1 RUSSIAN VISA APPLICATION - STEP BY STEP INSTRUCTIONS Important Note: When you begin the Russian Online Application, you will get a declaration ID and log in. It is very important to print this page and send with the application as it will allow us to make any corrections / adjustments needed. PAGE ONE: 1. Purpose of Visit: The first three drop down menus is for the reason you are going to Russia, please select in each your reason for travel. 2. Number of Entries: Single and Double entry apply to Tourist or Private Visas, Multiple Entry applies if you selected business as the Purpose of Visit. 3. Date of Entry: Provide the exact date of entry into Russia. 4. Date of Departure: Provide the exact date of departure from Russia.

2 PAGE TWO: 5. Nationality: Select you nationality from the drop down menu. 6. Surname: As it appears in your passport. 7. First Name: As it appears in your passport. (Note: Patronymic refers to any middle names in your passport). PAGE THREE: 8. Type of Passport: Select your type of passport (for most answer will be tourist or regular). If you select other please indicate what type of passport you hold. 9. Passport Number: Enter your passport number exactly as it appears in your passport. 10. Passport Issued By: Authority that issued your passport. 11. Date of issue/expiry: Enter the exact dates as they appear in your passport. PAGE FOUR: 12. Directive Number: For business travelers only: please enter a number if applicable or the number located on top of the invitation letter. Leave this space blank if you re applying for a tourist visa. 13. Tourist Company Reference Number: Is the number of the Tourist Organization that gave you your Confirmation for visit to Russia.

3 14. Number of Confirmation: Same as the voucher number on the Letter of Invitation from the hotel. 15. Name of Tourist Company: Tourist Company that gave you your Confirmation to visit Russia. 16. Route of Journey: Enter the town you are planning to visit if the town is not listed in the drop down menu please type it in the provided space and click the + tab PAGE FIVE: Please complete all the fields on page five, type n/a if the field is not applicable. PAGE SIX: 17. Medical Insurance: Please check yes or no. Specify the name of your insurance provider if check yes. Proof of insurance coverage is required for non US citizens only. 18. Who Will Pay for Trip and Stay: type self if the purpose of travel is tourism or the name of your employer if the purpose is business. PAGE SEVEN: 19. Marital Status: Select you current marital status. Note: if you select Married, Divorced or Separated will be required to enter your spouses information. PAGE EIGHT: Page eight refers to information about your parents. Mothers surname can be either her married or maiden name. Note: Patronymic refers to any

4 middle names. PAGE NINE: 20. Countries you have visited in the last 10 years: If you answer YES, list countries you have visited to the best of your memory. List all countries in your current passport which have an entry/exit stamp. 21. Passport issued by another Country: If you select YES, list other countries that have issued you a passport. PAGE TEN: 22. Have You Worked in Other Places: If you select YES, provide two last places of work, addresses and phone numbers. You can also write: out of business or not available if applicable. PAGE ELEVEN: 23. Whether you received education in higher educational establishment: If you select YES, list one or two universities or colleges that you attended. PAGE TWELVE: Are you a member of any Professional, Civil and Charity Organizations: If you selected YES, list the major organizations you were a member of.

5 24. Do you have specialized skills, training or experience related to fire arms, explosives or to nuclear, biological or chemical activities: If you selected YES, list special skills or training. PAGE THIRTEEN: 25. Military Service: If you selected YES to either question the Online Application will proceed to a page regarding your military information. PAGE FOURTEEN: Page Fourteen is a series of YES or No questions, if you answer YES to a question the Online Application will provide space for you to fill in the details. PAGE FIFTEEN: 26. Where you plan to stay in Russia: For Tourist visits you must provide the name of the hotel(s) you will be staying in, if you are providing the Tourist Invitation Letter on your own the information will be listed under accommodation on the voucher, if Travel Document Systems is providing the Tourist Invitation list the hotel(s) you will be staying in. TOURIST MUST PROVIDE A HOTEL NAME. If you check the PERSON field you can enter names and private residences but you must be applying for a private visit, student or Business visa. PAGE SIXTEEN: 27. Do you have any relatives currently in Russia: Select YES if have Russian relatives.

6 PAGE SEVENTEEN: 28. Is the application filled in by you personally: If you select NO you will be required to enter the details of the person that filled in the application. PAGE EIGHTEEN: 29. Notes: Do not leave this field blank; type NONE. PAGE NINETEEN: 30. Appointment Date: Enter the day your complete the Online Application. 31. Country: Select UNITED STATES 32. Visa Issuing Office: From drop menu choose last option Houston ILS. NOTE: IMPORTANT PRINTING INSTRUCTIONS: Before printing select NO PAGE SCALING and uncheck the CENTER AND ROTATE option.
