The Citizenship application Charge will not be refunded if a decision is made to refuse to grant Citizenship because the applicant did not satisfy the criteria. Delivery and courier charges You are responsible for all costs of delivering information to the Australian Embassy by mail or courier, including any additional information that may be request by the Australian Embassy. Part E Good Character (applicants over 16 years only) and Consent (all applicants) Part I Declaration (all applicants) Citizenship Application Charge paid Documents Main Applicant Full birth certificate issued by the relevant Ministry or Authority which contains: Birth name, date of birth, place of birth and names of both parents. Please be advised that the Citizenship by descent certificate will reflect the details in the official certificate issued by the Sangat/Commune. If the details in the official certificate issued by the Sangat/Commune is incorrect, please provide an updated/new official certificate issued by the Sangat/Commune.. 3 If you wish to appoint a Migration Agent or authorize another person to communicate with the department on your behalf about this application, complete and attach form 956 The applicant will need to provide Penal clearance certificates from oversea countries they have lived in the last 10 years since the age of 18 years, If: the total time spent overseas added up to 12 months or more; and the time spent in any one country was more than 3 months, or requested to do so by the department Australia should be considered an overseas country. If you live in Phnom Penh, you must provide a police certificate from Ministry of Justice. If you live outside of Phnom Penh, you must provide a police certificate from the Chief of Police of the local police authority. Documents Parents Marriage Certificate, if applicant s parents are married. Divorce decree if applicant s parents are divorce If the applicant s parents are not married, both parents are required to sign Part J Declaration. Therefore submit two Part J Declarations, one signed by each parents. Brief statement outlining the history of, and inception of the parents relationship. If the parents are no longer together the statement should also provide information relating to the cessation of the relationship. Australian parent/s must provide: o Copy of passport including bio page, visas and movement stamps commencing 12 months prior to the birth of the child. o Full birth certificate of Australian parent/s o Australian citizenship certificate (if granted citizenship) o Evidence of support and care the father provided to the applicant and mother before and after birth, such as; financial, emotional, social recognition (eg. photos). Non - Australian parent must provide: o Copy of passport including bio page, visas and movement stamps commencing 12 months prior to the birth of the child (if applicable) o Birth certificate, National ID card, Family Book and Resident book Page 3 of 5

4 If the Australian citizen parent is an Australian citizen by decent, evidence that parent had resided in for 2 years in Australia (such as passport showing entry to Australia, school or employment records, any documents issued by an Australian Government agency) Evidence of pregnancy ( Photos, medical records, ultrasound reports) Evidence of any name changes that have occurred for the child or parent/s, if applicable Family photos before and after the birth, including photographs of the applicant with both parents from birth to present (photocopies of the originals) Other Documents I hereby acknowledge that: I have provided all necessary documentation for a decision to be made on my visa application. or I have not provided all the information requested in the checklist.
