GASPARILLA ISLAND BRIDGE AUTHORIT BRIDGE/ROADWAY REPLACEMENT LIMITED AD VALOREM TAX AND BOND REFERENDUM In addition to toll revenues, shall Gasparilla Island Bridge Authority levy an annual limited ad valorem tax not exceeding 0.30 mills to secure bonds, to be issued in one or more series in a principal amount not exceeding $48,000,000, bearing interest at not exceeding maximum legal rate, maturing not more than 30 years from issuance to finance/refinance costs of replacing existing fixed and swing bridges and roadway replacement? 2011 MULTI-COUNTY (Charlotte and Lee County) RESULTS FAILED (Charlotte and Lee County) ELECTION

3 6/14/2013 4:55 PM TICE FIRE PROTECTION AND RESCUE SERVICE DISTRICT TICE FIRE PROTECTION DISTRICT ELECTION TO INCREASE THE MAXIMUM AVAILABLE MILLAGE TAX RATE To fund the operating costs that are necessary to provide for and maintain life safety and property protections, shall the Tice Fire Protection and Rescue Service District be authorized to increase the annual ad valorem maximum millage tax rate that it may levy within its boundaries from 3.0 mills ($3.00 for every $1, of property value), which was approved by referendum in 1993, to 3.75 mills ($3.75 for every $1, of property value)? UPPER CAPTIVA FIRE PROTECTION AND RESCUE SERVICE DISTRICT FIRE STATION IMPROVEMENTS NON-AD VALOREM SPECIAL ASSESSMENT 2012 UPPER CAPTIVA FIRE PROTECTION AND RESCUE SERVICE DISTRICT FAILED FAILED A proposal for levy, collection and enforcement of a non-ad valorem special assessment upon all non-exempt real properties on North Captiva Island; to fund capital improvements to the fire station; constructing an addition to the fire station and making changes to the existing station to accommodate the addition; for an assessment not to exceed $970,000.00; establishing method of assessment; repayment options, computation of assessments; and providing for liens. Shall the non-ad valorem special assessment be approved?

Shall the Town of Fort Myers Beach issue bonds, including general obligation bonds, maturing not later than 30 years from their issuance, in a principal amount not exceeding $7,000,000, bearing interest at market rates, to finance the acquisition and construction of a Town Hall, which bonds may be secured by and payable from ad valorem property taxes levied without limit on all taxable property within the Town in addition to other available revenue sources? ORDINANCE CHARTER AMENDMENT TO ALLOW COUNCIL TO ESTABLISH PERSONNEL STANDARDS WITHOUT RECOMMENDATION OF CITY MANAGER This proposal amends the Cape Coral City Charter by permitting City Council to establish personnel standards without the requirement of a recommendation by the City Manager. ORDINANCE CHARTER AMENDMENT ALLOWING COUNCIL MEMBERS TO DEAL WITH CITY EMPLOYEES DIRECTLY REGARDING INQUIRIES AND INVESTIGATIONS This proposal amends the Cape Coral City Charter by clarifying how City Council Members may deal with City officers and employees who are subject to the direct supervision of the city manager directly without going through the city manager for routine informational inquiries and Council authorized investigations. 5 6/14/2013 4:55 PM 5 ORDINANCE AMENDMENT ALTERING AMOUNT REQUIRED TO BE PAID UPON TERMINATION OF ASSISTANT MANAGERS AND DEPARTMENT HEADS 2011 CITY OF CAPE CORAL PASSED This proposal amends the Cape Coral City Charter by changing the severance pay required to be paid when the City Manager terminates the employment of Assistant City Managers and Administrative Department Heads, at will, from four months salary to an amount up to four months salary. CHARTER PROHIBITION OF CITY POLICY DISCRIMINATING AGAINST PERSONS 18 YEARS OLD OR OLDER Ordinance No amends Fort Myers City Charter to prohibit a city ordinance or policy, within the city code or otherwise, which discriminates against persons having reached the age of majority (18) based on race, creed, color, national origin, sex, sexual orientation, age, military status, or marital status. AMENDING CITY CHARTER TO REQUIRE AN ELECTED OFFICE OF CHIEF OF POLICE Ordinance No amends Fort Myers City Charter to require creation by ordinance of an elected office of chief of police who shall be a City resident of the City, with a four year term, elected by registered voters of the City of Fort Myers. CHARTER AMENDMENT ONE CLARIFY COUNCIL FORM OF GOVERNMENT