2 Chinese Visa Packet All crewmembers traveling to China are required to have a valid crewmember C Visa issued by the Chinese Consulate. Chinese visa s are valid for up to two years and depending on the length of time of your current passport is valid. Due to the high cost of a Chinese visa your passport should be valid for a minimum of 24 months. The Chinese visa office will not issue a visa if the passport is valid for less than 24 months. Visa applications must be brought to the Chinese Consulate in person, but that person can be a friend or colleague. Expedited service (24 hour) is available and most Visa Offices will/may issue the CREW Visa the same day if you arrive before 10:00 am*. YOU MUST verify if this service is available when appearing at the Visa Office The Chicago Visa office offers same day return to crew (unless same day volume exceeds capacity then you may have to return the following day to pick up passport). What to bring to the consulate: Your Passport A Chinese Visa request letter from the ORD Flight Administration Office. Please have your passport information available as it must be listed on the letter. {The letter must be printed on original company letterhead} Attach a 2 x 2 [color or black and white] white background o Bring a second photo in case the consulate requires a second photo. A photocopy of your AA CREW ID Payment of the visa fee* of $ in the form of cashier check payable to Chinese Consulate OR MasterCard/Visa credit cards only **The fees vary by Visa Office. Verify the total cost when submitting your application. Chicago Consulate will issue a two year visa. REIMBURSEMENT PROCEDURES You can be reimbursed for the following: Visa fee, expedite fees, passport photos *Incidental charges [i.e. Parking, Train fare, taxi fare, meals, etc.] are not reimbursable. 1. Complete an Expense Form (attached to this packet). Be sure to sign it. 2. Attach to the completed expense form: A. For Photos: ORIGINAL receipts only B. For Visa and Expedite Fees: 1. Cashiers Check Receipt/Money Order receipt copy or original or, 2. Original paid receipt stamped by Visa Office C. Please include a copy of the photo page of your passport. 3. Boardmail to: Elizabeth Serpe / Flight Admn MD B103 - ORD Please allow 2-3 weeks for reimbursement. Your reimbursement will be included in your paycheck.

6 Courier Service for China Visa Passports The courier service is offered by AA employee Tom Chau. Tom and his wife own a Visa Service in Chicago. Please contact the Chicago Flight Office if you have any questions Requirements for Passport Renewals: 1. Passport * 2. DS-82 form completely filled out. * 3. Two new passport photos. * 4. Imminent departure letter from flight office * 5. Signed passport authorization form for Tom to process your passport for you. * 6. Cashier Check made payable to US Department of State for $ * 7. Cash fee of $85.00 for courier service 5-7 days or $ for 2-3 days (courier service fee will NOT be reimbursed) OR one cashier check/personal check for the full amount made out to Tom Chau. Requirements for China Visa: 1. Passport * 2. China visa form completely filled out (foreign passport holders please request foreign visa application) * 3. Two new passport photos. * 4. Imminent departure letter from flight office. * 5. Copy of AA ID alone on an 8 1/2 x 11 piece of paper. * 6. Cashier Check made payable to Chinese Consulate of Chicago for $ * 8. Courier Service - $20 normal service $50 expedited service (courier service fee will NOT be reimbursed) OR one cashier check/personal check for the full amount made out to Tom Chau. Tourist Visa is $140 YOU MUST question ORD Flight Office before obtaining a tourist visa. Tourist Visa s may be expedited. Requirements for Lost Passport: 1.Report lost passport using form DS-64 from website: : keep copy before mailing. 2. Fill out form DS Two new passport photos 2x2. 4.Present Proof of Identity: Previous US passport (mutilated, altered or damaged passports are not acceptable as proof of identity), Naturalizations certificate, current and valid drivers license, government ID (city, state or federal) or Military ID. 5. Lost passport fee is: $ which includes expedite fee. 6. The courier fee is the same as passport renewals listed above. 7. You need to take all of the above paper work to your local post office (if they process passport paperwork) and ask them to seal all of the paperwork in an envelope for the US Passport Office in Chicago, advise them you are using a courier. YOU MUST SIGN ALL OF THE PAPERWORK IN FRONT OF POST OFFICE EMPLOYEE. 8.
