8 Candidate Document Glossary Candidate Guide - A guide covering the candidate process. This guide is available in the Elections Office prior to each state and/or local election. Signature in-lieu Petition A candidate may submit a petition containing signatures of registered voters in lieu of paying all or part of a filing fee. (This petition is optional. Not all elected offices require a filing fee.) Nomination Papers - A petition made available during the nomination period to be circulated by all candidates in order to gather valid nominating signatures. Declaration of Candidacy / Oath of Office - A document an individual must file during the nomination period with the appropriate filing official in order to become a candidate for an elected office. Statement of Qualifications The Candidate's Statement is an optional document submitted and paid for by a candidate for printing in the sample ballot pamphlet. It is designed to acquaint voters with a candidates' education and/or qualifications for the office they are seeking. FPPC (Fair Political Practices Commission) Financial Disclosure Statements 501 form - Statement of Intention - A candidate for state or local office must file this form with the appropriate filing official prior to solicitation or receipt of any contribution, or expenditure of any personal funds used for the election. Form Officeholder and Candidate Campaign Statement -To be filed by candidates who do not have a controlled committee; do not anticipate receiving contributions totaling $1,000 or more during the calendar year; and do not anticipate spending $1,000 or more during the calendar year. Form Statement of Organization - The Form 410 must be filed with the appropriate filing official within 10 days of receiving $1,000 in contributions in order to form a financial committee. Form Recipient Committee Campaign Statement - A candidate or officeholder who has a controlled committee, or who has raised or spent or will raise or spend $1,000 or more during a calendar year in connection with election to office or holding office. Form 462 Verification of Independent Expenditure Report Independent expenditures that cumulatively total $1,000 or more to support or oppose a single candidate during the 90 days immediately preceding the election. Form 496 Independent Expenditure Report Independent expenditures that cumulatively total $1,000 or more to support or oppose a single candidate during the 90 days immediately preceding the election. Form 497 Independent Contribution Report Independent contributions that cumulatively total $1,000 or more to support or oppose a single candidate during the 90 days immediately preceding the election. Form Statement of Economic Interests The form used to meet the Political Reform Act requirement that many state and local government officials must publicly disclose their personal assets and income. This form is filed with the Declaration of Candidacy and annually thereafter. Cal-Access Website The Cal-Access website provides financial information supplied by state candidates, donors, lobbyists, and others. Fair Political Practices Commission (FPPC) The Commission adopts and amends regulations regarding campaign financing and spending, develops required forms, prepares manuals and instructions, aids agencies and public officials with record keeping and reporting. The Commission also investigates alleged violations of the Political Reform Act, imposes penalties when appropriate, and assists state and local agencies in the development and enforcement of conflict-of-interest codes. In an effort to reduce these violations, the FPPC educates the public and public officials on the requirements of the Act and provides written and oral advice to public agencies and officials and conducts seminars and training sessions.
