3 Welcome June 7, 2018 Dear NARHC Attendees, On behalf of the Society Board of Directors, welcome to the 2018 North American Refugee Health Conference! I also want to personally thank all our Pacific Northwest attendees for making our first trip out west successful. Now that we have mastered the ability to move the conference around the country, don t be surprised if we end up back in your neighborhood in future years. Refugees come to our continent to start a new life and create a better future for themselves and their families. With each passing generation, we share the same tradition as our ancestors, welcoming these new immigrants with open arms and sharing in the joy and satisfaction of seeing them become part of our increasingly diverse society. The next three days are important to us. Serving this unique segment of our society is rewarding but comes with challenges. Attempting to manage complex medical conditions while at the same time working to overcome cultural, language and socio-economic barriers in order to deliver high quality care with respect, is a challenge. It is my hope that this conference will provide the tools to help you serve your patients better. In a way, this conference is our small contribution to the larger body of work being done throughout the world to meet the healthcare needs of refugees and immigrants. 10 Keynote Speakers Thursday June 7th Alex Neve Secretary General Amnesty International Canada Ottawa, ON Alex Neve believes in a world in which the human rights of all people are protected. He has been a member of Amnesty International since 1985 and has served as Secretary General of Amnesty International Canada s English Branch since In that role he has carried out numerous human rights research missions throughout Africa and Latin American, and closer to home to such locations as the Grassy Narrows First Nation in NW Ontario and to Guantanamo Bay. He speaks to audiences across the country about a wide range of human rights issues, appears regularly before parliamentary committees and UN bodies, and is a frequent commentator in the media. Alex is a lawyer, with an LLB from Dalhouse University and a Masters Degree in International Human Rights Law from the University of Essex. He has served as a member of the Immigration and Refugee Board, taught at Osgoode Hall Law School and the University of Ottawa, been affiliated with York University s Centre for Refugee Studies, and worked as a refugee lawyer in private practice and in a community legal aid clinic. He is on the Board of Directors of Partnership Africa Canada, the Canadian Centre for International Justice and the Centre for Law and Democracy. Alex has been named an Officer of the Order of Canada, a Trudeau Foundation Mentor and has received honorary Doctorate of Law degrees from the University of Waterloo and the University of Brunswick. Paul B. Spiegel md, mph Professor and Director Johns Hopkins Center for Humanitarian Health Johns Hopkins Bloomberg School of Public Health Baltimore, MD Dr. Spiegel is the Director of Johns Hopkins Center for Humanitarian Health at the Johns Hopkins Bloomberg School of Public Health where he lectures and undertakes research in humanitarian emergencies. Previously, Dr. Spiegel was the Deputy Director of the Division of Programme Support and Management at the United Nations High Commissioner for Refugees (UNHCR) in Geneva, Switzerland. He was also Chief of the Public Health and HIV Section ( ) and the Refugee Agency s Global HIV Coordinator for UNAIDS ( ). Dr. Spiegel is Chair of the Funding Committee for the DFID and Welcome Trust funded Research for Health in Humanitarian Crises or R2HC, and is on the editorial board of the journal entitled Conflict and Health. Before UNHCR, Dr. Spiegel worked as a Medical Epidemiologist in the International Emergency and Refugee Health Branch at the Centers for Disease Control and Prevention. He received his medical degree at the University of Toronto and his Masters in Public Health and specialty in Preventive Medicine at Johns Hopkins Bloomberg School of Public Health. David Shlim md Medical Director Jackson Hole Travel and Tropical Medicine Jackson Hole, WY David R. Shlim first visited Nepal in 1979 to work as a volunteer doctor for the Himalayan Rescue Association, at an aid post at 14,000 feet. After three stints near the base of Mt. Everest, he moved to Kathmandu in 1983 to begin what became a fifteen-year career as the Medical Director of the CIWAC Clinic Travel Medicine Center in Kathmandu, the world s busiest destination travel medicine clinic. He is the author of more than forty-five original