1 VIDEO SUBMISSION FORM This Voya 2016 Ethics Awareness Week Employee Video Contest Video Submission Form ( Form ) and all appropriate releases (as further described below) must be completed and returned to Voya Financial, Inc. 2 Publicity Release and Agreement ( Release ) PLEASE READ CAREFULLY BEFORE SIGNING 1. I hereby grant Voya Services Company, its parent, subsidiaries, affiliates, employees, agents, and assigns (collectively referred to as Voya ) for any and all purposes including, but not limited to, advertising, promoting and publicizing Voya and its businesses, products, services or any promotion or offer presented by Voya in any and all media, the absolute right and permission without any additional consideration: (a) to use my name, city and state of residence (collectively, Personal Information ); and (b) subject to Paragraph 2 below, to use, adapt, reproduce, disseminate, publicly display, incorporate into another work or otherwise exploit any video, image, likeness, drawing of me or other material submitted to Voya (and/or its designee) in connection with the Voya 2016 Ethics Awareness Week Employee Video Contest (each such video or other submitted material, a Submission ), without payment or any other consideration. 2. I shall have no right of review, approval, or compensation and no claim (including, without limitation, claims based upon invasion of privacy, defamation, right of publicity and/or infringement of an intellectual property right) arising out of any use of the Submissions. Further, I waive any right to royalties or other compensation arising or related to the use of the video(s). 3. I hereby agree not to sue nor to make any claim under any theory and hereby waive, release, exempt, and agree at all times to hold harmless and forever discharge Voya and anyone working on its/their behalf, and each of their respective affiliates, parent companies, subsidiaries, representatives, distributors and advertising/promotion agencies, and each of their respective directors, employees, officers, shareholders and agents (collectively, Promotional Parties ) from any and all actions, claims, demands, costs, liabilities, losses, damages and expenses (including, without limitation, attorneys fees, costs and disbursements) of any nature including, but not limited to, any negligence or derivative claim that I (or my heirs, executors, administrators, successors, assigns or third parties) may have against Voya (or any of the aforementioned parties in this Paragraph) (collectively, Claims ) whether now known or unknown, that are based on, arise out of, relate to, concern or involve (a) Personal Information or the use of Submissions, or (b) any activity or promotional effort related to any of the foregoing, or related to and/or covered by Paragraph 1 of this Release (clauses (a) and (b) above are, collectively, Covered Matters and each is, individually, a Covered Matter ) including without limitation any personal injury, accident, illness, breach of contract, death or loss or damage to any person or property, or any delay or inconvenience of any kind. 4. I agree, at all times, to indemnify, defend and hold harmless Voya and the other Promotional Parties from and against any and all Claims of any nature whatsoever including, but not limited to, any negligence or derivative claim that a third party may have against Voya (or any of the other Promotional Parties), whether now known or unknown, that are based on, arise out of, relate to, concern or involve any Covered Matter which involves or relates to me, my acts and/or omissions including, but not limited to, any personal injury, accident, illness, breach of contract, death or loss or damage to any person or property, or any delay or inconvenience of any kind. 2

3 5. I waive any right I may have to dispute the scope of this general release on the grounds that it is a general release and a release of unknown claims. I understand that I have the opportunity to seek independent legal counsel prior to signing this Release. 6. I agree that this Release shall be construed in accordance with and governed by the internal law of the State of New York (without reference to its rules as to conflicts of law) and venue for resolution of any dispute shall be federal, state and local courts located in New York, New York. 7. I declare that all information I submit in this Release is true and correct. I attest that I have read, or have had read to me, and understand all the foregoing provisions of this Release, prior to its execution, and agree to be bound by this Release. 8. I hereby affirm that I am over the age of 18 and have the right to contract in my own name. I have read the above Release prior to its execution; I fully understand the contents thereof. 4 Videographer Release and Agreement ( Release ) PLEASE READ CAREFULLY BEFORE SIGNING 1. I hereby grant Voya Services Company, its parent, subsidiaries, affiliates, employees, agents, and assigns (collectively referred to as Voya ) for any and all purposes including, but not limited to, advertising, promoting and publicizing Voya and its businesses, products, services or any promotion or offer presented by Voya in any and all media, the absolute right and permission without any additional consideration: (a) to use my name, city and state of residence (collectively, Personal Information ); and (b) subject to Paragraph 2 below, to use, adapt, reproduce, disseminate, publicly display, incorporate into another work or otherwise exploit any video, image, likeness, drawing, or other material submitted to Voya (and/or its designee) in connection with the Voya 2016 Ethics Awareness Week Employee Video Contest (each such video or other submitted material, a Submission ), without payment or any other consideration. 2. With respect to each of my Submissions, I represent and warrant that: (a) I own all copyrights in such Submission; (b) nothing in such Submission infringes on or misappropriates any rights belonging to any person or entity or violates any person s rights of privacy or publicity; and (c) all necessary releases and permissions have been secured. For each of my Submissions, I hereby grant Voya a worldwide, royalty-free, irrevocable, perpetual, non-exclusive license with the right to sub-license on a royalty-free basis (without my separate consent) to use and exploit all rights associated with such Submission for the purposes described in Paragraph 1 above. I understand that Voya will retain all my Submissions and they will not be returned to me. 3.
