1 102 4/10/2012 Tampering with Fire received a call from an RA reporting a fire extinguisher missing from a residence hall floor. Later that evening a RA reported to CPS that she recovered a fire extinguisher at the Boxer Statue at Vandervelden Apartments. CPS Officer found that the fire extinguisher was discharged on the Boxer statue. No further damage was located. No suspects at this time /3/2012 Staff member reported to that she was feeling very nauseous and dizzy. Staff member stated she had a severe ear infection and denied EMS services. Staff member said that her husband would pick her up and transport her to the emergency room for further medical evaluation /03/2012 Theft III $ Student report that her keys with her student ID attached were stolen from a classroom in Strain Hall. The keys had been left unattended with the student s bag in class room. No suspects identified /4/2012 Criminal Trespass II CPS Officers witnessed suspicious activity at concession stands at Lincoln Park Athletic Complex. Officers made contact with suspect and Identified him as a subject who had been previously Trespassed from University property. FGPD Officers responded, cited, and released suspect for Criminal Trespass II /04/2012 Motor Vehicle Theft Student reported their vehicle missing from E lot and possibly stolen. A check of campus could find no sign of the vehicle. Police were called and it was discovered that the vehicle had in fact been stolen and had been recovered by police in North Plains. Vehicle had been abandoned and set fire and is a total loss. Report filed with FG Police /6/2012 Harassment Female student contacted CPS to file a harassment complaint against a male student. Female student and male student were interview by CPS Officers. Male student was advised to not have any further contact, of any kind, with the female student. Female student was advised to contact CPS immediately if the male student continued to pursue contact. Pertinent staff was contacted, advised of the situation and will follow up with both students.

2 108 4/6/2012 Drugs / Alcohol Odor of marijuana was detected in the third floor hallway of Clark Hall. CPS Officers made contact with students after determining where the odor was coming from. Three students admitted to smoking marijuana. Marijuana, paraphernalia and alcohol were confiscated. Alcohol was disposed of at the scene /06/2012 Drugs, Criminal Trespass CPS made contact with 3 non-student juveniles at Cannery Field. One of the juveniles fled the scene, one was released, and the other detained until the Police Department s arrival. The male detained was found to be in possession of an illegal switch blade style knife, and police suspected the male to be in possession of trace amounts of heroin, and THC. CPS issued the male a Trespass Notice. Police confiscated the suspected heroin, and the knife and released him to his own recognizance /7/2012 Drugs/Alcohol CPS was contacted by Resident Assistants on duty in reference to the odor of marijuana in the second floor, west hallway. CPS Officers were able to determine the room where the odor originated. Marijuana and paraphernalia were confiscated. Alcohol was also found in the room and poured out by resident. Students referred to Conduct Review Committee /07/2012 found their cart had been tipped onto it s side outside a residence hall. Officers and a student righted the vehicle. Minimal damage was done to the vehicle. No suspects at this time /07/2012 Assault IV / Harassment was called by a Resident Assistant who reported a possible physical altercation occurring in a residence hall. Campus Public Safety arrived and separated the male and female parties who were reportedly involved. It was determined that an Assault IV / Harassment did occur but the female victim declined to file criminal charges against the male. Both parties mutually agreed not to have any further contact for the rest of the evening. Report forwarded to the Conduct Review Committee /07/2012 Alcohol was called by a Resident Assistant regarding a female student that was unresponsive, had been vomiting and appeared to be intoxicated. Student was assessed by fire and medical personnel and was determined to be medically stable. Student refused transport to the hospital. seized and destroyed alcohol from the room per protocol.

3 114 04/08/2012 Drugs CPS contacted two students smoking marijuana in a university parking lot. CPS Officers seized a marijuana pipe and small amount of marijuana as evidence from the students. Students were referred to a conduct advisor for review /9/2012 Alcohol received a text message from WCCCA in regards to a Alcohol Overdose Poisoning at Vandervelden Court Apartments. CPS Officers arrived to find victim conscious at courtyard bench with a FGPD Officer and victim s friends standing by. RA on Duty called 911 and a Metro West ambulance arrived shortly after. Victim denied further medical treatment and signed patient release 116 4/9/2012 Theft III $ Virginia Garcia Clinic Manager PEDRAZA reported a possible theft of cash bag from clinic. Bag last seen on 4/5/ hours /10/2012 Criminal Trespass II CPS Officer contacted a suspicious male inside Murdock Hall. Suspect was found to be in violation of a current trespass warning. The CPS Officer arrested the male and turned him over to Forest Grove Police. Suspect was cited for Criminal Trespass II and released /11/2012 Property Damage Officer located excess offensive graffiti on the ground, grass and trees surrounding the Spirit Bench. No suspects identified /13/2012 Vehicular Incident Staff member called to report that she struck a skateboarder with her vehicle in C-lot. The skateboarder, after being hit, took off and was not located. The was no damage to staff member vehicle and no obvious injuries to the victim.