research papers, numerous chapters in textbooks, and is the recent past-president of the International Society of Travel Medicine. He was the Director of the Himalayan Rescue Association for more than ten years. He also provided free medical care for a Tibetan Buddhist monastery for fourteen years, and developed a close relationship with the head of the monastery, Chokyi Nyima Rinpoche, and his father, Tulku Urgyen Rinpoche, who was recognized as one of the greatest meditation teachers of the twentieth century. He also offered free medical care to all newly-arrived Tibetan refugees who came over the Himalayan passes from Tibet. He is co-author, with Chokyi Nyima Rinpoche, of Medicine and Compassion: An American Doctor and a Tibetan Lama on How to Provide Care with Compassion and Wisdom. The book has been translated into four languages. He resides in Jackson Hole, Wyoming, where he practices travel medicine and teaches Tibetan Buddhism North American Refugee Health Conference

11 Keynote Speakers Friday June 8th Brian Gushulak md Migration Health Consultants, Inc. Qualicum, BC Originally from the Canadian province of Saskatchewan, Dr. Gushulak completed undergraduate and medical studies in the province interspersed with graduate studies at the University of Western Ontario. Joining the Immigration Medical Services of the Canadian Federal Government in the early 1980 s his career has focused on international health and migration. He has held positions in the federal health and immigration departments. He was closely involved in the initial planning for the revision of International Health Regulations. From 1996 to 2001 he worked in the international sector as the Director of Migration Health Services of the International Organization for Migration in Geneva. During that time he was involved in refugee and complex humanitarian emergencies in Eastern Europe, the Balkans, Asia and Africa. From 2001 until 2004 he was the Director General of the newly created Medical Services Branch in the Canadian Department of Citizenship and Immigration. Since then he has been engaged in research and consulting in the area of health and population mobility. His research interests include migration health and population mobility, international disease control and the history or quarantine practices. Chris George Executive Director Integrated Refugee & Resettlement Services New Haven, CT For the past 12 years, Chris George has been the Executive Director of IRIS, Integrated Refugee & Immigrant Services, a New Havenbased refugee resettlement agency that welcomed 530 refugees to Connecticut last year. Chris has spent most of his professional life living in, or working in, the Middle East. Before returning to Connecticut in 2004, he worked seven years in the West Bank and Gaza Strip. Chris directed a legislative strengthening project with the Palestinian Parliament and later established an emergency assistance program for Palestinian nonprofits. From 1994 to 1996, Chris was Executive Director of Human Rights Watch Middle East. Prior to that, he worked with Save the Children for nine years (mostly in the West Bank and Gaza) and three years with American Friends Service Committee (mostly in Lebanon). Chris began his international career in 1977 as a Peace Corps Volunteer in Muscat, Oman. Altogether, he spent more than 16 years living in the Middle East. He speaks Arabic. Saturday June 9th Yasmine Mustafa Founder and CEO ROAR for Good Philadelphia, PA Yasmine is a tech entrepreneur and community leader. Championing the underprivileged has been a core part of her identity since her family was rescued from the Persian Gulf War. After numerous immigration woes, she graduated summa cum laude with an Entrepreneurship degree from Temple University and subsequently founded her first startup, an affiliate marketing company helping writers monetize their content. After growing that company, she sold it to a prominent content marketing firm in Silicon Valley. Yasmine also started the Philadelphia chapter of Girl Develop It, a non-profit focused on providing affordable and accessible programs to women who want to learn software and web development. She sits on the board of Coded by Kids, a non-profit that helps inner city youth learn how to code. Once she received her naturalization status, she embarked on a six-month solo trek across South America which inspired Yasmine to found her latest venture, ROAR for Good, a safety wearable technology company aimed at diminishing attacks against women and addressing the underlying causes of violence. Yasmine was selected as one of the BBC's 100 Women of 2016 as well as Upstart 100's most dynamic people shaping business. She was also recognized as one of Philadelphia s Top 20 Tech Disrupters for her work empowering women North American Refugee Health Conference 9

