4 1. General Information 1.1 Web Site The DCHOA web site ( is the most current source of information from DCHOA. The web site contains the information in this document and much more. Reporting Game Misconducts, Match Penalties, etc. 2.1 Game Misconducts and Match Penalties (WAHA) Make sure the score sheet clearly indicates why the Game Misconduct or Match Penalty was assessed. Keep a clear copy of the score sheet, preferably the top copy. Let the coach and/or team official know that the player/coach is suspended from the next scheduled game (Game Misconducts, or from practices and games for 30 days or until a hearing (Match Penalty). First, report it on the WHOA website at: If you are having difficulty, call and report the incident to the WHOA Referee-in-Chief within 24 hours. Fans ejected for zero tolerance violations, fans fighting in the stands, officials confronted and or verbally abused off of the ice surface by anyone, threats made to an official leaving the building are examples of things reported on the Incident Report. 6 3936 Maple Grove Drive #12 Madison, WI Game Disqualifications (WIAA) The ejection of a player, coach, or fan of any WIAA member school must be reported on the Officials Report Form within 48 hours of the incident. The form is available at This applies to any ejection at any level varsity or JV. PLAYER EJECTION 1. Within the rules of the sport, announce the ejection by making eye contact with the ejected player. Do not make physical contact with the individual. Eye contact can be made from a distance of twenty feet, but should not require an official to be closer than six to ten feet from the athlete. 2. Request the athlete to go immediately to the bench and to the head coach. 3. One contest official will go to the sidelines, make eye contact with the head coach and announce that the athlete has been ejected from this contest and state the reason. Use the athlete's number, position, etc. to clearly identify him/her. 4. Unless athlete creates a disturbance, he/she may remain on the bench. Athletes should not be left unsupervised at any time. 5. It is vital that the coach knows the reason for which the athlete has been ejected. Communication is vital. 6. Official must fill out an Officials Report and forward it to the WIAA within 48 hours. Officials are not required to contact the school's athletic director to advise the administration of the ejection. The WIAA will forward a copy of the Officials Report Form to the appropriate school personnel upon receipt of the report from the official. COACH EJECTION 1. The ejection of a coach requires that the official approach the coach, announce the ejection to the coach and request that the coach leave the competition area. In addition, the official must ask which coach or school representative will function as the head coach in the ejected coach s absence. The school representative cannot be a parent from the stands, unless that parent is an employee of that school district. It is not the responsibility of the official to go through a drawn out procedure to verify the status of the replacement. 2. The ejected coach shall leave the competition area and go to the locker room (indoor sports) or the bus or other area away from the field of play (outdoor sports) before the contest shall continue. An ejected coach may not sit in the stands to watch the remainder of the contest. 3. If the coach refuses to leave, the contest shall be terminated immediately and the contest is ruled a forfeit. This should be your last resort, but may be necessary. 4. Officials must fill out an Officials Report Form and forward it to the WIAA within 48 hours. As with the player ejection, you do not have to contact the involved schools. The WIAA will forward a copy of the report to the appropriate school personnel. 5. The appropriate school official will be asked to respond to the Officials Report Form by sending a written response to the WIAA, which shall include a summary of the action taken by the school against the coach. Coaches are NOT required to miss the next event for having been ejected from a contest. This rule applies only to players. FAN EJECTION Officials should never become involved with crowd control/unsportsmanlike conduct concerns of spectators. If a problem occurs, the contest should be stopped. The officials should request that contest management resolve the problem before play is allowed to resume. If the host school does not have an administrator or contest manager, the coach must assume this responsibility. Crowd control problems must also be reported on the Officials Report Form within 48 hours. Whenever an official submits an Officials Report Form to the WIAA, in return they will receive a blank report form and a copy of the initial letter sent to the school. Constitution and By-Laws for the Dane County Hockey Officials Association, Inc. Article I - Purpose The purpose of the Dane County