Verbal or written notice to the OCJAAF COMMISSIONER of your intent to join OCJAAF. (Text, notices are acceptable). 2. Presentation to the OCJAAF Executive Council on or before the April meeting. 3. New Chapter must elect Executive Board consisting of a minimum 5 officers. Attach Board List to this application. 4. Prepare Chapter By-Laws. Attach copy to application. 5. Provide copy of current State & Federal nonprofit status. Or, Initiate incorporation proceedings with state and federal government for nonprofit, charity organization-501(3) (c). 6. Establish chapter boundaries. Any conflict with an existing OCJAAF chapter must be negotiated prior to submitting application. 7. Because of this purpose the OCJAAF has grown to as high as 32 chapter members, currently 18 now embrace the cities from Orange, Los Angeles and Riverside counties. It is because of this diversity we do provide the best opportunity for our player and cheerleader participants, and of course, the best in competition! The OCJAAF has a strong commitment to the word of FAMILY. Families come in many combinations and we celebrate the word of family as meaning; team, chapter, community and the OCJAAF. There is nothing stronger than the spirit in the word FAMILY and you will see it and feel it within OCJAAF. As good as it s been---we know our Program s best days are ahead; our commitment to improve everything we do is ongoing. The OCJAAF is also proud of its innovative procedures and computer programs which assist in the administration of chapter member responsibilities (Roster Program, computerized scheduling, etc). We are proud to be a leader in these procedures and programs that continue to develop per the needs and desires of our chapter members. THE OCJAAF IS KNOWN FOR ITS DIVERSITY & COMPETITION and it is through our diversity which gives us the strongest competition on the field; both in football and in our cheerleading programs. The OCJAAF continues to develop its commitment to the academic side of our studentathletes through the Scholarship and Scholastic Recognition Programs. We offer up to $1,000 scholarship to eligible high school seniors from each of our chapter members and awarded to these recipients at our annual Scholarship Banquet in June of each year. The Scholastic Recognition Program provides scholastic patches, plaques and certificates to eligible chapter participants who maintain a sound academic record throughout our season. Nothing can be more important than to provide rewards and incentives for our student-athletes, this is a commitment made on behalf of all participating chaptermembers and from the leadership of OCJAAF. The OCJAAF Cheerleading Program is comprised of an outstanding group of young ladies who continue to develop in their skills and talents as performing athletes. We have been stunting since 2012 allowing stunt up to Level 3. Our cheer teams are none the less, far and above in their cheerleading and dance performances as evidenced by their National Championship placements each year. These are but a few of the highlights which make the OCJAAF number one as a leading youth sport organization. 3 OCJAAF CHAPTER-MEMBER APPLICATION This application has been prepared to help and assist potential chapters in the process of chapter-membership requirements. Please complete this application in its entirety and be sure to include all required documentation as stated below. Thank you. THE OCJAAF OBJECTIVE. The objective of this program is: to inspire youth, regardless of race, color, creed, or national origin, to practice the ideals of health, citizenship and character; to bring our youth closer together through the means of a common interest in sportsmanship, fair play and fellowship; to impart to the game elements of safety, sanity, and intelligent supervision; and to keep the welfare of the player first, foremost and entirely free of adult lust for glory. REQUIREMENTS FOR NEW CHAPTER APPLICATION 1. Application for new chapters will be accepted by the Executive Council no later than the first Tuesday in March. A New Chapter may present them self to the OCJAAF Executive Council at any meeting prior to or at the March meeting of the current year. Or, notification to the OCJAAF Commissioner followed by the completion of this application to be received no later than the March meeting of the current year. 2. All new chapters are to be conditionally accepted or rejected by the first Tuesday in April. The applicant may make their final presentation to the Executive Council at the April meeting of the current year. 3. A new chapter may not use the name of a team that is already being used without prior written consent of that existing chapter. 4. By election, the new chapter must establish an Executive Board consisting of, as a minimum, President, (not in a head coach capacity), Vice President, Secretary, Treasurer, Athletic Director and Cheer Coordinator. A Board List of these chapter officials must be submitted with this application. Said Board List must include: name, position, address, phone numbers (home, work and cell if applicable), and address. 5. Prepare a set of By-Laws consistent with those of the OCJAAF. (Attach a copy to this application). 6. Adopt a Chapter Name and initiate incorporation proceedings: state and federal filings for 5013c nonprofit, charitable corporation status. 7. Submit a map of the requesting Chapter s boundaries as well as a written description of the Chapter boundaries, i.e.; northern, southern, eastern and western boundary. (See samples of other chapter s written boundaries inside the OCJAAF Schedule Book). 8. The new chapter must agree to field at least two (2) teams in the current season. 9. Must post in advance, a registration fee of $50 per team. Check is preferred made out to: OCJAAF. 10. Shall accept responsibility for all indebtedness incurred in the purchase of equipment, uniforms and other items necessary to field teams. 11. 4 NEW CHAPTER QUESTIONNAIRE On a separate sheet please answer the following questions. 1. Name of new chapter organization. (The name in which you will also file with the state and federal government). 2. Complete CHAPTER REGISTRATION FORM; see copy attached to this packet. 3. Number of teams and what division you will field in the current season. If your head coaches have already been appointed for the current season, please complete the attached TEAM REGISTRATION FORM. 4. Will you field cheer teams in the current season? If so, how many and for what division? 