To avoid this unfortunate situation we offer you to have our notifications being sent to your mobile phone as text messages. Price for this service is: 1.00 +HST, and it will be added to the total service charge. We shall be using the data provided by you in your SMS application form solely and exclusively to send your application status by SMS to your registered mobile number, and to send you these details also by e-mail at the indicated e-mail address. We may also provide your information to any government or government agency in connection with the processing of your application. BY SIGNING THIS FORM YOU ARE NOTIFYING US THAT YOU CONSENT TO RECEIVING THIS INFORMATION BY SMS AND EMAIL. Please note that if you give your consent, you have the right at any time to withdraw this consent by notifying us at VisaCenter by email info@visacenter.ca. Disclaimer and Terms and Conditions 1. The Short Messaging Service ( SMS ) provided to Visa Applicants is to update on current status to the applicant with regards to Visa Application Process. The information provided by SMS is based among others on the information provided by the Embassy/Consulate. While VisaCenter ensures that information is made available to the visa applicant promptly and accurately, the Visa Applicant shall at all times be responsible for providing and maintaining an SMS text compatible mobile phone number where VisaCenter can send text messages regarding the status of the Visa Application to the Visa Applicant. The Visa Applicant is responsible for ensuring connection to a mobile network capable of receiving the SMS Services. The Visa Applicant understands that if The Visa Applicant s mobile phone is switched off, disconnected for any reasons or is out of coverage for a period of 24 hours or more, The Visa Applicant s mobile network provider may delete any SMS messages to be received by The Visa Applicant. 2. The Visa Applicant confirms that the Visa Applicant has provided the accurate mobile number for receiving SMS and that the Visa Applicant is the owner or its legitimate user, or that the Visa Applicant has the consent of the owner or legitimate user, of the mobile phone using for the Services. The Visa Applicant acknowledges that using another person's mobile phone/providing inaccurate mobile phone number/unauthorized use of mobile phone number for receiving the SMS may entail disclosure of Visa Applicant s confidential information which disclosure shall be at the sole risk of the Visa Applicant 3. The Services, once obtained, will be available to the Visa Applicant from the time of application till complete processing of the application or such other period as VisaCenter may advise via our website located at www.visacenter.ca. VisaCenter reserves the right to withdraw this service at any time and without notice. 4. The Visa Applicant must not use (or permit any third party to use) the SMS Service to send any message or communication which is Spam, illegal, offensive, abusive, indecent, obscene or menacing or causes annoyance, inconvenience or needless anxiety or infringes the rights of third parties. VisaCenter reserves the right to withdraw the SMS service to such Visa Applicant if The Visa Applicant is in breach of this paragraph 4. VisaCenter may also withdraw the Services if VisaCenter in its sole discretion apprehends that the SMS service is being used for such purposes. 5. For operational reasons VisaCenter may vary the technical specification of the Services with or with out notice. In the event of any change to the Service these Terms and Conditions is to be treated as varied accordingly. 6. The time to deliver the SMS is dependent on several factors such as upon the traffic on the mobile network and whether The Visa Applicant mobile phone is within reach and switched on and cannot therefore be guaranteed by VisaCenter. VisaCenter is not a mobile network operator and does not guarantee the delivery of SMS text messages. 7. The Visa Applicant acknowledges that the SMS Services may, at any time, be adversely affected by problems with The Visa Applicant mobile phone network, type of mobile account, force majeure events including, without limitation, interference to the network coverage. VisaCenter is not responsible or liable to The Visa Applicant for any loss, damage or expenses incurred directly or indirectly by The Visa Applicant as a result of any difficulties experienced by the Visa Applicant s mobile phone service provider. Subject to the constraints described within this paragraph 7, VisaCenter shall carry out the services with reasonable care and skill. 8. If The Visa Applicant does not receive SMS relating to these Services. the Visa Applicant should inform VisaCenter through email. 9. The SMS service delivers the Visa Application Status based on the information received by VisaCenter from Embassy/Consulate. 10. The Visa Applicant agrees to comply with all instructions we may give concerning the Services, including any security instructions. We will be entitled to treat any failure by Visa Applicant themselves to comply with these instructions as a breach of these Terms and Conditions, which will entitle us to deny Visa Applicant access to the Services. 