12 Invited Plenary Speakers Thursday June 7th Karen Musalo jd Professor of Law and Director, Center for Gender & Refugee Studies Hastings College of Law, University of California San Francisco, CA Karen is a Professor of Law at the University of California, Hastings College of Law in San Francisco. She has written extensively on refugee law issues, and has contributed to the evolving jurisprudence of asylum law not only through her scholarship, but also through her litigation of landmark cases. She was the lead attorney in Matter of Kasinga (fear of female genital mutilation as a basis for asylum), and was the attorney in Matter of RA, and Matter of LR two cases that established the principle that women fleeing domestic violence may qualify for refugee protection. Professor Musalo is recognized for her innovative work on refugee issues. She was the first attorney to partner with psychologists in her representation of traumatized asylum seekers, and she edited the first handbook for practitioners on cross-cultural issues and the impact of culture on credibility in the asylum context. Her current work examines the linkage between human rights violations and migration, with a focus on violence against women and children in Guatamala, El Salvador, and Honduras and its relation to request for refugee protection from these countries. She is the founding director of the Center for Gender & Refugee Studies, which is internationally known for its research and legal advocacy and for its program of expert consultation to attorneys around the world. Martin Cetron md Director Division of Global Migration and Quarantine Centers for Disease Control and Prevention Atlanta, GA Dr. Cetron is currently the Director for the Division of Global Migration and Quarantine (DGMQ) at the U.S. Centers for Disease Control and Prevention (CDC). The DGMQ mission is to prevent introduction and spread of infectious diseases in the U.S. and to prevent morbidity and mortality among immigrants, refugees, migrant workers, and international travelers. Dr. Cetron s program is responsible for providing medical screening and disease prevention programs to 1.2 million immigrants and 80,000 refugees prior to U.S. resettlement each year. He has authored or co-authored more than 175 publications. He teaches and lectures worldwide and is frequently quoted in the media. Dr. Cetron served on the 2016 U.S. Olympics Committee and has functioned as an expert on several intergovernmental and international committees. He is a graduate and advisor to the National Preparedness Leadership Institute at the Harvard School of Public Health and the Kennedy School of Government. Dr. Cetron has worked at the CDC since 1992 where he has led several domestic and international outbreak investigations. He received his B.A. in Biochemistry summa cum laude from Dartmouth College and his M.D. from Tufts University School of Medicine. Gretchen Heinrichs md, dtm&h Associate Professor of Obstetrics and Gynecology School of Medicine, University of Colorado Anschutz Medical Campus Professor of Obstetrics and Gynecology at Denver Health Aurora, CO Dr. Heinrichs is an OBGYN physician and researcher who completed a Diploma in Tropical Medicine and Hygiene at the Gorgas Course in Lima, Peru and a Certificate in Public Health in 2011 from the Colorado School of Public Health. She has been in academics since 2006 and has received several teaching awards for her work with residents and medical students. She works clinically at Denver Health with a special focus on refugee and asylum seeking patients and her Global Health work is currently focused on projects in Guatemala through the Center for Global Health. She has worked in India, Mexico, Rwanda and Nigeria training physicians, nurses, and health care workers on gender-based violence, family planning, and obstetric and gynecologic conditions, as well as adapting international practice recommendations to local and low resource settings. She recently completed a mission with Doctors Without Borders in Northern Nigeria in Emergency Obstetrics. Her scholarly interests include Maternal Mortality, Female Genital Mutilation, Infectious Diseases and Refugee health services. Dr. Cetron holds faculty appointments in the Division of Infectious Disease at Emory University School of Medicine and the Deparment of Epidemiology at Rollins School of Public Health North American Refugee Health Conference

