1 NSLI-Y RAF and Bolshoi Way Visa Application Instructions The following information is designed to facilitate your visa application/registration process. Please visit the Russian Consulate s website to familiarize yourself with all visa requirements prior to starting the process. Online Visa Form: Please, follow the step-by-step instructions below. The whole process takes approximately minutes. 1. Please complete the Electronic Visa Application DURING THE PERIOD OF JUNE 10-17, Go to the Electronic Visa Application Form, a web-based application, at 3. The website will assign you an application ID and ask you to pick a password. Please write down your password and Application ID for your records and this information to Maria Kaushansky (mkaushansky@russianamericanfoundation.org). RAF may need to use this information to access your application and make corrections at the request of the Russian Consulate. 4. Once you have completed the application, please print a hard copy, sign it, and bring along with 2 brand new professional passport-sized photos and passport to BBASI NY PDO Session I on June 26, Please note: the online application contains several pages of questions relevant to the Russian visa application. Each time you proceed to a new page, the data in the previous sections is saved. However, just to be sure, it s never a bad idea to click save draft before heading to the next page. Photo requirements (PROFESSIONAL PHOTOS ONLY!!!): Photos must be brand new, 2 x2 color photograph on a white background, professionally done, taken within the last 6 months, with a clear view of the face and half shoulders, as for the US passport. PLEASE MAKE SURE THAT PHOTOS ARE OF GOOD QUALITY AND CLEAR! Passport: there must be at least 2 blank visa pages in the passport. Additional Requirements: All participants must submit a copy a recent HIV test All participants under 18 years old will need to submit the following additional documentation with their visa applications: 1. Copy of birth certificate. 2. Copy of both parents passports. 3. If the mother changed her maiden name, please also submit a copy of the marriage certificate. Hints and help language: English, or your first language. 3. Check the box next to the red text I have read this information. 4. Press Complete new application form and proceed to the next screen. 3 Screen 2: Password 1. Choose a secure password and enter in the box. 2. Enter the same password again to confirm. 3. Make sure you write down your password and send it with your ID Number to Maria Kaushansky at mkaushansky@russianamericanfoundation. 4. Press Submit and proceed to the next screen. Screen 3: Your ID Number 1. Please write down your ID number and send along with password to Maria Kaushansky at mkaushansky@russianamericanfoundation.org 2. 4 Screen 4: Visa Details 1. Nationality: United States 2. Under If you had USSR or Russian nationality at some time please select yes and indicate why you lost it: Select NO. 3. Purpose of Visit (section): select STUDY 4. Purpose of Visit: select STUDY 5. Visa category and type: select COMMON EDUCATIONAL 6. Number of entries: select SINGLE 7. Date of entry into Russia (DD/MM/YYYY): Enter 17-July Date of exit from Russia (DD/MM/YYYY): Enter 27-August Press Next and proceed to the next screen. 5 Screen 5: Personal details Fill this out precisely as it is shown in your passport and double check everything you enter. 1. Enter your Last name (Surname) exactly as it appears in your passport. 2. Enter your First name and middle name(s) exactly as they appear in your passport. (Please note that if you have more than one middle name, you must enter both middle names in this section). 3. Answer the question regarding any other names that have been used. If your answer is YES a new field will appear in the form where you can enter your other names used. 4. Enter your gender as stated in your passport. 5. Enter your date of birth as stated on your passport in the following format dd-month-yyyy. 6. Enter your place of birth exactly as it appears in your passport. 7. Answer the question If you were born in Russia. 8. Enter your marital status as appropriate. 9. Press Next and proceed to the next screen. 6 Screen 6: Passport Details Fill this out precisely as it is shown in your passport and double check everything you enter. 1. Under Type of passport : Select Tourist. 2. Enter your passport number (this is the 9 digit number that appears in the upper right hand corner of your passport). 3. Enter the date your passport was issued in the following format: dd/month/yyyy. 4. Enter the date your passport expires in the following format: dd/month/yyyy. 5. Enter who your passport was issued by exactly as it appears in your passport. (Usually this is: U.S. Department of State). 6. Press Next and proceed to the next screen. 7 Screen 7: Visa Details: 1. Under Which institution you are going to visit? select Organization 2. Name of Organization: Enter Bolshoi Ballet Academy 3. Address: Enter 5 Vtoraya Frunzenskaya, Moscow 4. TIN organization: leave this BLANK 5. Directive (telex) number: leave this BLANK 6. Invitation number: leave this BLANK for now as we are still waiting for the invitations from the Academy. We will add this information to your application later. 7. Itinerary (places of visit): type MOSCOW and select MOSCOW REG. 8. Do you have medical insurance policy valid in Russia? select YES and type in ASPE Seven Corners under Provide details 9. Who will pay for you trip to and stay in Russia: Select Company 10. Name of Organization: Enter Russian American Foundation 11. Notes: Leave this BLANK 12. If your application was completed by a parent or guardian or another close family member, then there is no need to indicate that the application was not completed by you. 3. Enter your permanent address including your street address, city, state, and zip code. 3. Under Name of employer enter the full name of your current place of study. 4. Under work position please enter student. 5. Answer remaining questions with information from your current place of study. 6. Press Next and proceed to the next screen. 11 Screen 12: Appointment Details: 1. Select the location where you will be applying for your visa: select VISA APPLICATION CENTER ILS (NEW YORK) 2. Press Next and proceed to the next screen. Screen 13: Proofreading: 1. Review the information presented to you on this screen by doing the following: 2. Carefully reviewing your application to check for any errors in date formats, number sequences, or spelling. 3. Carefully review your application to ensure that you did not accidently omit any necessary information. 4. Carefully cross check any bio data information which is listed in your passport and ensure that it is written exactly the same way on your visa application. 5. Carefully cross check any information details provided to you in these instructions and ensure that they are written exactly the same way on your visa application. 6. Click Save and proceed to the next screen. Screen 14: Printing: 1. Print a hard copy single-sided. 2. Sign and date the form. 3.
