in the seven stable states reached (as per administrative data) 2,209,018 children under 5 against the target of 2,134,587. Preliminary results from independent monitoring show coverage reached of 92 per cent. Both first and second round of polio NIDs have been expanded to include accessible areas of conflict affected states but being implemented in a staggered manner as soon as counties have first completed their three rounds of SIAD campaigns. During the last four weeks, 39,951 people accessed basic health care services, out of which 21 per cent were children under 5. Since January, a total of 134,092 consultations have been provided. A total of 38,701 long-lasting insecticide treated nets (LLITNs) have been distributed since January 2015 through RRMs and in PoCs to children under five and pregnant women. Community volunteers in Bentiu continue to provide the integrated package of child health interventions, having referred over 12,282 children to health facilities in April. 4 UNICEF supports IRC, IOM, Health Link and IMC to provide prevention of mother to child transmission (PMTCT) of HIV services in six IDP camps. A total of 499 pregnant women were counseled and tested for HIV during the reporting period, of which 7 tested HIV positive (1.4 per cent) and 4 were given long term antiretroviral therapy. From January to May 2015, 4,462 pregnant women were tested of which 81 tested positive (1.8 per cent) and 47 (58 per cent) enrolled on treatment. NUTRITION: UNICEF continues to provide both financial and technical assistance to partners implementing nutrition programmes, with 39,119 children under 5 with SAM admitted for treatment so far this year. This includes children reached by UNICEF through direct implementation during RRMs in remote and inaccessible areas where there are no services by other partners. Mass screening of children aged 6-59 months was conducted on 23 April in Bentiu Trend of SAM Admissions in Bentiu PoC by Week PoC. Of the 10,547 children 6-59 months 250 screened, 187 (1.8 per cent) were identified with SAM and 699 (6.6 per cent) with MAM with GAM rate of (8.4 per cent). This is a low proxy GAM rate as compared to the previous results from the PoC which could mainly be attributed to the large number of 50 children screened at the gate with relatively better nutrition situation. Disaggregated 0 data shows a proxy GAM of 2.8 per cent for new arrivals and 8.3 per cent for children residing in the PoC. In Bentiu PoC, the four UNICEF-supported outpatient therapeutic programmes (OTPs) admitted 147 children 6-59 months of age with SAM. In Jonglei, a total of 9,870 children were screened from Akobo, Duk and Pibor. The proxy GAM rate stands at 33.6 per cent while the proxy SAM stands at 2.7 per cent. The high GAM rates, with a low number of SAM cases, is of concern and could be attributed to food insecurity. However, a nutrition causal analysis will be conducted in Akobo in June to understand the root cause of malnutrition in the county. A total of 231 children were admitted to OTPs in these counties during the reporting period. In Upper Nile, the proxy GAM is 22.7 per cent while the proxy SAM stands at 1.1 per cent from the 819 children screened. This trend although similar to what is seen in Jonglei has to be interpreted with caution as few children were screened. High defaulter rates were noted in Melut (31 per cent), Fashoda (36 per cent) and Malakal (29 per cent) respectively due to insecurity and displacement. GOAL will conduct a mass MUAC screening in the area to understand the nutrition situation of remaining population in this areas. Data quality in round 15 was poor for half of states, including the conflict affected areas, Warrap and Western Equatoria. While the MUAC measurements were acceptable, errors were mainly due to missing variables (e.g. sex, age, MUAC etc.). Access constraints for partner and UN staff hindered field supervision in conflict-affected areas. UNICEF provided field supervision in Eastern Equatoria and Northern Bahr el Ghazal. GAM rates in the conflict affected and high burden states ranged from 15.4 per cent in Upper Nile to 21.1 per cent in Warrap. These high GAM rates in the conflict and high burden states is worrying in advance of the lean season (May to July). GAM rates are usually high during the lean season due to depletion of harvest, access constraints due to the rains and disease outbreaks. Any resumption of violence will also greatly hinder humanitarian services. It is also important to consider the high GAM and SAM (5.3 per cent) in Northern Bahr el Ghazal despite the current innovative social 4