13 Invited Plenary Speakers Friday June 8th Nina Marano dvm, mph Branch Chief Division of Immigrant, Refugee, and Migrant Health Centers for Disease Control and Prevention Atlanta, GA Dr. Marano is an epidemiologist at CDC. Her work has focused on prevention and control of infectious diseases. In 2015, she was appointed Chief of the Immigrant, Refugee, and Migrant Health Branch, which works to promote and improve the health of immigrants, refugees and migrants; and prevent the importation of infectious diseases and other conditions of public health significance into the U.S. A native New Yorker, Dr. Marano trained in veterinary medicine at the University of Georgia and in Public Health at Emory University. She has co-authored over 60 publications in peer-reviewed literature, and is an Associate Editor for CDC s Emerging Infectious Diseases Journal. Aniyizhai Annamalai md Associate Professor Departments of Psychiatry and Internal Medicine Yale School of Medicine New Haven, CT Dr. Annamalai is associate professor in psychiatry and internal medicine at Yale School of Medicine. She is the director of the adult Yale Refugee Clinic that partners with the local refugee resettlement agency to provide comprehensive health screening and longitudinal care for all newly arrived refugees in New Haven, Connecticut. The clinic serves as a central point for several clinical, educational and research programs to improve health of refugees resettling in New Haven. Dr. Annamalai is the editor of Refugee Health Care, a reference book used by healthcare practitioners who provide clinical care for refugees. Her academic interest is in integrating physical and mental health care for vulnerable populations, including refugees. Carey Jackson md Adjunct Professor, Global Health University of Washington Medical Director, EthoMed Seattle, WA Dr. Carey Jackson is a professor in the Department of Medicine and an adjunct professor in the Department of Global Health at the University of Washington. At Harborview, he serves as medical director of the International Medicine Clinic, Refugee and Immigrant Health Promotion Program, the Northwest Health and Human Rights Coalition, and Interpreter Services Department. He is a co-founder of the Community House Calls Program and EthnoMed website, and is also an affiliate investigator at the Fred Hutchinson Cancer Research Center. Dr. Jackson is a general internist with graduate training in anthropology and public health, with extensive experience in the practice of cross-cultural medicine, community health promotion and health services research among diverse immigrant patient populations in Seattle. His professional interests include refugee and immigrant health care, health services delivery to the non-english speaking poor, medical anthropology, health disparities and disease prevention. Dr. Jackson has been the recipient of numerous honors for his work, including American College of Physicians Internist of the Year in 2013, Sterling Munro Public Service Teaching Award in 2007 and the Robert Wood Johnson Foundation's Community Health Leadership Award in Saturday June 9th Emily Jentes phd, mph Commander, U.S. Public Health Service Lead, Domestic Team Immigrant, Refugee, and Migrant Health Branch Centers for Disease Control and Prevention Atlanta, GA Emily Jentes PhD, MPH is currently the Team Lead for the Domestic Program with the Immigrant, Refugee, and Migrant Health Branch at the Centers for Disease Control and Prevention. Emily began her career at CDC with the Epidemic Intelligence Service program as an officer in the Division of Global Migration and Quarantine at CDC, where she worked on projects both with the Immigrant, Refugee, and Migrant Health and Travelers Health Branches. She has both domestic and international research and epidemiology experience in public health. Although her research has primarily focused on tropical medicine, travel medicine, and vector-borne and zoonotic diseases, she has worked on refugee health projects in Guinea, Thailand, and Kenya. Emily earned her BS in Biology at Truman State University, MPH in Epidemiology from Emory University, and her Ph.D. in Parasitology from Tulane University North American Refugee Health Conference 11

