Please complete, sign and send to: Los Portillos Maintenance Corporation, c/o AMMCOR, 910 Calle Negocio, Suite 200, San Clemente, CA along with a deposit check in the amount of $ payable to Los Portillos Maintenance Corporation. This deposit is refundable upon submittal of the completion notice found on page 3. The approval process takes approximately one month. Any work completed before approval of the Architectural Committee constitutes a violation of the CC&R s and you may have to modify or remove the unauthorized work or improvements at your own expense. Read CC&R s Section XII Architectural Control, Pages Read Homeowner s Manual, Section 6, Grading and Landscaping. 3. Complete this form in its entirety. 4. Please enclose sketches and detailed plans/specifications that provide the following information. A. Complete dimensions of improvement proposed (drawn to scale). B. Measurements of improvements in relationship to home and lot lines. C. Identification of building materials and color scheme to be used. (Provide color chips). D. Drawing showing affected elevations (patio covers, gates, etc.) E. Description of how drainage will be affected. F. For landscape improvements provide a Landscape site plan indicating scope of work, material and plants to be used. 5. Deposit will be reimbursed at time of completion of improvement and verification, by Architectural Committee, that the Common Area has not been damaged. Deposit to be made payable to Los Portillos Maintenance Corporation. 6. 4 NEIGHBOR AWARENESS: The intent of this section is to advise your neighbors who own residences adjacent to yours of your planned improvement. Failure to obtain these signatures could delay review and/or approval in cases where visibility, drainage, nosie, etc. are involved. Neighbors who disagree may be invited to attend the Architectural Committee Meeting to discuss this proposed improvement.
