Please read carefully these instructions for traveling to Jamaica. To obtain your Work Permit to serve in Jamaica, please collect and send these documents: Photocopy of passport ID page 7 Front facing passport photos (white background) 1 Profile facing passport photo (side view of just your head, white background) Original or Certified Birth Certificate 2 Original notarized police clearance letters 2 Character reference letters 1 Health Certificate (needs to declare that you are in good physical and mental health and has no infectious diseases). Application form (please only fill out lines and sign and date on line 29. We will take care of the rest) Passport (must be valid for the full time you are on your mission). ALL DOCUMENTS THAT ARE NOT IN ENGLISH NEEDS TO BE TRANSLATED BY AN APPROVED TRANSLATOR IN YOUR COUNTRY.
