FORM OF CERTIFICATE OF POSTING, PUBLICATION AND SEPARATE MAILING TO WATER CUSTOMERS AND RESALE CUSTOMERS OF THE SUGAR CREEK FALLS PUBLIC SERVICE DISTRICT'S NOTICE OF APPLICATION TO CHANGE RATES Pursuant to the requirements of Rule 19-A of the Commission's Rules of Construction and Filing of Tariffs, I certify that I am Counsel for the Sugar Creek Public Service District, a public utility, furnishing potable water distribution services in Brawton, Clay and Cilmer Counties, West Virginia. On the day of May, 201 7, the District filed an Application to change its tariff pursuant to Rule 19A and that notice to the public of the Staff recommended rates with regard to this application has been posted, published, and the District's customers have been provided with individual notice on their December 2017 bills; as required by the applicable rules and regulations or Tariff Rule 19A as follows: I. That the Sugar Creek PSD utilizes a postcard bill and on the December 2017, customer bills, the following language was placed on each individual customer's bills, "This utility is seeking a rate increase. Details in newspaper publications or by calling (304) " 2. That on or about thel9th of December, 2017, or sometime thereafter a District representative posted the Notice of Application To Change Rates proposed by the Commission Staff for public inspection at the business office of the Sugar Creek Public Service District. 3. That on or about December 27'20 17, and January 3,2018, the Notice of Application To Change Rates was, or will be, published in the Braxton Citizens' News. An Affidavit of Publication from the aforesaid newspaper, evidencing publication, is attached hereto and made a part of this case file. The Notice was also posted at the Braxton County Courthouse, Sutton, West Virginia. Also, a Press Release was issued to the Braxton Citizens' News. a newspaper published and of general circulation in the county in which the District provides potable water distribution services, containing the substance of the Notice of Application To Change Rates.

5 ", PUBLIC SERVICE COMMISSION OF WESTVIRGINIA CHARLESTON TARIFF FORM NO. 10 NOTICE OF APPLICATlONTO CHANGE RATES (Tariff Rule 19-A) CASE NO PWD-19A NOTICE OF APPLICATION TO CHANGE RATES On May 22, 2017, Sugar Creek Public Service District filed with the Public Service Commission of Wes Virginia, an Application requesting increased rates and charges for furnishing water service to approximatel! 585 customers in Braxton, Clay, and Gilmer Counties, West Virginia. The District did not request specific rates. The Staff of the Public Service Commission have reviewed the Application and have recommended increase( rates and charges designed to produce approximately $ annually in additional revenue, an overall increasc of 8.59%, for 24 months. After 24 months, rates will decrease by 4.93%.