11. The Visa Applicant is responsible for paying charges for the message origination. Charges once paid for SMS service will not be refunded under any circumstances. 12. The Visa Applicant shall at its sole risk be responsible for taking all reasonable steps to prevent unauthorized persons gaining access to the Services. 13. While VisaCenter takes every precaution transmission of information to transmit information. VisaCenter shall not be responsible or liable for any unforeseen events and circumstances beyond the reasonable control of VisaCenter. 14. VisaCenter may in its sole discretion temporarily suspend the provision of the Services if such provision could materially affect the quality of any telecommunications service, including the Services, provided by VisaCenter. 15. Q2 VISA - FAMILY VISA - Issued to those who intend to visit relatives who are Chinese citizens residing in China or foreigners with permanent residence status in China; the intended duration of stay is limited to no more than 180 days. (1) An invitation letter issued by a Chinese citizen or a foreign citizen with a Chinese permanent residence permit who lives in China. S1 VISA - VISITING RELATIVES VISA - Issued to relatives of foreigners working or studying in China (having study or work permit, but do not have permanent residence status in

China) for the purpose of long-term visit, or to those who intend to visit China for other private reasons (intended duration of stay exceeding 180 days). Relatives refer to spouses, parents, sons or daughters and parents-in-law. S2 VISA - VISITING RELATIVES VISA - Issued to those wishing to visit family members that are foreigners working or studying in China (having study or work permit, but do not have permanent residence status in China), or to those who intend to go to China for other private reasons. The intended duration of stay in China is limited to no more than 180 days. "Family members" refers to spouses, parents, sons, daughters, spouses of sons or daughters, brothers, sisters, grandparents, grandsons, granddaughters and parents-in-law. For private affairs, documentation identifying the nature of the private affairs should be provided as required by the consular officer. G VISA - TRANSIT VISA - Issued to those who are going to transit through China en route to a third country (or region).

An onward air (train or ship) ticket with confirmed date and seat to the destination country or region. C VISA - CREW VISA - Issued to foreign crew members of aircraft, trains and ships, motor vehicle drivers engaged in cross-border transport activities, and also to the accompanying family members of the above-mentioned ships crew members. A letter of guarantee issued by a foreign transport company or an invitation letter issued by a relevant entity in China. X1 VISA - STUDENT VISA - Issued to those intending to study in China for a period of more than 180 days. (1) Original and photocopy of the Admission Letter issued by a school or other entities in China. (2) Original and photocopy of "Visa Application for Study in China" (Form JW201 or Form JW202). Holders of X1 Visa shall, within 30 days from the date of their entry, apply to the exit/entry administrations of public security organs under local people's governments at or above the county level in the proposed places of residence for foreigners' residence permits. X2 VISA - STUDENT VISA - Issued to those intending to study in China for a period of not more than 180 days. Original and photocopy of Admission Notice issued by a school or other entities in China. R VISA - INTRODUCED TALENT VISA - Issued to those who are high-level qualified talents or whose skills are urgently needed by China. The applicant should submit relevant certification in accordance with relevant regulations, and meet the relevant requirements of the competent authorities of the Chinese government on high-level talents and individual with special skills urgently needed by China. J1 JOURNALIST VISA - Issued to resident foreign journalists/media staff of foreign news organizations stationed in China. The intended duration of stay in China exceeds 180 days. Visa Notification Letter issued by the Information Department of the Ministry of Foreign Affairs of China and an official letter issued by the media organization for which the journalist works Applicants should contact Inter-government Relations and Press Office of the Consulate General in advance and complete relevant formalities. Holders of J1 Visa shall, within 30 days from the date of their entry, apply to the exit/entry administrations of public security organs under local people's governments at or above the county level in the proposed places of residence for foreigners' residence permits. J2 JOURNALIST VISA - Issued to foreign journalists/media staff on temporary news coverage missions. The intended duration of stay in China is limited to no more than 180 days. Visa Notification Letter issued by the Information Department of the Ministry of Foreign Affairs of China or other authorized units in China and an official letter issued by the media organization for which the journalist works. Applicants should contact Inter-government Relations and Press Office of the Consulate