5 mobilization intervention that has seen many children admitted to nutrition programmes. Information from the FSNMS fed into the Interagency Phase Classification analysis which concluded on 3 May with reports to be released once approved by the Government. 25 Trend of State-Level Malnutrition Based on FSNMS rounds % PREVALENCE ces EES Jonglei Lakes NBS Unity UNS Warrap WBS WES GAM SAM NB: The dark blue bars are the current round 15 results WASH: So far in 2015, UNICEF and implementing partners have reached 243,940 people with access to safe water and 132,145 with safe sanitation. The deteriorating security situation has resulted in an influx of IDPs into Malakal and Bentiu PoCs, creating challenges in maintaining Sphere standards in the provision of WASH services. Sphere Standards for latrine use of one stance per 50 people (1:50) are not being met with Malakal PoC at 1:70 and Bentiu PoC at 1:65. IDPs are waiting for 1 to 1.5 hours at water collection points. In both Malakal and Bentiu PoCs, water supply has reduced from over 15 litres per persons per day (L/p/d), to about 10 and 12.7 L/p/d, though it remains within the Sphere Standards of L/p/d. Additional water supply systems and latrines are being established to accommodate new arrivals are expected to be completed in the next three weeks. However, solid and liquid waste management are an increasing challenge in both PoCs, inadequate waste management trucks and inaccessibility of disposal waste sites outside of the PoCs due to insecurity. UNICEF in coordination with IOM and UNMISS is discussing with local authorities to allow continuous access to the existing sites. In the interim, temporary onsite disposal arrangements have been made to minimize the risk of accumulated waste. In Bentiu PoC, drilling of one new borehole is complete, however, drilling of the second borehole may be delayed due to unavailability of pumps and construction material due to increasing insecurity. The drilling of the second borehole is tentatively scheduled to start on 13 May and should be completed in two weeks. UNICEF is also directly implementing the construction of 682 latrines, of which 53 have been completed. The speed of latrine construction has increased with the establishment of a temporary workshop with electric saws at the construction site. This has also led to job creation and the opportunity for IDPs to learn carpentry skills. 5

6 Hygiene promotion activities and distribution of WASH supplies are ongoing. To date, 106,608 people have benefited from key health and hygiene messages through the integrated Health, WASH and Nutrition outreach services. In addition, 223,469 people have benefited from WASH supplies including soap, buckets, collapsible jerry cans, filter cloths, water purification products and hygiene kits. WASH interventions continue in the Greater Pibor Administrative Area (GPAA), with 8,500 people in five payams of Gumuruk benefiting from WASH supply distribution as part of dry season strategy to reach people before the rainy season. In addition, 55 pit latrine digging kits were distributed in Gumuruk to enable families to construct household latrines, targeting all 8,000 people in Gumuruk town. Further WASH activities are planned for the GPAA including drilling of two boreholes in Lekuangule. WASH facilities and services were also provided in the new Interim Care Centre (ICC) in Labrab. This included the provision of safe water supplies through filtration (Lifestraw Filter Technology) or flocculation (Using PUR ) of surface water. Three latrines were constructed for the ICC along with hand washing stations. WASH supplies including buckets, filter cloths, water purification products and soap were distributed at the ICC and surrounding communities benefiting over 5,000 people. In Eastern Equatoria state, the provision of safe water reached over 28,000 people in Torit (8,000), Kapoeta East (8,500), Kapoeta South (5,000) and Kapoeta North (6,500) through surface water treatment or the repair or rehabilitation of boreholes. However, drilling teams in Guinea Worm endemic areas are facing challenges due to heavy rains. The rains are also creating a shortage of spare parts, affecting operation and maintenance activities. EDUCATION: To date, UNICEF and implementing partners have provided 78,583 children (34 per cent girls) aged 3-18 with Education in Emergencies (EiE) services through the provision of technical assistance, supplies and operating costs to partners as well as through RRM direct implementation. A total of 2,771 (21 per cent females) teachers, Parent- Teacher Association (PTA) members and other education personnel have been trained to effectively support EiE. In the less-affected states, most schools were closed for spring break during the reporting period. In conflict affected states, some TLS remained opened to ensure children are catching up with on lost learning time during schools breaks. In Bentiu, Bor and Malakal PoCs, the term break was used to make improvements to the school environments and construct new temporary learned spaces. In Jonglei, state Ministry of Education, Science