3 )$67&RPPHUFLDO'ULYHU3URJUDP F ree Ad Secure Trade (FAST) Commercial Driver Program is the result of the Smart Border Declaratio 30 Poit Actio Pla ad is a harmoized program ivolvig the Caada Customs ad Reveue Agecy (CCRA), Citizeship ad Immigratio Caada (CIC), the U.S. Customs Service (USCS), ad the U.S. Immigratio ad Naturalizatio Service (USINS). :KDWDUHWKHEHQHILWV" If you are approved to participate i the FAST Commercial Driver Program, you will receive a card to use at the border that will allow you to: use FAST dedicated laes i Caada ad the U.S. (where available); cross the border with accelerated customs ad immigratio processig; ad, trasport eligible goods for FAST approved carriers ad importers. :KRTXDOLILHV" You may qualify to participate i the program if you are a citize or permaet residet of the U.S. or Caada, age 18 or over ad possess a valid driver s licese. You must be admissible to Caada ad the U.S. uder applicable immigratio laws. However, you may ot qualify if you: provide false or icomplete iformatio o your applicatio; have bee covicted of a crimial offece; have bee foud i violatio of customs or immigratio law; or fail to meet other requiremets of the FAST Commercial Driver Program.

4 Both coutries must approve your applicatio. If you do ot meet the requiremets of both coutries, your applicatio will be deied. +RZGR,DSSO\" Your participatio i this program is strictly volutary. If you complete the attached applicatio form, we will use the iformatio you provided to determie your eligibility. If you meet iitial requiremets, you will be ivited to visit the erollmet cetre for fial processig. Durig this stage, we will: review the iformatio you provided o your applicatio form to esure it is accurate ad valid; review origial documets such as proof of citizeship, residecy, work permits, ad/or visas; take a full set of figerprits; take your photo for your FAST Commercial Driver card; explai the terms ad coditios of the FAST Commercial Driver Program; ad followig appropriate backgroud checks, you will be otified if you have qualified. &RPSOHWLQJWKHDSSOLFDWLRQ Each perso who wats to participate i the FAST Commercial Driver Program must fill out a separate applicatio form. The followig sectios give details about certai parts of the applicatio form. Preferred Laguage Idicate Eglish or Frech. Caadia officials will correspod with you i the laguage of your choice. U.S. officials will correspod with you i Eglish.

Sectio A Persoal Iformatio Other ames used If you have ever chaged your ame (icludig by marriage), provide the previous ame(s) you have used. Citizeship ad Residece Idicate whether you are a citize of Caada, the Uited States, or aother coutry. Be sure to idicate all coutries of which you are a citize or atioal (i.e. you are a dual atioal). Check the box that correspods with your coutry of permaet residece. Proof of Citizeship ad Residecy Acceptable proof of Caadia or U.S. citizeship icludes birth certificate ad photo idetificatio, a passport or citizeship certificate or card, a certificate of registratio of birth abroad, or a certificate of aturalizatio. Proof of Caadia or U.S. permaet residecy icludes a record of ladig i Caada, a valid Caadia permaet residet card or a valid U.S. permaet residet card. Please provide copies of ay visas you are required to preset. Be prepared to provide proof that your citizeship, residecy ad immigratio status are as stated o the applicatio form. Two photocopies of proof of citizeship ad residecy, alog with two photocopies of your valid drivers licese, must be set with your applicatio form to the FAST Commercial Driver program processig cetre. Failure to do so will result i the applicatio beig retured to you. However, if the documet idicates that it is a violatio to copy it, please ote this o your applicatio ad brig the origial with you to your iterview. You will eed to brig all the origial documets required for your admissio to the U.S. ad Caada to the erollmet office to be verified as part of the iterview process.

6 Sectio B Address history for the last 5 years You must provide details of your address history for the past 5 years. Use the spaces provided, startig with your curret address. Attach a separate sheet, if ecessary. Sectio C Employmet history for the last 5 years You must provide details of your employmet history for the past 5 years. Use the spaces provided, startig with your curret employer. Attach a separate sheet, if ecessary. Lack of employmet will ot disqualify you from participatio, if you are otherwise admissible to the U.S. ad Caada, ad meet all other program requiremets. To use a FAST card you must be trasportig eligible goods for a FAST approved carrier ad importer. Sectio D Additioal iformatio Idicate ay covictios for offeces i ay coutry for which you have ot received a pardo for example, alcohol related offeces. You must also idicate ay immigratio or customs violatios. For Caadia backgroud checks o admissibility, you may be questioed with regards to offeces committed outside Caada, pardos grated by coutries other tha Caada ad deemed rehabilitatio. For U.S. backgroud checks, you may be questioed about your full crimial history, icludig arrests or pardos. You must also idicate if you have ever applied to CIC for rehabilitatio. You must also idicate ay waivers of iadmissibility you have bee issued by the USINS. Sectio E Certificatio The laws that protect persoal iformatio collected by govermet agecies differ i Caada ad the Uited States. You should read the U.S. ad Caadia privacy

