1 15 Mar of PURPOSE This section provides list of possible awards and honours that could be considered for LPF directors and employees (where applicable.) The Last Post Fund recognizes the hard work and dedication of its directors, members and employees through the presentation of various honours and awards. 2.0 REFERENCES Governing Council Minutes Executive Committee Minutes Governor General of Canada Web Site ( POLICY 3.1 The Last Post Fund undertakes an award program focused on current serving directors, members and employees aimed at recognizing their good deeds and contributions to LPF objectives. 4.0 Awards and Honours 4.1 The following is a list of awards and honours that could be considered for nominated candidates for approval and presentation at the appropriate level: Governor General of Canada The Order of Canada - to honour a lifetime of achievement The Meritorious Service Decoration - to honour exceptional deeds or activities The Governor General s Caring Canadian Award - to honour selfless voluntarism Provincial Recognition of excellence, achievement and contributions to the social, cultural and economic well-being of the province and its residents Order of Quebec (Ordre national du Québec) (G.O.Q., O.Q., C.Q.) Saskatchewan Order of Merit (S.O.M.) Order of Ontario (O.Ont.) Order of British Columbia (O.B.C.) Alberta Order of Excellence (A.O.E.) Order of Prince Edward Island (O.P.E.I.) Order of Newfoundland and Labrador (O.N.L.) Order of Manitoba (O.M.) Order of New Brunswick (O.N.B.) Order of Nova Scotia (O.N.S.) Veterans Affairs Canada Minister of Veterans Affairs Commendation - awarded annually to those exceptional Veterans or individuals who have contributed in an exemplary manner to the care and well-being of Veterans; and/or to the remembrance of the contributions, sacrifices and achievements of Veterans.

2 15 Mar of 9 Last Post Fund LPF Long Service Medal To recognize long service with the Last Post Fund to a person who is a director or staff member of the Last Post Fund-Fonds du Souvenir who has completed a minimum of twelve years service with the Last Post Fund-Fonds du Souvenir with one or more of the Branches or the National Office; and who has a record of exemplary service of such high standards as to merit the award of the medal. See paragraph 5.3 LPF Award of Merit - to honour LPF Directors and members who have contributed exceptional service to the goals of the LPF (employees are exempted this award). See paragraph 4.2 below. LPF Regular Membership for Life - to recognize outstanding services with or generosity to the LPF Honorary Member of the LPF - to honour individuals who support with distinction the mission of the LPF Achievement - to recognize through a presentation of a certificate to directors, members and employees for an outstanding achievement or deed Appreciation - to express through a presentation of a certificate, gratitude to a director, member or employee for having provided outstanding service and contribution to the LPF Service - to provide an official certificate of service to document and to express appreciation for a period of service with the LPF. 4.2 Last Post Fund Award of Merit Terms of Reference (as approved by the Governing Council in 1999 and modified by the Executive Committee in 2007) The Last Post Fund (LPF) Award of Merit recognizes outstanding contributions to the advancement of the Fund s mission or for outstanding meritorious services provided to the Fund over a period of several years. The Award is not an annual award; it may be presented more than once per year, but need not be presented every year The Award is made of Plexiglas with engraving based on the stained glass window installed on the second floor of the Gate of Remembrance at the National Field of Honour, in Pointe- Claire, Quebec. The stained glass window has been inspired from the Streathham memorial by Albert Toft which is located in England Recommendations for presentation of the Award will be submitted to the National President by the nomination form as per Annex A of this LPF MOM Recommendations may be submitted by: A person who has already received the Award; A Branch President; or A member of the Governing Council.

3 15 Mar of Salaried employees of the Fund are not eligible to receive the Award Once approved by the National Executive Committee, a letter of congratulations will be sent by the President to the recipient with copies to the local branch president and to the person having submitted the successful nomination. The Award will be presented thereafter by the National President or by his delegate at the earliest convenient time and place, with appropriate publicity and local press releases. 5.0 Nomination Procedures 5.1 There are many other awards and honours that can be recommended at provincial, regional and local levels. The above list is not exhaustive. Branch Boards of Directors are urged to organize and standing Honours and Awards Committee whose role would be to submit nomination recommendations for approval and concurrence by their Board of Directors. Nomination for the Governor General Awards and Veterans Affairs Awards should be channelled through the LPF national Executive Committee for support. 5.2 Nominations for the LPF Award of Merit and for LPF Regular Membership for Life should be submitted by February 15 of each year. A copy of the LPF Award of Merit nomination form can be downloaded from the LPF Resource site. 5.3 Regulations Governing the Award of the Last Post Fund Long Service Medal Short Title 1. These regulations may be cited as the Last Post Fund Long Service Medal Regulations. 2. In these regulations Interpretation Advisory Committee means the committee established pursuant to sub section 7. (1). Bar means the Bar described in 4. (2). Branch means one of the provincial Branches of the Last Post Fund. Directors means a volunteer director of the Last Post Fund. Exemplary Service means service characterized by good conduct, industry and efficiency that serves as a model for others. Fund means the Last Post Fund. Governing Council means the volunteer board that governs the fund.