Hockey Officials Association, Inc. (hereinafter called the DCHOA) is to register and train hockey officials in Dane County and surrounding areas for the purpose of improving the quality of amateur hockey in the area; to provide a forum for discussion and a medium for dissemination of information on hockey rules and interpretations; to insure uniformity of rules interpretation; to develop more efficient officials; to maintain the highest standards of officiating; and to create a better understanding between officials, coaches, and players. In addition, the DCHOA will cooperate with any organization in assigning all amateur league games, out-of-town games, and all tournament games in Dane County. The qualifications of said officials will be the responsibility of DCHOA. In order to accomplish these purposes, the DCHOA will offer clinics in rules, skating and procedures; test officials; critique officials; and maintain close contact with USA Hockey and its District Referee-in-Chief. Section 1 Membership Qualifications Article II - Membership Membership is restricted to persons having paid the annual dues of the DCHOA. Membership in the DCHOA shall be renewed each season. The membership season shall be December 1 through November 30 each year. All Honorary Life Members shall be nominated by the Board of Directors and approved by a majority vote at a general membership meeting. Honorary Life Members shall have no financial obligation to the DCHOA. Section 2 Voting Rights Each properly registered member of the DCHOA is entitled to one vote on any issue requiring the consent of the general membership. The president has no voting rights at any meeting, with the exception of where a vote needs to be cast to break a tie. Article III - Jurisdiction The DCHOA shall have final jurisdiction over all DCHOA-registered officials working DCHOA-assigned games. Section 1 General Meetings Article IV - Meetings General meetings will be held as deemed necessary by the Board of Directors. The annual meeting shall be held in the spring of each year for the purpose of electing Directors and for the transaction of any other business which may come before the membership. Section 2 Special Meetings Special general meetings of the membership may be called by the President, by majority vote of the Board of Directors, or by not less that one-fifth of all the members of the DCHOA in attendance at a regular meeting. 9 Section 3 Notice of Meetings In the event that a special general meeting is required, written notice shall be delivered not less than seven nor more than 30 days prior to the date of the meeting. In the event that a special meeting of the Board of Directors is required, written or oral notice will be given not less than 24 hours prior to the meeting. Section 4 Quorum A majority of the members of the Board of Directors shall constitute a quorum at any meeting. Section 5 Proxies Members of the DCHOA or its Board of Directors may not be represented by proxy. Section 1 General Powers Article V - Board of Directors The Board of Directors shall have the power to run the business and general affairs of the DCHOA with final approval of the general membership where required by these By-Laws. Section 2 Duties The duties of the Board of Directors are as follows: 1. Elect a President, Referee-in-Chief, Vice President, Treasurer and Secretary from within the Board of Directors within 30 days of the annual meeting. 2. Appoint a Scheduler. 3. Appoint chairmen for all standing committees. 4. Hear and rule on any matters or disputes for which another procedure is not specifically provided for in these By-Laws. 5. Attend meetings of committees to which the Board member has been assigned. 6. Set game fees and fines. 7. Nominate all Honorary Life Members. Section 3 Number of Members The number of Directors of the DCHOA shall be 14. There are twelve elected Directors from the general membership. The appointed Scheduler shall be an ex-officio member of the Board of Directors with full voting rights. The current Region 4 Hockey Council (R4HC) Vice President shall be a non-voting ex-officio member of the Board of Directors. The R4HC Vice President shall not be required to be a member of the DCHOA as stated in Article II, Section 1. Each elected Director shall hold office for two years, each officer for one year; six Directors will be elected each year. In the event of a vacancy by a Director due to resignation, removal, or for other reason, the President shall appoint a voting member of the DCHOA to fill such unexpired term; in the event the President fails to make the appointment within 90 days from the date of vacancy, the Secretary shall call for a special election of the general membership to fill said vacancy. Section 4 Meetings Meetings of the Board of Directors will be held as deemed necessary by the President or any other officer or Board member according to the By-Laws. Section 5 Guests The President may invite other interested parties, including the USA Hockey District Referee-in-Chief, or the Wisconsin Hockey Officials Association (WHOA) Referee-in-Chief, to attend Board meetings. These guests may not vote at any Board meeting unless they have been properly elected to the Board of Directors under the provisions of these By-Laws. Section 6 Removal Any member of the Board of Directors may be removed by a vote of nine or more of the voting members of the Board of Directors when the Board judges that the removal is in the best interest of the DCHOA. 