General in advance and complete the required formalities. D VISA - PERMANENT RESIDENT VISA - Issued to those who are going to reside in China permanently. The original and photocopy of the Confirmation Form for Foreigners Permanent Residence Status issued by the Ministry of Public Security of China. Holders of D Visa shall, within 30 days from the date of their entry, apply to the exit/entry administrations of public security organs under local people's governments at or above the county level in the proposed places of residence for foreigners' residence permits. NOTE: If you apply at one of our locations it is recommended to bring with you already filledout application form. For rush processing all documents must be received by Head Office: before 8.45 am of the processing day and will be available for pick up next business day by 4.30pm. (Toronto Location only). For child visa original birth certificate (long form) is required. Holders of official passports must personally apply at the Chinese Embassy. Applicants with passports of African countries as well as passports of Afghanistan, Iraq and Iran must present the invitation letters in original. Applications of the passport holders of Iran, Nigeria and Pakistan will take longer time than indicated. China Business Visa Requirements (additionally to the requirements above) F VISA - EXCHANGES, VISITS, STUDY TOURS AND OTHER ACTIVITY VISA - Issued to those who are invited to China for non-profit cultural and academic exchanges, conference attendance, study tours, compete in non-commercial games and other activities. VISACENTER DOES NOT PROCESS F VISAS. APPLICANTS MUST SUBMIT THEIR APPLICATIONS TO CHINESE CONSULATE DIRECTLY. An invitation letter issued by a relevant entity or individual in China. M VISA - COMMERCE AND TRADE VISA - Issued to those who are invited to China for commercial and trade activities (visiting clients, suppliers, visiting company branch or headquarters, handle corporate related business). (1) Documents on the commercial activity issued by a trade partner in China, or trade fair invitation or other invitation letters issued by relevant entity or individual. The invitation letter should contain: a) Information on the applicant (full name, gender, date of birth, etc.). b) Information on the planned visit (purpose of visit, number of intended entries into China, arrival and departure dates, place(s) to be visited, relations between the applicant and the inviting entity or individual, financial source for expenditures). c) Information on the inviting entity or individual (name, contact telephone number, address, official stamp, signature of the legal representative or the inviting individual).

To qualify for Multiple Entry Business visa applicant must have completed two entries into China within the last 24 months or have previously expired multiple entry visa in last 12 months and invitation letter should explicitly state that multiple visa is required. Z VISA - WORK VISA - Issued to those who intend to work in China (those taking up a post or employment, or giving commercial performances in China) One of the following documents: (1) Foreigners Employment Permit of the People's Republic of China issued by Chinese government authorities for Human Resources and Social Security, as well as Invitation Letter of Duly Authorized Entity or Confirmation Letter of Invitation issued by relevant Chinese entities. (2) Permit for Foreign Experts Working in China issued by the State Bureau of Foreign Experts as well as Invitation Letter of Duly Authorized Entity or Confirmation Letter of Invitation issued by relevant Chinese entities. (3) Registration Certificate of Resident Representative Offices of enterprises of foreign countries(regions) issued by Chinese authorities of industrial and commercial administration, as well as Invitation Letter of Duly Authorized Entity or Confirmation Letter of Invitation issued by relevant Chinese entities as well as Invitation Letter of Duly Authorized Entity or Confirmation Letter of Invitation issued by relevant Chinese entities. (4) An approval document for commercial performances issued by the Chinese government authorities for cultural affairs or Invitation Letter of Duly Authorized Entity or Confirmation Letter of Invitation issued by relevant Foreign Affairs Office of provincial governments of China. (5)Letter of Invitation to Foreigners for Offshore Petroleum Operations in China issued by China National Offshore Oil Corporation; Holders of Z Visa shall, within 30 days from the date of their entry, apply to the exit/entry administrations of public security organs under local people's governments at or above the county level in the proposed places of residence for foreigners' residence permits.
