The purpose of this letter is to request approval of a contract amendment to the FFY contract with DFS for SNAP-Ed to address increased printing and mailing costs. As you know, the Human Services Department (HSD) and the Health Services Agency (HSA) were awarded a combined total of 500,000 from the California Department of Social Services (CDSS) and the California Department of Public Health (CDPH) for the SNAP-Ed Plan. HSD received 250,000 for the period of October 1, 2011 through September 30, 2012 to provide nutrition education and obesity prevention services to low-income families in Santa Cruz County. A component of the plan provides nutrition.information to CalFresh applicants and recipients, and HSD contracted with DFS to provide printing and mailing services of nutritional materials to CalFresh recipients. An amendment is needed to increase the contract budget by 1,210 as the printing and mailing services have increased as a result of the CalFresh caseload rising at a higher rate than originally anticipated. The increase in the DFS contract has been approved by CDSS and there is no increase in County costs. 18

4 Contract No AMENDMENT TO AGREEMENT The County of Santa Cruz, by and through the Human Services Department (HSD), hereinafter referred to as "COUNTY" and Document Fulfillment Services (DFS) hereinafter referred to as "CONTRACTOR," hereby amend contract number as stated below. Amendments to Agreement: 1. Paragraph 2 subpart A entitled "COMPENSATION," is amended to increase the reimbursement costs, in accordance with the expansion of contracted services, from 65,604 to 66,814 and is modified as follows: Mailing services compensation as agreed on a per-unit basis as stated in Attachment 2 - DFS Pricing Schedule & Budget: SNAP-Ed Project Print and Mailing Services. Compensation for services not to exceed 66, Attachment 2, "Document Fulfillment Services (DFS) Snap-Ed Project Printing and Mailing Services" (attached), is amended to reflect a total project budget cost of 66, All other provisions of said contract, excepting those mentioned above, shall remain the same.