10 Section 1 Number Article VI - Officers The officers of the DCHOA shall be the President, Referee-in-Chief, Vice President, Treasurer and Secretary. Section 2 Removal Any officer may be removed by a vote of nine or more of the voting members of the Board of Directors when the Board judges that the removal is in the best interest of the DCHOA. Section 3 Vacancies Any vacancy in any office shall be filled by a member of the Board of Directors for the unexpired portion of the term. The vacant Board position will be filled by the Board of Directors from the general membership. Section 4 President The President shall be the chief executive of the DCHOA and shall, in general, supervise and control all the business and general affairs of the DCHOA. The President shall preside at all meetings, serve as an ex-officio member on all committees (except the Board of Directors), call special meetings of the DCHOA or Board of Directors that are considered necessary, and appoint committees as required by these By-Laws or as deemed necessary. Section 5 Referee-in-Chief The Referee-in-Chief will be responsible for all activities of the DCHOA officials with respect to training, rules, procedures and conduct. The Referee-in-Chief shall be responsible for coordinating these activities with USA Hockey through the District Referee-in-Chief. S/he shall be a member of the Rules and Registration Committees, an ex-officio member of the Ethics Committee, and Chairman of the Education Committee. Section 6 Vice President In the absence of the President or the event of the President s inability or refusal to act, the Vice President shall perform the duties of the President, and when so acting shall have all the powers of the President. The Vice President shall be the designated representative to the R4HC. Section 7 Treasurer The Treasurer shall have charge over, custody of, and be responsible for all the funds of the DCHOA. The Treasurer shall provide an annual report at the final meeting and any other reports deemed necessary by the President or the Boards of Directors. Section 8 Secretary The Secretary shall keep the minutes of all meetings, see that all notices are given in accordance with these By-Laws, and in general perform all duties incidental to the office of the Secretary. Article VII - Elections The slate of candidates for the Board of Directors shall consist of a minimum of seven nominees to be presented at the February general meeting. Further nominations from the floor will be open at the March general meeting. All general elections for Board of Directors positions shall be done by paper ballot. The newly elected Board will assume control at the close of the March general meeting. Section 1 Procedure Article VIII - Amendments The Constitution and By-Laws may be amended or changed at a meeting of the general membership held as per Article IV Meetings. Each proposed amendment must be submitted to the President who shall present it to the Rules Committee. The Rules Committee shall have 30 days to decide whether or not to recommend its adoption at the next general meeting. No amendment shall be eligible for adoption unless it has been presented to the membership in person, by mail or by electronic communication at least thirty (30) days prior to the general meeting in which it will be voted on. 11 Section 2 Adoption The proposed amendment shall be presented at the next general meeting, with or without the Rules Committee s recommendation. After discussion at the meeting, a vote of two-thirds of the eligible voters present shall be required to adopt the amendment. Section 3 Amendments Not Requiring General Membership Approval Changes to the Constitution and By-Laws necessary to coincide with the rules and regulations of USA Hockey or other affiliate member shall automatically be incorporated into the DCHOA Constitution and By-Laws. Such changes do not require approval from the general membership. Article IX - Parliamentarian The Secretary shall act as Parliamentarian at all meetings and the DCHOA shall follow Robert s Rules of Order. 