Kalinga Rao Road, Subbaiah Circle, Bangalore Date: CIRCULAR Subject : Submission of all kinds of Bills with regard to Supplementary Exams of Jan * * * * * Chief Superintendents of all the D.Pharm Exam Centres involved in conduct of Supplementary Exams of Jan 2013 are hereby informed that in order to speed up the billing process, a new billing format has been designed incorporating the necessary changes in the old format. Kindly note that the following guidelines should be strictly followed in submitting the Examination Bills to this office. FILE A : Centre Bills (Staff Bill): Bills pertaining to Theory examination, Practical examination, Postage charges, TA/DA/LCA for Theory Examination, Question Paper Collection work (If Q.P. Collection TA/DA/LCA bill is already submitted to BEA, this bill may be ignored), etc. - All these Bills should be submitted in File-A and the consolidated statement of claims is to be given as per Format A. FILE B : Practical Examiners Internal/External Examiners Bills and TA/DA/LCA of Practical Examiners alongwith consolidated statement of claims is to be submitted in File-B, as per Format B. Guidelines listed below should be compulsorily followed for submission of bills: a) All pages to be numbered b) Index for different bills enclosed with page numbers is to be given. c) Bills pertaining to particular examination should be submitted once only for all the different works. d) Bills have to be submitted in a spring file within 10 days after the examination. e) A C D containing soft copy of all the bills is to be pasted on the 2 nd page of the Spring file - FILE A & FILE B separately. 2 -2- f) A covering letter giving details of Remuneration claimed for different works with an undertaking that he/she will be responsible for disbursement of remuneration amount to the concerned persons is to be given. g) Acknowledgment for receipt of money received should be submitted within 30 days after the receipt of cheque. h) Contact details of the Principal / Concerned case worker to be provided for clarifications. 3 BOARD OF EXAMINING AUTHORITY, BANGALORE BILLING PROCEDURE FILE: A CENTRE BILL STAFF BILL 1) THEORY EXAM a) Details to be filled as per ANNEXURE 1 with signature of all concerned Staff along with signature of the Chief Superintendent. b) Attendance Certificate for Staff from other Colleges is to be attached. 2) PRACTICAL EXAM a) Details to be filled as per ANNEXURE 2 with signatures of all concerned Staff along with signature of the Chief Superintendent. b) Attendance Certificate for Staff from other Colleges is to be attached. 3) POSTAGE CHARGES Details to be filled as per ANNEXURE 3 along with Postage receipts. 4) TA/DA/LCA BILLS FOR THEORY EXAMS a) Consolidated Statement as per ANNEXURE 4 along with journey, halting and other details etc., b) Individual claim forms to be submitted by each examiner. c) Attendance Certificate for Staff to be attested by the Chief Superintendent. d) If Q.P.