and Technology (smoest) has received 12,000 text books (different categories) from UNICEF during the reporting period. They will be distributed to targeted schools when next school term starts on 5 May 2015 to improve quality of learning. The Education Cluster is supporting the smoest to identify the schools most in need. UNICEF and partners continue to scale up education services in the Greater Pibor Administrative Area (GPAA), with over 6,000 children newly enrolled. During the reporting period, this included: Pibor Town: Langachot ALP centre has newly opened for 271 students (20 per cent) and Langachot Early Childhood Development (ECD) centre has enrolled 119 children (31 per cent girls). The centres have received supplies and will start classes next month. Gumuruk: A total of 2,040 students (22 per cent girls) have been enrolled in six operational schools in the county, of these, 135 are boys released from Cobra Faction. Additionally, 428 children (34 per cent girls) have been enrolled in two ECD centres. Vertet: 200 children (40 released boys) are registered in the newly established temporary learning space. The community identified four volunteer teachers who have the capacity to operationalize the school. 6

7 Lekuangole: A total of 515 students (20 per cent girls) have been enrolled at Lekuangole Primary school and are attending classes. An additional 167 learners (5 per cent girls) have begun attending classes at the ALP centre. Labrab: UNICEF implementing partner CAD arrived in Labrab on 28 April and has set up two temporary learning spaces, identified teachers and registered students. Community mobilization to increase enrolment has begun. Recreational activities for registered children will take place during the training as well as orientation sessions to get children ready to start school again. In cooperation with the MoEST and the Ministry of Culture, Youth and Sports (MoCYS), the UNICEF Peacebuilding, Education and Advocacy (PBEA) programme was showcased on 28 April in an event hosted by the Deputy Minister of Education. This programme has undertaken an innovative learning approach, using multi-media strategies such as sports, media, livelihood initiatives used to provide knowledge, values and attitudes that a culture of peace. More than 5,000 children and youth have been reached so far. In partnership with Free Voice, the event was used to launch the 13 children s radio programmes aimed at addressing various thematic areas in life skills and peacebuilding through various radio stations using different native languages. Following the comprehensive youth dialogues and community peace conferences in 2014 in Tonj East where innovative livelihood initiatives were suggested to reduce dependence on pastoral farming. UNICEF established partnership with ACROSS to work in pastoralist communities using a three pronged intervention approach focusing on literacy and numeracy, ox-plough farming and peacebuilding with the aim was to reduce intra-inter-conflicts, increase social cohesion and build resilience within the community. ACROSS has already delivered 40 ox-ploughs and 2,320 mother tongue literacy books and have developed 60 different peacebuilding, life skills and agriculture messages that will be downloaded into solar powered digital audio players. The digital audio players are distributed to literacy listening groups to benefit 1,500 children in the cattle camps. CHILD PROTECTION: Against the backdrop of increased insecurity in Upper Nile and Unity States, UNICEF and partners have rapidly mobilized supplies and human resources to ensure life-saving care is available for GBV survivors. In Malakal, community outreach on where to seek help within the PoC was extended to new arrivals and safety audits were conducted on a daily basis. An additional 2,274 people received prevention and response services, while efforts to mainstream GBV in the ongoing emergency programming also continue. For example, the latrines in the new extension in Malakal include colour coding, structural improvements and separated access points to minimize GBV risk (see picture). Similar improvements are also included in the latrines under construction in the new PoC site in Bentiu. Assessments were carried out in Wau Shiluk, Lol and Kodok and the findings will help inform GBV programming in those locations. Gender-sensitive latrines (constructed by IOM) in Malakal PoC designed to mitigate the risk of GBV. On April, UN Action Against Sexual Violence in Conflict (UN Action), the UN s global coordination mechanism on sexual violence in conflict visited South Sudan to support implementation of the Joint Communique on the prevention of conflict-related sexual violence, which was signed between the UN and the Government in October 2014 and outlines minimum commitments for preventing and addressing conflict-related