7 statemets ad the certificatio statemet o the applicatio form carefully. You must sig ad date the certificatio statemet. The U.S. Privacy Act applies to U.S. citizes ad U.S. permaet residets, icludig Caadia ad other atioals who fall ito either of these two categories. The Caadia Privacy Act applies to all persos applyig to the Caadia portio of the program. Sectio F Fee paymet For every applicatio there is a o-refudable fee for the FAST Commercial Driver Program of $80.00 Caadia or U.S. $ Program participatio will be valid for a five-year period. This fee is equally divided by the two coutries ad icludes the existig U.S. $25.00 applicatio fee for a lad border alterative ispectio program as well as the $40.00 Caadia applicatio fee. You must iclude a moey order or a certified cheque for the total amout i Caadia or U.S. dollars made payable to the Receiver Geeral for Caada. As a optio you may provide a Visa or MasterCard umber with your sigature. Do ot sed cash. +RZGR,GHFODUHJRRGV" As a FAST Commercial Driver participat, you must declare all goods you are importig wheever you cross the border ito the U.S. or Caada. If you are eterig the U.S. ad are importig goods for your persoal use, you must make a oral declaratio to the officer i the booth. For more iformatio, U.S. residets should refer to the U.S. pamphlet called Kow Before You Go ad Caadia residets visitig the U.S. should see the publicatio called Visitig the Uited States.

8 If you are eterig the U.S. ad have goods to declare i excess of your persoal exemptio, you must use the regular lae. If you are eterig Caada ad are importig goods for your persoal use that do ot exceed your persoal exemptio, you must make a verbal declaratio to the officer i the booth. For more iformatio about importig goods ito Caada, Caadia residets should see the CCRA pamphlet called I Declare ad U.S. residets should see the pamphlet called Customs Iformatio for Visitors to Caada ad Seasoal Residets. If you are eterig either Caada or the U.S. ad have goods to declare i excess of your persoal exemptio, you must use the regular lae. Note: Caadia residets have the optio of obtaiig Traveller Declaratio Cards (TDCs) that ca be preseted ay time you cross ito Caada usig the FAST lae. The card must be completed before arrivig i Caada. Whe you arrive, had your TDC to the officer i the booth. If you exceed your persoal exemptio, the duties ad tax owig will be charged to your credit card. $UHWKHUHUHVWULFWLRQVRQWKHJRRGV,FDQ LPSRUWIRUSHUVRQDOXVH" You caot import cotrolled, restricted, or prohibited firearms or weapos, icludig mace, pepper spray, ad stu gus; you caot import cotrolled, restricted, or prohibited goods, aimals, or plats. 9 if you brig or carry more tha US $10,000 i currecy ad moetary istrumets ito or out of the U.S., you must use the regular lae ad make a declaratio to U.S. Customs. :KDWHOVHGR,QHHGWRNQRZ" The card is valid for a period of 5 years provided you cotiue to meet all the terms ad coditios of this program. Oly approved FAST Commercial Drivers ca use FAST dedicated laes (where available). As a FAST Commercial Driver, you remai subject to compliace ad eforcemet checks at ay time you cross the border. If you use the FAST dedicated lae, everyoe i the vehicle must be i possessio of a valid FAST commercial driver card ad trasportig eligible goods for a FAST approved carrier ad importer. We will take a full set of figerprits to perform crimial records checks. This biometric will be shared with other govermet ad law eforcemet agecies. I additio, ay persoal iformatio you provide will be shared with other govermet ad law eforcemet agecies. Your applicatio will be deied if you do ot meet both Caadia ad U.S. requiremets for admissio ito the FAST Commercial Driver Program. Your FAST Commercial Driver card is ot a travel documet that authorizes your admissio to the U.S. or Caada. Your FAST Commercial Driver card validatio ad expiratio date will deped o your uderlyig citizeship or immigratio status for each coutry. Your immigratio status must be valid at all times, ad you must possess appropriate immigratio ad idetity documets i additio to your FAST Commercial Driver card. These may iclude your passport (required for some coutries), birth certificate, visa, permaet residet card, employmet authorizatio, work permit, ad other related documets.

10 Pealties for the Provisio of False Iformatio The failure to provide true, accurate ad complete iformatio i a applicatio to the FAST Commercial Driver Program may result i deial of your applicatio. Pealties are provided by law for kowigly ad willfully falsifyig or cocealig a material fact, or usig ay false documet i submittig your applicatio. A false represetatio also may result i the deial of this ad ay other applicatio you may make for ay beefit uder U.S. Immigratio laws. U.S. FAST Lae Coditios Applicats ackowledge ad agree that if they violate ay coditios of this program, or ay law or regulatio of ay Federal Ispectio Service, or are otherwise determied to be iadmissible to the U.S., their participatio i this program may be revoked ad they may be subject to other applicable sactios. Such sactios may iclude, but are ot limited to, crimial prosecutio, removal proceedigs, impositio of civil moetary pealties, ad seizures of merchadise ad vehicles. Coditios by which applicats must abide iclude, but are ot limited to, the followig: a) adherece to all state, federal, ad local laws regardig the importatio of alcohol ad agriculture products, possessio ad importatio of cotrolled substaces, ad all other laws ad regulatios uder the jurisdictio of ay federal agecy. b) adherece to all requiremets of the Immigratio ad Natioality Act, as ameded, U.S. Customs regulatios, ad all Federal Ispectio Service regulatios regardig documetary requiremets. If you violate the terms ad coditios of this program, we may cacel your privileges ad you may face fies, pealties, ad crimial charges. We will strictly eforce the law.