14 Invited Plenary Speakers Saturday June 9th William Stauffer md, msph, dtm&h Professor, Departments of Medicine and Infectious Disease University of Minnesota Medicine Advisor, Centers for Disease Control and Prevention Minneapolis, MN Dr. Bill Stauffer is a Professor, Departments of Medicine, Division of Infectious Diseases. He also holds appointments in the Pediatrics, Infectious Diseases & in the School of Public Health. He is clinical faculty in Infectious Diseases HealthPartners Specialties Center/Regions Hospital. He is an expert in travel and tropical medicine working in clinical medicine, surveillance and policy development. He serves as the lead medical advisor to the Centers for Disease Control and Prevention (CDC), Division of Global Migration and Quarantine (Immigrant, Refugee, Migrant Health Branch) where he works on issues of human mobility and how it effects human health (e.g. travel medicine, refugee & immigrant health). He Co-Directs the UMN/CDC Global Health Course and other online courses. He works extensively overseas in clinical medicine education, research and in public health, most recently in Tanzania, Uganda, Thailand, Malaysia. His research areas have included infectious disease surveillance, malaria diagnostics, and neglected tropical diseases, cost evaluation of public health programs, strongyloidiasis and viral hepatitis. He acts as an advisor to the European Centers for Disease Control, the World Health Organization, and chairs an Inter-Governmental Refugee Health Work-Group aimed at aligning medical management of refugees. Theresa S. Betancourt scd, ma Salem Professor in Global Practice Boston College School of Social Work Director, Research Program on Children and Adversity Boston, MA Dr. Betancourt s central research interests include the developmental and psychosocial consequences of concentrated adversity on children, youth and families; resilience and protective processes in child and adolescent mental health and child development; refugee families; and applied cross-cultural mental health research. She is Principal Investigator of an intergenerational study of war/prospective longitudinal study of war-affected youth in Sierra Leone (LSWAY). This research led to the development of a group mental health intervention for war-affected youth that demonstrated effectiveness for improving emotion regulation, daily functioning and school functioning in war-affected youth. This intervention, the Youth Readiness Intervention (YRI), is now at the core of a scale-up study within youth employment programs now underway in collaboration with the World Bank and Government of Sierra Leone as a part of the NIMH-funded Mental Health Services and Implementation Science Research Hub called Youth FORWARD. Dr. Betancourt has also developed and evaluated the impact of a Family Strengthening Intervention for HIV-affected children and families and is leading the investigation of a home-visiting early childhood development (ECD) intervention to promote enriched parent-child relationships and prevent violence that can be integrated within poverty reduction/social protection initiatives in Rwanda. Domestically, she is engaged in community-based participatory research on family-based prevention of emotional and behavioral problems in refugee children and adolescents resettled in the U.S. She has written extensively on mental health and resilience in children facing adversity including recent articles in Child Development, The Journal of the American Academy of Child and Adolescent Psychiatry, Social Science and Medicine, JAMA Psychiatry, AJPH and PLOS One. Her work has been profiled in the New Yorker, National Geographic, NPR, CNN.com and in an interview with Larry King on the program PoliticKing. Janine Young md, faap Medical Director, Denver Health Refugee Clinic Associate Professor, Department of General Pediatrics University of Colorado School of Medicine Denver Health and Hospitals, Lowry Family Health Denver, CO Dr. Young is a general pediatrician at Denver Health and Hospitals and an Associate Professor at the University Of Colorado School Of Medicine. She was an undergraduate at Columbia University, received her medical training at Harvard Medical School, and pediatric residency at the Boston Combined Program and UCSF Benioff Children s Hospital in Oakland, CA. Her career focus has been in providing care for new immigrants and refugees and has presented talks nationally regarding the development of standard of care medical screening guidelines for these populations, including diagnosis and management of FGC in children. She is the lead author of the American Academy of Pediatrics Immigrant and Refugee Toolkit screening guidelines and has consulted for the Office of Refugee Resettlement on medical screening of unaccompanied children. Currently, she is the Medical Director of the Denver Health Refugee Clinic. She is funded through the State of Minnesota s CDC grant working to develop a Refugee Centers of Excellence.