5. What location will your practices and games be held? (Please provide name of field/facility and complete address). Please complete form attached: FIELD REGISTRATION FORM. 6. Are there any members on your current Board of Directors who have been dismissed from another youth sport program? If so, please provide name, position and explanation. 7. If you are an existing organization, please advise from what organization you were previously associated with and reasons for leaving? 8. It is particularly important that we know you are organized and have substantial support from a complete board. This is not a job for one person and we will require at minimum an individual dedicated to the positions as indicated in item #4 on page 1 of this application. Furthermore, the OCJAAF shall require additional members appointed to your board responsible for the following: uniforms/equipment, fundraising, scholastic, cheer, football coach s representative, roster program coordinator, ID photographer and a chapter player agent. These are key positions requiring specific responsibilities to ensure you are compliant with OCJAAF Rules. A mandatory OCJAAF Orientation is scheduled in mid-may of each New Year, whereby members of your Executive Board and some of those mentioned above, must be in attendance. There are four (4) specific orientation groups to address these roles and responsibilities. What is the current size of your board, and will you be expanding the size of your current board? Do you foresee any problems or conflicts which would prevent your chapter representatives from participation at the April Orientation? 9. The financial recordkeeping of a nonprofit, charity organization must be kept current and regular reports must be filed with your board members at each regularly scheduled board meeting. Which system of recordkeeping will your organization keep? (Manual, Computerized, other?). What is the current status of your financials? Do you have a budget for the New Year? If so, please attach a copy to this application. Will you require help and assistance in the area of financial recordkeeping? What types of fundraising will your organization conduct in the current season? OCJAAF NEW CHAPTER APPLICATION Page 2 of 3
5 10. The OCJAAF maintains a current web site to provide communication to all members: chapters, staff and parents. Does your chapter currently have, or will make plans to build a web site for your organization? If so, what is the current address of your web site? Also, for the purpose of our web site, do you have a.jpeg file of your logo? (If so, please send it to our OCJAAF Web Master, Lora Gonzalez via to: ocjaafwebmaster@gmail.com). In 2016 the OCJAAF is moving towards a more cohesive and complete Website/Roster Program and Scheduling/Scores & Standings designed for complete administration of and managing a youth sport program. It is highly recommended that each OCJAAF Member utilize this free website as a means to access the Roster Program/Scheduling capabilities. (See the attached BLUE SOMBRERO/HQ, Dick s Sporting Goods handout for more information and go online to check them out and us at OTHER POINTS OF INTEREST The following are requirements of membership, whereby the Chapter-Member shall be financially responsible for paying for this service direct to the provider. 11. The OCJAAF shall require all volunteer/staff to submit to a BACKGROUND CHECK by simply entering basic information online. Each Chapter-Member will acquire their own account for obtaining a background check program, and currently we are using the services provide by Protect Youth Sports at: or you can call them ( ) to establish your account now, with an initial set-up fee of $200. We require a BASIC background check; cost is about $7.50 per person. You will be responsible for provide the OCJAAF with regular background reports on cleared members for your chapter. All volunteers 18 & older must submit to a background check clearance. 12. The OCJAAF also requires each Chapter-Member to provide their own insurance & medical plan for their participants. We suggest that you use the services through GAGLIARDI INSURANCE SERVICES. See attached list of basic requirements as a member of OCJAAF also indicated in our Rule Book,Article II-Player eligibility, Section C-Insurance, Item #1-3. You can find Gagliardi Insurance services Online at: Please provide an insurance certificate in the name of Orange County Junior All American Football: 2319 Songbird Lane, Rowland Heights, CA The OCJAAF Executive Council meetings are held the first Tuesday of each month, except where noted in our calendar (SEE CALENDAR COPY ATTACHED), and requires the attendance of all Chapter Presidents. 8 Article II: PLAYER ELIGIBILITY Section: C-INSURANCE Item: #1-#3 Page: 9 Vote: 12/16 RULE CHANGES FOR Each Chapter shall have at least one (1) individual identified as the Chapter Insurance Representative. Said representative s name shall be submitted to the OCJAAF by the January Meeting per the Chapter Registration Form, and no later than the March Meeting. 2. Each Chapter Member shall be responsible in providing their own program coverage at the following minimum coverage plans. Chapter Insurance Program Must Meet These Basic Standards of Coverage: a) The OCJAAF is also named as a additional insured organization. (Requires separate Certificate) b) $5,000,000 General Liability Aggregate Limit c) $1,000,000 General Liability per Occurrence Limit d) $1,000,000 Personal and Advertising Injury Limit e) $2,000,000 Products/Completed Operations Aggregate Limit f) $ 300,000 Coverage for Damage to Rented Premises Limit g) $1,000,000 Non Owned and Hired Auto Limit h) $2,000,000 Abuse & Molestation Coverage Aggregate Limit i) $1,000,000 Abuse & Molestation Coverage per Occurrence Limit j) $1,000,000 Participant Legal Liability k) $ 100,000 Medical Benefit Option l) $ 25,000 AD&D Benefit Limit 3. The current certificate of insurance shall be provided to OCJAAF at the MARCH OCJAAF Executive Council Meeting, at the time of Renewal of Franchise, and a current one if the insurance renewal plan is after March. 4. A player requiring the services of a doctor (illness or injury) cannot return to practice or game play until he has written release from the doctor. The release is to be attached to the team copy of the Player Contract and shall be available for review by any OCJAAF official, upon request. The Head Coach is responsible for ensuring that the written releases are obtained. OCJAAF INSURANCE REQUIREMENTS