12 5. General Rules A. Registration All Dane County Hockey Officials Association, Inc. (DCHOA), USA Hockey, and WHOA registrations must be completed by December 1 st of each year for all officials who have registered the previous season with the DCHOA. No official will be assigned association games until s/he possesses a current and updated registration card. Registration fees are the individual responsibility of each official. Officials registering with DCHOA must submit the registration fee with the application form. For purposes of voting, registration will be considered complete as soon as the DCHOA fee is paid. B. General Meetings Meetings are typically scheduled for the third Monday of every month starting in August and ending in April. General Meetings consist of an educational portion and a business portion. All members are required to be present for the educational portion at all meetings for the entire time unless previous arrangements are made with an officer of DCHOA. The time and location for meetings is available on the DCHOA website. Alternative meetings may be held on different evenings at different locations. The minutes of all DCHOA Board meetings that have been held since the previous general meeting shall be available at the following general meeting. Game assignment priority will be given to officials in attendance at the General meeting (or alternative meeting) and to officials who have an excused absence. C. Board Meetings The Board will meet as needed and in accordance with the Bylaws and other Rules. Notice shall be provided for all regular Board meetings at least 7 days in advance of the meeting. Notice shall be provided to each Board member via , telephone or in person. Note: Article IV, Section 3 of the Bylaws requires 24 hour notice for Special meetings of the Board. Officer elections will be held at the first scheduled Board meeting following the Annual General meeting, per Article V Section 2.1 of the Bylaws. Note: In the event that there are no remaining elected Officers of the Board after elections are held per Article VII of the Bylaws, Officer elections will be held at the Business Meeting held directly after the Annual General Meeting. The Business Meeting will be presided over by the outgoing President (or highest ranking outgoing Officer in attendance) until such a point as new officers are elected. Board meetings may be conducted in person or by other means when all members of the Board have an equal opportunity to participate. Board business may be conducted via in such a case that: 1. The President rules that the specific item of business has been adequately discussed by the Board in advance. 2. Adequate notice is given to the Board 3. All Board members are given a reasonable opportunity to participate 4. No amendments to the rules and bylaws are made 5. No Board member objects to the specific item of business being conducted via . Note: Motions approved via shall be recorded in the minutes at the next regular or special meeting of the Board. Section 1 General D. Committees The size of any committee in the succeeding sections will be the number stated, plus the President and any other exofficio members. Each committee will consist of at least one member of the Board of Directors who will be responsible for reporting to the Board of Directors. Ad hoc committees may be created for the necessary length of time. Section 2 Audit Committee The Audit Committee will consist of a minimum of three members of the DCHOA. 13 The committee shall be responsible for overseeing the books and entries of the Treasurer. The committee shall inspect all books after they are closed, as soon as possible after the close of the fiscal year. The committee shall compile a report to be presented at the June Board of Directors Meeting and the September General Meeting. Section 3 Education Committee The Education committee will consist of a minimum of three members of the DCHOA. The committee chair shall be the Referee-in-Chief. The committee shall be responsible for the presentation of developmental and instructional materials, as deemed necessary, the General Meetings. The committee shall be responsible for providing educational and instructional opportunities to all members of the DCHOA. Section 4 Equipment Committee The Equipment Committee will consist of a minimum of three members of the DCHOA. The committee shall be responsible for ordering and maintaining a record of all equipment and apparel purchased by DCHOA. The committee shall be responsible for maintaining and distributing equipment for use by first year officials. Section 5 Ethics Committee The Ethics Committee will consist of a minimum of four members of the DCHOA. The Referee-in-Chief and Scheduling Secretary shall be ex-officio members of the committee. The committee shall be responsible for hearing and ruling on complaints and taking appropriate action against any members for violations of the Constitution and the By-Laws or DCHOA Rules or for any conduct detrimental to amateur hockey or the DCHOA. Section 6 Information Committee The Information Committee will consist of a minimum of three members of the DCHOA. The Information Committee shall be