4 15 Mar of 9 Medal means the Medal described in 4. (1). Designation 3. There should be a medal designated as the Last Post Fund Long Service Medal. Description 4. (1) The Last Post Fund Long Service Medal shall consist of a circular medal 1d inches in diameter. On the obverse is an image of a bugler above a laurel wreath bearing the motto the motto LEST WE FORGET-JE ME SOUVIENS, on the reverse a maple leaf inscribed with the year 1909, signifying the year the organization was founded, and the organization name "LAST POST FUND-FONDS DU SOUVENIR. The medal is suspended from a medal bar with a maple leaf centre. The whole suspended to a ribbon 1¼ inches with a black centre, to symbolize mourning, with light blue (Air Force) stripes on each side followed by scarlet (Army) stripes with navy (Navy) blue stripes on each edge to represent each branch of the Canadian Forces. (2) The Bar to the Medal shall consist of a plain bar with a stylized maple leaf centered thereon. 5. The medal may be awarded to a person who: Eligibility for Medal and Bar (a) is a director or staff member of the Last Post Fund-Fonds du Souvenir on or after. (b) has completed a minimum of twelve years service with the Last Post Fund-Fonds du Souvenir, not necessarily continuous, with one or more of the Branches or the National Office. (c) has a record of exemplary service of such high standards as to merit the award of the medal. 6. A person who has been awarded the Medal is eligible to be awarded a Bar for each additional ten year period with the Last Post Fund-Fonds du Souvenir up to two bars, following the period for which the Medal was awarded providing the service is of the same standard. Eligibility for Medal and Bar 7. (1) There shall be an Advisory Committee consisting of the National President of the Last Post

5 15 Mar of 9 Fund-Fonds du Souvenir, the Executive Director of the Fund and the Branch President of each of the Branches. (2) The Chairperson of the Committee will be the National President of the Fund. (3) The Advisory Committee shall advise the Governor General on such matters concerning the award of the Medal and Bar as the Governor General may refer to the Committee for consideration. (4) There shall be an Awards Committee for the Fund and each of its Branches. (5) An Awards Committee established pursuant to section (4) shall consist of such members as are appointed by the National President and the Branch President for each Branch. The Chairperson of these committees will be chosen by its members. (6) Notwithstanding sections 5 and 8 the Governor General may on recommendation of the Awards Committee make extraordinary awards of the Medal. Nomination Procedure 8. (1) A nomination of a person for the Medal or a Bar shall be made to the Director of Honours, the Chancellery by the Chairperson of the Awards Committee of the jurisdiction in which the person is serving through the National Office of the Fund which shall be responsible for the appropriate observance of the following procedure. (2) A Chairperson of an Awards Committee shall submit a written nomination in respect of the nominee; (a) setting the date on which the required period of service for the Medal or Bar was completed by the nominee; (b) stating that the conduct and performance of the nominee have been judged as being exemplary and deserving of recognition by the award of the Medal or Bar; (c) signed by the Chairperson or a member of the committee designated for that purpose. Awards 9. Award of the Medal or Bar shall be made by instruments of the committee designated for that purpose.

Unless the Governor General otherwise directs, the Medal or Bar shall be presented by the Chairperson of the appropriate Awards Committee or immediate supervisor on behalf of the Governor General: 11. (1) The Medal and Bar shall be worn in the sequence prescribed and in the manner described in publications issued by the Chancellery. (2) A person to whom the Bar has been awarded may wear a small Rosette on the ribbon when only the ribbon is worn. A maximum of three such rosettes may be worn. (3) A person to whom a Medal has been awarded may wear a miniature on appropriate occasions. Cancellation and Re-instatement 12. (1) The Governor General may on advice of the Advisory Committee: (a) cancel or annul the award of the Medal or Bar to any person; and (b) restore the award of the Medal or Bar that has been cancelled/annulled under (a). (2) When the award of a Medal is cancelled or annulled under subsection (1) the name of the person to whom it was awarded shall be deleted from the Register referred to in (1) The Director, Honours the Chancellery shall: (a) prepare the instrument of award for signature by the Governor General. (b) maintain a register containing the names of recipients and such other records relating to the award as the Director considers necessary. (c) perform such other functions in respect of awards of the Medals and Bars as the Governor General may require the Director to perform. 14. (1) National Office of the Last Post Fund-Fonds du Souvenir shall: (a) coordinate the submission of all nominations for the Medal and Bar emanating from all Awards Committee and submit them to the Director of Honours. (b) acquire the Medals, Bars and certificates at no cost to the public.

7 15 Mar of 9 (c) arrange for award ceremonies or send the Medals and Bars for presentation to the recipients and (d) perform such other functions in respect to the awards of the Medals and Bars as the Director of Honours may require. General 15. Nothing in these regulations limits the right of the Governor General to exercise all powers and authorities of the Sovereign in respect to the Medal and Bar. Annex A See following page.

Use complete sentences without abbreviations. Limit is 300 words. Additional biographical material may be attached.

9 15 Mar of 9 NOMINATOR INFORMATION Name (print): Signature: : AWARD RULES Award recipients are selected by vote of the Last Post Fund Executive Committee: each committee member reviews the nomination forms and ranks the candidates based on the significance of the accomplishments cited and their relevance to the Last Post Fund. Rules: 1. The nominee must be a current member of the Last Post Fund. 2. Employees of the Last Post Fund are not eligible to receive the award. 3. Unsuccessful nominations will be retained in the active file through three award cycles. 4. Nominations must be submitted to the Last Post Fund National Office by 15 February.
