(prohibited in CA, NY, FL, PA)

14 Introducing Our New Toxicology Test Kit Ø Similar to the DNA test kit. Ø Specialized toxicology test kit with a unique NDC code. Ø Doctors cost is $85/kit. Ø Doctors profit is approximately $200/test. This is a huge new revenue stream opportunity for the doctors office and the rep!

15 How do doctors know which kit to use? To use the wet kit, the kit must be prior authorized. The process to get authoriza;on is through our PBM Our PBM Pharmacy Benefits Manager- is Vantage Rx. Rep supplies the forms to the doctors office to get signed up. Set up cost with Vantage Rx is $850 Then $50/month. Trestles covers the $850 sign- up. Reimbursed from first payments. Once set up with Vantage Rx, the doctor will know immediately which pa;ents are covered for wet kit. Vantage Rx authorizes and then bills for the kit for the doctor. Dr. paid by insurance company, not by lab.
