Use the Surname field for your last name and the Given Name field for your first name and middle name. 1.2 Please type your Chinese name, if applicable. (This applies to anyone who has ever had a Chinese name including children adopted from China.) 1.3 Please type any other Chinese or other names you have used. This includes maiden, religious, professional, and alias names if applicable. 1.4 Please select your sex. 1.5 Please type your date of birth using the (yyyy-mm-dd) format. 1.6 Please type your current nationality as in your passport. (Most US citizens would type American here.) 1.7 Please type your former nationality if applicable. (For a person adopted from China, type Chinese. If none, type NA.) 1.8 Please type the name of your birth place (City, Province/State and country). 1.9 Please type your local identity card number if applicable. (Most US residents would type their driver s license number here. If person is too young to have a driver s license, type their passport number. NOTE: For Americans, type: USA Part 2: Travel Information 2.1 Please select the purposes of your visit(s) to China. If your purpose(s) to China are not listed, please specify. 2.2 Please select the number of entries requested for your visit(s) to China with the visa. Most US citizens should check the Multiple entries valid for 12 months from application box. NOTE: As of December, 2014, most US citizens receive 10 year multiple-entry visas. 3 2.3 Please select if you are applying for express service. NOTE: As of 2013, the Houston Consulate no longer provides express visa service. All visa applications take 4 to 5 business days to process. Please plan ahead. 2.4 Please type the date of your first planned entry into China. 2.5 Please type your longest intended stay among all entries of your intended visits in China. NOTE: Generally speaking, the Houston Consulate will approve stays of up to 90 calendar days per entry. Longer stays may be possible but usually require application for a work or student visa. Please call or e mail us if you have questions about stays longer than 90 days. 2.6 Please type the address details for your intended itinerary in China. Start with your entry date and work forward to your departure date. Please keep the information in correct time sequence. If necessary, you can type this information on a separate piece of paper and include it with your application. Please type NA in any blank fields on the form. NOTE: You must list the Chinese address and telephone number of at least one hotel in China. If you plan to stay with friends or family, you must list their Chinese address and telephone number. It is OK to type these addresses using English characters. They do not need to be typed in Chinese characters. Effective 8/1/12,if you are applying for a Tourist visa, you must include a photocopy of the roundtrip airline ticket and hotel reservation. If you plan to stay with friends or relatives in China, you must include an invitation letter. See our "Additional Visa Requirements" document for details. 2.7 Please type who will pay for your cost of traveling and living during your stay in China. 2.8 Please type the name, address and phone number of your inviter or contact unit in China. NOTE: Business visa applicants must complete this field. Tourist visa applicants who plan to stay with friends or family in China should also complete this field. Tourist visa applicants who plan to stay in hotels can type NA in all four rows. 2.8 Families Traveling for Adoption Only please note the following: IF you are listed by name on the LOA (Letter of Acceptance), the Houston Chinese Consulate will allow you and your children to submit visa applications before final trip reservations have been made. IMPORTANT! If other relatives or friends are planning on traveling together with you on the adoption trip, they are NOT covered by your LOA and they must apply for a regular tourist visa separately. To do this, they must submit flight and hotel reservations. Otherwise, please select No. 3.3 If you have any criminal record in China or any other country, please select Yes. Otherwise, please select No. 3.4 If you are affiliated with any of the following diseases, please select Yes. Otherwise, please select No. 3.5 If you visited countries or territories infected by infectious diseases in the last 30 days, please select Yes. Otherwise, please select No. 3.6 If you select Yes to any questions from , please give details below 3.7 If you have more information about your visa application other than the above to declare, please give details below. NOTE: Normally, most US citizens do not need to provide more information, so just type NA in this field. IF, however, you had a previous China visa and have lost the passport that contains that visa, please type the following: I cannot locate my cancelled passport therefore I cannot provide a copy of my previous visa to China. 3.8 If someone else travelling with you shares the same passport, please affix their photo and give their information. NOTE: For all US citizens, this section is NOT applicable. The US does not allow shared passports. You MUST type NA in all nine boxes on the form. Part 4: Declaration and Signature 4.1 Please read and review each of the 3 declarations. After you have read and reviewed each declaration, please sign and date the visa application. 6 Part 5: Additional Signature Information If the application was completed by you on someone s behalf or for a minor, please complete the information in boxes 5.1 to 5.5. If you did not complete the application for someone else, please type NA in each box. 5.1 Please type the name of the person who completed this application form. 5.2 Please type your relationship to the applicant (for example mother, father, secretary, etc.). 5.3 Please type your address. 5.4 Please type your phone number. 5.5 Please sign and date the declaration. In recent years, China has tightened their visa requirements to require additional supporting documentation. We have summarized the current requirements on our website. In a general way, the Houston Consulate checks to make sure that all of the information you provide on the application form is consistent with your supporting documents. So, for example, let s say that the name you type on the application is not exactly the same as the name in your passport. The Consulate officials will notice that inconsistency and will likely require you to submit additional documentation to account for the difference. Please review your application carefully and make sure that the information provided matches that found in your passport or any other supporting documents.
