2 Testing/Self Promotion Model Release The test photographic or other Images created by ( Photographer ) of me are only for testing, Photographer's self-promotion, and my portfolio. For valuable consideration, the adequacy and receipt of which is hereby acknowledged, I hereby grant to Photographer the right to use the Images for purposes of Photographer's testing and self-promotion, only. Any other use of the Images by me or Photographer will require negotiation and the written consent of me and Photographer prior to use. For Valuable consideration, the adequacy of which is acknowledged, for your photographing me, and knowing you have and will expend substantial expenses and time in reliance upon this Release and Consent, I irrevocably and absolutely consent to the unrestricted use by and its/his successors, assigns, and designees ( photographers ), and those acting with photographer s permission and authority, of my name and any and all photographic or other images ( Images ) of me that photographer creates or makes, for all purposes, in any form, and in any and all media, including, without limitation, advertising, solicitation, or trade. 2. I waive any right to inspect or approve the finished Images, advertising copy, text, or other printed matter that may be used in conjunction therewith, or to the eventual use that the Images may be applied. 3. I release and discharge Photographer and those acting under Photographer s authority from any and all liabilities, claims and demands arising out of or relating to any blurring, distortion, or alteration whether intentional or otherwise, that may occur or be produced in connection with the Images, or in connection with any processing, alteration, transmission, display, publication or other use of the Images. 4. 4 MODEL RELEASE Model Release and Consent Agreement Date Model Subject Description: For valuable consideration (receipt of which is hereby acknowledged): I hereby give (Photographer) the absolute, irrevocable right and permission, forever and throughout the world, in connection with the photographs he has taken of me (or the minor listed above), or in which I may be included with others, along with the following: (A) The right to use, reuse and distribute, in any manner, said photographs, in whole or in part, either by themselves or in conjunction with other photographs, in any medium and for any purpose whatsoever, including without limitation, all promotional, stock, advertising uses, and other trade purposes, in any manner or medium, whether now or hereafter devised, as well as using my name in conjunction therewith, if he so desires, and (B) The right to copyright these photographs in his own name or any other name that he may select. I understand that the photographer may contract with a stock photo agency and the above described photograph(s) may be included in stock files. I hereby forever release and discharge the Photographer, his heirs, legal representatives, agents, assigns and the party for whom the photographs were made, from any and all claims, actions and demands arising out of or in connection with the use of said photographs or film footage, including without limitation, any and all claims for invasion of privacy and libel. As further consideration for my grant of right, title and interest herein, the photographer agrees to divide pro rate among the models depicted, 10% of his net collected proceeds which exceed $1, per individual license or sale. I hereby warrant that I am of full age and have the right to contract in my own name. I have read the above authorization, release and agreement, prior to its execution, and am familiar with the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns.