sexual violence. When released, recommendations from this mission will be used to inform the review of the SRP, the implementation of the MARA and UNICEF s GBV programming. Following the final planned release of children associated with the Cobra Faction, the total number of released children is 1,757, including 5 girls. Around 70 per cent of released children have now returned to their families and are being followed up by the National DDR Commission and partners to monitor the children s reintegration, identify signs of distress and provide support to these children and their families to access broader reintegration services. The establishment of services in Labrab continues to be a logistical challenge, due to inaccessibility by road. To date, UNICEF has chartered nine flights to deliver essential construction, WASH, health, education and other supplies. Access challenges are expected to intensify both in Labrab and in all other release locations which will all become separated and inaccessible by road with the onset of the rainy season. In preparation for this, UNICEF has already begun pre- 7

8 positioning supplies and UNICEF has now negotiated for regular humanitarian flights to Labrab to enable regular staff rotation, with negotiations on-going for other areas. Following a dedicated effort by the Child Protection Sub-Cluster to support partners to report against the new 5Ws template, the number of new children reached with child protection services has increased to 127,587. Around 33 per cent of these children were reached with prevention messaging through direct implementation during Rapid Response Missions. An additional 11,048 people (71 per cent children) were reached with mine risk education during this reporting period. For family tracing and reunification (FTR), UNICEF and its partners are providing FTR services for 7,532 active cases, of which 1,222 are unaccompanied (41 per cent girls), 4,574 are separated (49 per cent girls), and 2,329 are reported as missing (56 per cent girls). Sixty-six reunifications were successfully facilitated by UNICEF and its partners during this reporting period, bringing the total number of children reunified with their families to 1,176 children (47 per cent girls) since Eighteen per cent of all registered unaccompanied children and 19 per cent of all registered separated children have been reunified to date. UNICEF partners are beginning to use a vulnerability framework to ensure unaccompanied and separated children in the most vulnerable situations receive more frequent follow-ups and fasttracked tracing, however reported follow rates remain low and appears to be related to data transfer challenges resulting from limited connectivity and the ongoing RapidFTR upgrade. The upgrade is taking longer than anticipated, but is expected to be completed in May. COMMUNICATION FOR DEVELOPMENT (C4D): UNICEF supported a capacity building workshop on communication theories and methodologies for 33 participants in the greater Nyal area in Unity state. Fifteen participants were health workers from Sign of Hope and the rest were drawn from other health partners, teachers, church leaders and local authorities. Around 30,000 IDPs are living in Nyal, displaced by occasional attacks from Lakes State. Sign of Hope runs a Primary Health Care Centre and three other Primary Health Care Units in payams in greater Nyal. The health workers carry out health education on prevention of common diseases. This type of training is being conducted throughout South Sudan (a similar one was held in Mingkaman earlier in April) and contributes to greater understanding of behaviour and social change theory and how attitudes and norms can be addressed through communication. SUPPLY & LOGISTICS: The Logistics Unit in Juba dispatched 6 trucks containing 62,000 LLITNs to Ministry of Health facilities in Warrap, Aweil, Yambio and Wau. Four trucks carrying Education and WASH supplies were sent to Mingkaman, Bor and Bentiu while another 4 trucks were despatched to Eastern Equatorial state with WASH and Nutrition supplies. 730 cartons of high energy biscuits and WASH supplies were dispatched to Pibor. Two Charter flights were scheduled to deliver cargo from Rumbek to Pibor in the past week but only one rotation was successfully completed due to heavy rains in Rumbek and Pibor which prevented a second rotation with Education supplies from taking off. As a result, the flight had to be cancelled and the Logistics Unit is in communication with the Charter Company on re-scheduling the cancelled rotation. FUNDING: Against UNICEF s 2015 requirement of US$ million, US$ 42 million has been received. UNICEF South Sudan would like to thank the Government of Sweden for the generous unearmarked contribution of SEK 4 million (US$ 465K) which has been allocated by UNICEF South Sudan against critical WASH priorities.