responsible for maintaining the DCHOA website, producing and distributing the DCHOA newsletter, and maintaining and updating all social media of DCHOA. The Information committee shall be responsible for producing information pertaining to recruiting new members and retaining current members. Section 7 Mentoring Committee The Mentoring Committee will consist of a minimum of four members of the DCHOA. The Referee-in-Chief and Scheduler shall be ex-officio members of the committee. The Mentoring Committee shall be responsible for overseeing and maintaining a record of the feedback provided to members of the DCHOA. The Mentoring Committee shall produce an end of season report of the feedback provided to member officials, and present it to the Education Committee Section 8 Nominating Committee The Nominating Committee will consist of a minimum of three members of the DCHOA. The Nominating Committee shall be responsible for presenting a slate of candidates for the Board of Directors. The nominations must be announced at the January Meeting. Section 9 - Rates Committee The Rates Committee will consist of a minimum of three members of the DCHOA. The Rates Committee shall be responsible for determining the rates to be paid for games assigned through the DCHOA and presenting the rates to the organizations for which the DCHOA assigns officials. Section 10 Recruiting Committee The Recruiting Committee will consist of a minimum of three members of the DCHOA. The Recruiting Committee shall be responsible for coordinating efforts to recruit new officials and retain current officials. Section 11 - Referees Room Committee The Referees Room Committee will consist of a minimum of three members of the DCHOA. The Referees Room Committee shall be responsible for maintaining and improving the locker room facilities provided to the members of the DCHOA. 14 Section 12 Rules Committee The Rules Committee will consist of a minimum of three members of the DCHOA. The Rules Committee shall be responsible for the consideration, introduction and interpretation of proposed rule changes or amendments to the Constitution and By-Laws of the DCHOA. Section 13 Special Events Committee The Special Events Committee will consist of a minimum of four members of the DCHOA. The Committee Chair shall be the Scheduler. The Referee-in-Chief shall be an ex-officio member of the committee. At least one member of the committee shall be a member of the DCHOA/USA Hockey evaluation staff. The Special Events Committee shall, when requested to do so by the Scheduler, assist in scheduling non-wiaa Special Events games. Section 15 Committee Ratification All Committees, standing or otherwise, Committee Chairpersons and members, are subject to review and approval by the Board of Directors. E. Scheduler The Board of Directors shall appoint the Scheduler. The Scheduler shall be responsible for the equitable assignment of qualified officials for all DCHOA assigned games, except where otherwise noted. The Board of Directors will determine the assigning procedure in reference to assigning all DCHOA assigned games. The Scheduler shall keep records of assignments, as necessary. The scheduler shall at all times ensure that at least one other member of the Board of Directors is functionally familiar with the scheduling process. F. Game Assignments In this section, any reference to the appropriate governing body shall refer to USA Hockey for USA Hockey games, WIAA for WIAA games, etc. Assignment of game officials will be the responsibility of the appointed Scheduler or appropriate scheduling committee. The intent of the DCHOA is to assign all games in accordance with DCHOA rules and regulations and in accordance with the appropriate governing body guidelines. The officials of the DCHOA shall receive all games assigned through the DCHOA from the Scheduler or the Special Events Committee. Any member of the DCHOA who is solicited for the purpose of assigning officials in any capacity is required to inform the DCHOA Board of Directors, the Scheduler, or the appropriate assigning committees in order to avoid assignment conflicts. Officials accepting games from other sources are personally responsible for making arrangements for payment of game fees. The following procedures must be followed when you cannot make a particular DCHOA game assignment: 1. If you are scheduled for a game at a time when you did not make yourself available, you must the Scheduler at: Scheduler@DCHOA.org immediately. If you do not receive a reply within 48 hours, contact the Scheduler again. You are responsible for the game until you receive a reply from the Scheduler. 