2 Enter your current residing address if it is not the same as the address which will be printed on the passport. Enter your valid phone number, mobile number and address which will be point of contact in case of document related enquiry. Enter your Father s and Mother s Name as mentioned in your passport. Enter country and nationality of your mother and father Enter passport number as mentioned in your old/existing passport. Please enter the issue date and place of issue as mentioned in your old/existing passport Click to save the details and exit in case you wish to fill the form later. Note your Temporary ID. In case of minor, thumb print as per specification is required. Question no. 3: Permanent address (a) and (b) must be different from each other. You must provide both India and USA addresses Fill the form with blue/ black pen from point 7 to 29 and if any of the questions is not applicable, please write N/A in the corresponding space. Please enter name and address of relative or friend residing in India or USA. Enter USA driving licence number. Enter date when you visited India previously. Enter the approximate date when you left India for the first time to go to USA and not any other country other than USA.

6 Enter the total duration (months/ years) you have lived continuously in USA. In between, if you have visited/ resided in location other than USA then exclude that period. Example If you have stayed in US for 3 years and visited Dubai for 4 months then enter duration as 2 years 8 months. Passports with endorsement ECR means Emigration check is required (check 2 nd or 3 rd page of the passport, select ECR if it specifies ECR). Passports with endorsement ECNR means Emigration check not required (check 2 nd or 3 rd page of the passport, select ECNR if it specifies ECNR or if nothing is specified). Enter USA and India reference address. Enter details if you are part of any Consulate or Embassy. Enter details of child/children to be removed from your existing passport. Applicant s signature/ in case of minor, thumb print as per specification. Your current city and current date. Provide both parents details in case of Minor applicant only. In case of minor, thumb print as per specification is required.

7 In case of Minor applicant, both parents signature is mandatory. Write your current city and current date. Applicant (minor as well) needs to sign inside the boxes. Common Mistakes to avoid in your Online NRI Passport Application on the Government of India Website: 1. CAPITAL LETTERS must be used to fill the Online NRI Passport Application form on the Government Website. 2. All details must be filled MANUALLY using black/blue ballpoint pen in the Online NRI Passport Application form from Q.7 Particulars of person to be intimated in the event of death or accident. Handwrite all details between Q.7 and Q29. Do not leave any fields blank. 3. Please Sign the Passport Application Form wherever indicated before submission. 4. Ensure that the signatures on the passport and on the Application Form are the same. 5. Ensure that the following details are the same as mentioned in your old passport. Ensure that in the Place of Birth field, you do not put India as the city and state. 7. If you have more than 1 original Passport, then you need to provide the copies of all the passports. 8.