4 120 04/13/2012 Staff member reported to have been injured at the University Center. Found custodian who had been struck in head by a trash can lid. First Aid provided. Patient taken to personal physician by family member. Accident report submitted /15/2012 Theft III (Recovered) $ On 4/15/12 (V) KUMPEL reported that her bicycle that was secured to the railing outside of her apartment at Vandervelden Court was stolen sometime since 1900 hours on 4/14/12. There are no suspect information at this time /16/2012 $ received multiple reports of offensive chalk written statements throughout campus. Custodial was notified and is addressing the cleaning aspect of the incident. There are no suspect(s) at this time /17/2012 Suspicious Circumstances Staff member reported to a suspicious circumstance that occurred 4/12/12. Some one was seen taking photo s of students in Creighton Hall 3rd floor, from the 4th floor (room 430) of building 2 across the street /18/2012 Lost or Stolen Property $1, Student came to CPS office to report that her wallet was stolen. The student thinks she dropped it in the parking lot as she was getting her keys out of her purse. Later that evening her credit card company called to inform the student that there are number of charges on her card. No suspects identified at this time /18/2012 $ received a report of a bike rack and trash can at the Soccer Stadium that had been spray painted white. A report was completed for documentation purposes. There are no known suspects at this time.

5 126 04/19/2012 Drugs CPS located an odor of marijuana coming from a academic building. After search of the the building the odor stemmed from an office, the office had a smoky haze about it, CPS officers located burnt marijuana joints and residue in a trash receptacle. The burnt joints and residue were seized for evidence /20/2012 Alcohol, Drugs, CPS Officers came across a male who was non-responsive. Officer s were able to wake the student who was found to have overdosed on alcohol and marijuana laced cookies. Police, and emergency medical personal responded. The student was transported to the hospital via ambulance /21/2012 Officers responded to a report of an ill student in a residence hall. Fire and EMS units were on scene. Medical aid provided and student transported by ambulance to Tuality Hospital /22/2012 Tampering with Fire Equipment $74.99 was contacted by a student who reported that another student had stolen a fire extinguisher from a residence hall. Student suspect was contacted by and the fire extinguisher was returned to the University. Student referred to Conduct Review. 130 $25.00 Facilities staff reported graffiti on the back of a map kiosk located at the corner of Pacific Avenue and Cedar Street. Unknown when the graffiti appeared and there are no suspect information at this time. A work request has been issued to clean off the graffiti. 131 On 4/23/12 (RP) CARREON-SERNA reported that (V) GOSSACK fell while skateboarding near Warner Hall. GOSSACK had minor lacerations to palms of both hands and the exterior of his left knee. GOSSACK was treated at the scene and released.

6 132 $70.00 CPS notified of vandalism to the women s restroom at the baseball complex. No suspects identified. 133 Theft III $ Student reported theft of bicycle from the Library bike rack. Bike was not secured. No suspects at this time. Referred to FGPD. 134 Alcohol & Drugs Resident Assistants reported the odor of burnt marijuana coming from Residence Hall hallway. CPS Officers responded and located the source. CPS officers confiscated marijuana, alcoholic beverages, and drug paraphernalia. Suspect referred to the Conduct Review Advisor /24/2012 CPS responded to a report of an injured student after they were struck by a door. First Aid was provided. Student refused further care / transport /26/2012 Campus Pubic Safety received a call of a elderly female that fell on the walk way on the south side of Jefferson Hall. The patient refused to have EMS called or to be transported /26/2012 Criminal Trespass II made contact with a non-student juvenile male who had been previously trespassed. Officer attempted to arrest the male for trespass at which point the male became non compliant. Officer took the male to the ground and placed him in handcuffs. Police responded, charged the juvenile male with trespass, and released him to the custody of his father.

7 138 4/28/2012 received call from student requesting ice pack for injured wrist. Ice pack was given to student for swollen right wrist due to a fall from tripping in stairwell eight hours earlier. Student said she was going to her urgent care provider this morning for medical attention /29/2012 Disorderly Conduct II / Assault was called by anonymous male regarding an argument between a male and female student occurring outside of a residence hall. Caller stated his concern the verbal argument was going to become violent. Officers located the female student and determined that a physical altercation had occurred between both parties. Police arrived and interviewed the male and female involved. After investigating police determined that the female student was the aggressor in the incident. No charges were filed by police /3/2012 Dean of Pharmacy reported to that a student poked her finger with needle while practicing injections in class. No first aid was given. Accident report form sent to student. Sharps protocol followed by Pharmacy staff.
