In consideration for your allowing me to participate in the event, I understand and agree to the following: I represent that the above information concerning me is accurate, and that at the time of my entry in the contest, I was years of age, and that neither I nor any member of my family was or is an employee of the station, its owner, parent, subsidiaries, affiliates, or advertising, promotional or judging agencies or sponsors of the contest. I have read, received an explanation on any of my questions concerning, and understand the official rules of the The Key & KFFX Fox 11 s Junior Idol contest and I represent that I have complied fully with these rules, and that I understand that any failure to comply could result in my ineligibility to compete or to receive any prize or in the revocation of any prize. I warrant that I am in good health and have no physical condition that would prevent me from participating in the event. I fully realize, understand and acknowledge the dangers of participating in the event which involves a karaoke contest and that no amount of care, caution or expertise can eliminate such dangers. I EXPRESSLY AND VOLUNTARILY ASSUME RESPONSIBILITY FOR ALL RISK OF PERSONAL INJURY OR DEATH SUSTAINED WHILE PARTICIPATING IN THE EVENT WHETHER OR NOT CAUSED BY THE NEGLIGENCE OF STATION AND/OR ITS EMPLOYEES OR AGENTS. I hereby acknowledge receipt of the event and hereby accept same as my prize as a winner in the contest on station. I am responsible for any taxes and fees of any type associated with my receipt and/or use of any prize including, without limitation, income taxes and sales and use taxes, and any expenses associated with any prize, which are not specifically provided for in the official rules. I authorize the Station to withhold from any cash prize any amounts required to be withheld pursuant to federal, state or local law. I, for myself and my personal representatives, heirs, successors, and assigns, in consideration for being involved in the event, hereby hold harmless, release, defend, indemnify and discharge from liability and waive any claim which I might state against any person or entity involved with the event including but not limited to, the sponsoring organizations, GAP Broadcasting Tri-Cities, LLC, KFFX Fox 11, Ranch and Home, Benton Franklin Fair and their respective subsidiaries, affiliates, officers, directors and employees, volunteers at the event, and individual property owners from all liability, claims, judgments, demands, controversies, agreements, damages, actions, and causes of action whatsoever, either in law or equity, no matter what the cause of my loss, including the negligence of the above named parties, which I have or may have at any time arising out of my association with, travel to or from, or participation in the event or any of its associated activities, or are in any other way related to or resulting from the event or any prizes awarded in connection therewith. I further agree to indemnify and hold harmless any of the above named parties from any claim arising out of my participation in the event, even if the claim arises out of their negligence. I acknowledge that station and the officers, directors, agents and employees of GAP Broadcasting Tri-Cities, LLC, are not acting as the

4 manufacturer or distributor of the event. Station, acting as a consumer, acquired the event for the purpose of awarding it to a winner in the contest. With respect to any claims I may have as a result of the cvent, I covenant that I shall look solely to its provider, or others in the chain of production and distribution but excluding station, GAPWEST Broadcasting, its employees and/or affiliates. I acknowledge that the station has not made any warranties with respect to the event. I understand and agree that the sponsors of the event may subsequently use for publicity and/or promotional purposes, my name, likeness, photographs, videotapes or other recordings of me participating in the event, without any consideration to me. I consent and permit emergency treatment, medical or otherwise, in the event of injury or illness to myself. I further release all persons associated in any manner with the event from any claim whatsoever on account of first aid, treatment or services rendered me during my participation in the event. I agree the release I have granted herein shall extend to and protect the agents, employees, representatives; assigns and successors in interest of the parties enumerated in paragraph three above. I certify I am over the age of 18 and am legally competent to sign this release and understand that the representations are contractual and not mere recital. If not over the age of 18, my parent or legal guardian has read and understood this agreement in its entirety. I have fully informed myself of the contents of this release and I have signed this document of my own free act. I acknowledge and agree that I have read and fully understand all of the provisions of this agreement, that I have been given the opportunity to review all of the aspects of this agreement with an attorney, and that I knowingly accept all of the terms of this agreement in their entirety and without reservation, and that I may not participate or have a chance to win any prize if I violate those rules. This release and any claims associated with it or the event shall be construed according to Washington law, and venue and jurisdiction shall only be proper in the District Court for the County of Franklin, State of Washington. If any provisions of the within release shall be deemed to be unenforceable and void, the remainder of the release shall not be affected thereby and the remainder of the release shall be fully enforceable. 5 Jr. Idol - Contest Rules All participants must fill out an official application. Complete the form and submit to GapWest Broadcasting; 2621 W. A Street, Pasco, WA by 5pm July, 31 st. This contest is open to amateur competitors only excluding all employees of GapWest Broadcasting and Fox 11 Television. Contestants restricted to 5-16 years of age. Entrants shall not have released any songs to any radio stations prior to competing in this contest. All entrants under the age of 19 years of age must have parental consent and be accompanied by an adult for the preliminary and for performance night at the Benton Franklin Fair & Rodeo. All entrants must perform as a single act. No groups or duos will be allowed. Entrants choosing to perform with props must supply their own equipment. All entrants are required to perform one (1) song for the competition. Each song performed in the contest cannot exceed 4 minutes. In the event any song runs longer than 4 minutes, the performance will be faded out at 4:05. The entrant is responsible for providing music to perform with in the form of a CD. If you supply your own music bed only CDs will be accepted NO Cassettes! In the event your own music fails or skips during your performance, that song will be scored accordingly and no restart will be given, unless due to faulty equipment. All entrants are required to be at the contest location no later than ½ hour prior to the competition/performance. The preliminary competition starts at 9 am at Ranch & Home in Kennewick. Performer s participation is not confirmed until the entry form has been received and confirmed by GapWest Broadcasting. A maximum of twenty (20) performers will be accepted to perform in each division. GapWest Broadcasting is responsible for the selection of judges. A minimum of three (3) judges will be provided for each competition, and all judges decisions are FINAL! Score sheets are the property of GapWest Broadcasting and will not be shared with anyone other than the contest officials. Each competitor will be scored by each judge individually. The judges will use the following score sheet based on the following criteria. There are five categories with up to 5 points earned in each. A total score of 25 will be considered a perfect score. THIS IS NOT A KARAOKE EVENT. TV monitors will not be supplied or allowed.