2. You are responsible for finding a replacement for yourself for any DCHOA assigned game for which you cannot work. The replacement must be from the list of DCHOA registered officials and the official must be qualified to officiate at the game level based on their registration with the appropriate governing body or bodies and on skill level. Except under special circumstances, the official must place the game or games on the Open Games list of the DCHOA web site and the replacement must request assignment through the web site. Officials may find replacements and notify the Scheduler but the replacement process (except under special circumstances) must be through the Open Games on the web site. You are responsible for the game until receiving notification that a replacement has been made. 3. If you fail to find a qualified replacement prior to the game, and/or if you fail to show up at the game, you will be fined and disciplined as described in the NO-SHOW policy. If you accept a previously scheduled game from another official (other than the Scheduler), you must the Scheduler at Scheduler@DCHOA.org as soon as possible, giving your name, the name of the official you replace, game date, game time, and game location; Note: this is only necessary if the game was accepted under special circumstances and not through the Open Games on the web site. You should send this before the game date, if possible. If the is sent after the game fees are reconciled for the month, you will not receive credit for working that game and you will be responsible for collecting the game fee from the original official. 5. Officials are encouraged to stay after their last game until the start of the next game. If an official is late or fails to show, try to stay and work the game. It is generally not necessary to stay and work an unscheduled game if there are at least two scheduled officials present for the game; Exception: all WIAA games require three WIAA licensed officials to start the game. If you officiate a game alone, you will be compensated double the game fee. If three officials are scheduled and only two officiate the game, you must report it to the Scheduler, and the officials who work the game will be paid based on the two-official scale. the Scheduler and report all no shows so that you can receive proper credit for working that game and so that the official who was late or failed to show can be contacted by the Scheduler to start disciplinary actions if necessary. 6. If officials request assignment to Open games on the web site, they do not receive assignment of that game until notified by the Scheduler. However, they are assumed available for that game until (A) their request is denied, or (B) they withdraw the request through the web site. Courtesy s are sent for each of the aforementioned actions but it is the responsibility of the official to know the status of the request. Game assignments will be posted on the DCHOA web site and a courtesy will be sent. It is the responsibility of the official to know his or her schedule. Your attendance at the monthly meetings is mandatory for you to receive preference for game assignments, unless an officer of DCHOA has approved the absence. All officials must possess a current USA Hockey registration card before being assigned any USA Hockey games. A WIAA registration card is required before being assigned any WIAA games. WHOA registration also must be completed before any USA Hockey games can be assigned. All officials must be fully registered with DCHOA to submit availability or be assigned games. This includes having their DCHOA fees paid for the dates of the games. H. The Mentoring Program The Mentoring Program shall be a voluntary program where experienced officials are asked to act as contact people and as mentors for officials. In general, each participating adult official will be asked to act as a role model for a group of officials in order to promote the development of self-confidence and professionalism in officiating. Specific points of emphasis include: 1) Providing feedback on games watched and monitoring any feedback received from peers through the website. 2) Being available for consultation regarding rule interpretation and the basic mechanics of officiating. 3) Being available to field exam questions. 4) Being available to go watch games for each official in their group. In general, the goal is for the experienced participants to be that friendly face they know from DCHOA who acts as a point person for all their questions and concerns. I. Payment Periods Payments to the officials for all games assigned through the DCHOA are to be made directly by the requesting party to the official. Each month the DCHOA will provide information to all requesting parties for payments to be made to officials and for scheduling fees. Payments to the officials shall be received by the officials no later than the third Monday of the month after the games took place. The official shall be responsible for ensuring that he/she has been paid for all games. If payment is not received the official shall report to the President and appropriate Scheduler the amounts and games not paid. The DCHOA will hold requesting parties responsible for the payment of all officials in a timely manner. Section 1 Promptness J. Game Policy Tardiness and laziness by any official will not be tolerated. All officials must arrive at the rink at least 15 minutes prior to the scheduled game time. For Special Event and Tournament games, officials must arrive at the rink at least 30 minutes prior to teams taking the ice for warm-ups (NOTE: Different leagues may run their warm-up differently. 