Please complete, sign and send to: Los Portillos Maintenance Corporation, c/o AMMCOR, 910 Calle Negocio, Suite 200, San Clemente, CA along with a deposit check in the amount of $ payable to Los Portillos Maintenance Corporation. This deposit is refundable upon submittal of the completion notice found on page 3. The approval process takes approximately one month. Any work completed before approval of the Architectural Committee constitutes a violation of the CC&R s and you may have to modify or remove the unauthorized work or improvements at your own expense.