16 the responsibility of the official to be aware of the league rules.). An official who has repetitive violations of the Game Policy shall be referred to the Ethics Committee for disciplinary consideration. Tardy: All officials must be fully dressed and ready for the ice 5 minutes before warm-ups begin. Officials must be on the ice before the teams can warm up or re-enter the ice after a resurfacing intermission. Any official who does not comply with these requirements shall be considered tardy. Late: Any official that is ready to work after the start of the game but prior to the completion of the first period shall be considered late. An official who is late will be paid for working the game but will be fined the game fee. No-Show: Any official who refuses to work a game upon arrival, or an official who is ready to work after the completion of the first period, shall be considered a no-show (see No-Show Policy). Under all circumstances, every effort shall be made to work the game. All actions will be considered during a review by the Ethics Committee as prescribed in Disciplinary Actions. In addition to disciplinary consideration for violations of the Game Policy, officials may also be required to reimburse the clubs for the cost of ice time lost in the event they are not on the ice at the proper time. It is the ultimate responsibility of the officials to ensure that they are able to re-enter the ice at the scheduled time. Violations of this policy shall be sent to the Scheduler. Section 2 Dress Code All officials must be in proper uniform when working games. This uniform consists of: 1. A black hockey helmet. 2. A black and white striped pullover sweater. The approved style is over the trousers, not tucked in. For USA Hockey games, a current crest must be properly attached. No crest should be attached for WIAA games. 3. Turtlenecks are not allowed. Visible colored T-shirts are not allowed unless they are black. 4. Black trousers loosely fit to allow free movement (no jeans, sweat pants, or wind pants). 5. Shirt and trousers must be clean and wrinkle-free. 6. Polished skates and clean white laces. 7. No gloves. 8. A Fox 40 Force or Acme Thunderer finger mount whistle. 9. Refer to the proper USA Hockey officiating manual for additional dress requirements. Section 3 Score Sheets Make sure that the score sheet is properly filled out. At the bottom of the score sheet sign your name and print your DCHOA number legibly. One official must retain a copy for their records for any games in which a Game Report must be filed. Section 4 Serious Penalties Make a note at the bottom of the score sheet of any problems, Game Misconduct Penalties, Match Penalties, or Game Disqualifications. For all games involving a USA Hockey team, make sure to report all Game Misconduct Penalties and Match Penalties to WHOA. You must submit the report to the WHOA website ( within 24 hours of the conclusion of the game For all games involving a WIAA team, you must report any Game Disqualification on the WIAA web site within 48 hours of the conclusion of the game. K. No Show Policy If you fail to find a qualified replacement prior to the game, and/or if you fail to show up at the game, you will not be paid for the game and will be fined double the game fee. If this situation occurs twice within any 12-month period, you will be suspended for the remainder of the month and the following month. In either case, the Ethics Committee may review the incidents. If the situation occurs for the third time during a 12-month period, you will be suspended for the following 12 months. Suspensions begin immediately upon notification by the Scheduler. Appeals of fines or suspensions should be addressed to the DCHOA President within 30 days of suspension and the Ethics Committee shall hear the appeal. The decision of the Ethics Committee can be appealed to the Board of Directors following the appeal process described in Disciplinary Actions. L. Disciplinary Actions Violations of the rules set forth in the DCHOA By-Laws and in the DCHOA General Rules will be dealt with in the following manner, unless other procedures are specifically listed in the By-Laws or General Rules. Each official shall be notified in writing of a violation for which disciplinary action may be imposed and the nature of the possible disciplinary action (for example: fine, suspensions). The official will be given an opportunity to present their version of Dane County Hockey Officials Association DCHOA Manual: /16/2018

17 the incident orally or in writing to the Ethics Committee. The Ethics Committee shall determine whether or not to impose any disciplinary action and shall notify the official in writing of the Committee s decision within 60 days of receiving the complaint. A disciplined official may appeal the decision of the Ethics Committee to the Board of Directors in writing, within 30 days of receiving the written decision of the Ethics Committee. The official shall be notified in writing of the decision of the Board of Directors within 30 days of the Board meeting at which time the appeal shall be heard unless unusual circumstances warrant a longer period of time. The decision of the Board of Directors is final. A written record of all disciplinary actions, hearings, and appeals shall be kept by the Secretary. All appeals should be addressed to the DCHOA President. A suspended official may not accept transfer games from another DCHOA official. Examples of offenses for which disciplinary action may be imposed: 1. Not showing up for an assigned game (see Game Assignments and No-Show Policy) 2. Sending an unqualified replacement to a game (see Game Assignments) 3. Dress (see Game Policies) 4. Late arrival (see Game and No Show Policies) 5. Failure to follow DCHOA rules 6. Any conduct detrimental to amateur hockey and USA Hockey (including, but not limited to, drinking in uniform, chewing tobacco on the ice, and use of profanity while representing DCHOA and/or USA Hockey) M. Special Events and WIAA Policy All Special Events contests will be assigned by the Special Events Committee. Special Events contests will include, but are not limited to, the following: AAA Major Bantam and AAA Midgets AAA 19U Girls WAHA Regional Playoffs and State Tournaments USA Hockey Tier I and II Playoffs and Tournaments ACHA games Great Lakes Hockey League games WIAA Junior Varsity and Varsity Any DCHOA member who is contacted regarding scheduling any DCHOA Special Events level game shall immediately forward the request for assignment to the Chair of the corresponding committee unless the individual is acting in an official capacity (e.g., USA Hockey) outside DCHOA. No DCHOA member shall assign or assist in the assignment of any Special Events level game outside of the committees without the approval of the DCHOA Board of Directors. Disciplinary action may be imposed for the following violations of Special Events Policy: Missing an assigned game, including late arrival (i.e., after start of game) Sending an unqualified replacement to a game, including failure to get approval for the replacement from a member of the Special Events Committee. Improper off-ice dress; inappropriate attire includes jeans, shorts, sweats, warm-ups, T-shirts, tennis shoes, and sandals. Failure to arrive by the required arrival time: WIAA Junior Varsity games: 30 minutes prior to contracted game time WIAA Varsity: 60 minutes prior to contracted game time All other Special Events games: 30 minutes prior to game time, unless stated otherwise Failure to follow DCHOA rules Any conduct detrimental to amateur hockey Each official shall be notified in writing of a violation for which disciplinary action may be imposed and the nature of the possible disciplinary action. The official will be given the opportunity to present his/her version of the incident either orally or in writing to the Ethics Committee. The Ethics Committee will then determine whether or not to impose any disciplinary action and shall notify the official in writing of the Committee s decision within 14 days of receiving the complaint. The official may appeal the decision of the Ethics Committee to the DCHOA Board of Directors, in writing, within 7 days of receiving the written decision of the Ethics Committee. The official shall be notified in writing of the decision of the DCHOA Board of Directors within 7 days of the Board meeting at which the appeal was heard. The decision of the DCHOA Board of Directors will be considered final. All appeals must be addressed to the DCHOA Dane County Hockey Officials Association DCHOA Manual: /16/2018

For violation of the Special Events Policy, the following disciplinary action may include: First offense in a 12-month period: Fine equal to triple the game fee; Removal and replacement from all DCHOA assigned games, including Special Events games, for the next 30 days; A letter detailing the disciplinary action taken will be forwarded to the appropriate parties. Second offense in a 12-month period: Fine equal to triple the game fee; Removal and replacement from all DCHOA assigned games, including Special Events games for the next 12 months; A letter detailing the disciplinary action taken will be forwarded to the appropriate parties. Disciplinary actions taken by DCHOA with regard to violations of Special Events Policy are in addition to any other non-dchoa imposed sanctions.