Section 8 of the Charter shall be amended to clarify that unless explicitly stated otherwise in the Charter, the terms City Council or Council as used throughout the Charter shall mean the Mayor and six City Council members collectively as one governing body and, thereafter, City Council may adopt by ordinance appropriate amendments to the City Charter to correct usage to this intent. CHARTER AMENDMENT TWO TERM LIMITS Section 13(c) of the Charter shall be amended that no one can serve more than eight consecutive years (two terms) in the singular position of Mayor or Council, or more than sixteen consecutive years (four terms) in any combination of the positions. 9 6/14/2013 4:55 PM 9 CHARTER AMENDMENT THREE REFERENDUM SPENDING; CAMPAIGN SPENDING 2010 CITY OF BONITA SPRINGS PASSED Section 15 of the Charter shall be amended to modify referendum and campaign spending limits by increasing the maximum cap annually based on the Consumer Price Index for All Urban Consumers(CPI-U) or three (3%) percent, whichever is less. CHARTER AMENDMENT FOUR DECREASE RUN-OFFS WITH MULTIPLE CANDIS Section 17 of the Charter shall be amended that instead of 50% plus one voter, the person with the highest number of votes cast for the seat shall be elected. A run-off will beheld in the event of an exact tie on the earliest date that the run-off election can be conducted. CHARTER AMENDMENT FIVE DEPUTY MAYOR / FILLING VACANT COUNCIL SEATS Sections 18 and 21 (C) of the Charter shall be amended to clarify that the Deputy Mayor is elected by City Council and, if a vacancy occurs, the Deputy Mayor serves as Mayor unless the vacancy is more than 180days, in which case the seat is filled by special election for the remainder of the term. CHARTER AMENDMENT SIX VOTING REQUIREMENTS Section 26 of the Charter shall be amended to permit voice votes instead of roll calls on certain votes taken by City Council, and require a majority of the eligible voting Council members present rather than an affirmative vote of four members to be valid and binding. 10 6/14/2013 4:55 PM 10 CHARTER AMENDMENT SEVEN ORDINANCE REQUIREMENTS 2010 CITY OF BONITA SPRINGS FAILED Section 27 of the Charter shall be amended to permit the City Council to adopt resolutions to establish, alter, or abolish any City department office, or agency; or to grant, renew, or extend a franchise, instead of enacting an ordinance. CHARTER AMENDMENT EIGHT ORDINANCE REQUIREMENTS IN GENERAL Section 29 of the Charter shall be amended to eliminate the mandatory thirty day waiting period on every ordinance and publication requirements after passage having an effective date in accordance with state law or ten days after enactment. Nothing precludes City Council from adopting an ordinance with a different effective date, consistent with state law. CHARTER AMENDMENT NINE CITY CLERK AND FINANCE DIRECTOR AS TWO SEPARATE OFFICES Section 37 of the Charter shall be amended to distinguish the functions and responsibilities of the City Clerk and Finance Director as separate City officers. 11 6/14/2013 4:55 PM 11 ORDINANCE NO CITY OF FORT MYERS AMENDING CITY CHARTER TO REQUIRE (citizens initiative) CITIZEN OVERSIGHT PANEL FOR FORT MYERS POLICE DEPARTMENT Ordinance No amends Fort Myers City Charter to require creation by ordinance of an elected citizen oversight panel for independent civilian oversight of Fort Myers Police Department with members elected from each ward and a chairperson elected citywide, with powers limited to reviewing policies, investigating misconduct, making recommendations to city managers and police chief, provided with subpoena powers, professional staff, legal counsel, with annual budget funded by City. EXCHANGE OF LAND OWNERSHIP WITHIN THE ENVIRONMENTALLY SENSITIVE LANDS CONSERVATION DISTRICT 2009 CITY OF SANIBEL PASSED Do you approve the exchange of.6 acres owned by the City (Block 14, Lots 11, 13, 19, 25; Block 13, Lot 6) for.6 acres owned by Sanibel-Captiva Conservation Foundation (Block 17, Lots 1-4; Block 6, Lot 5), with both properties located in Sanibel Gardens Subdivision and both designated Environmentally Sensitive Lands Conservation District, in order to consolidate SCCF property along the Sanibel River and City property along Island Inn Road? REMOVAL OF LAND FROM THE ENVIRONMENTALLY SENSITIVE LANDS CONSERVATION DISTRICT FOR DOG PARK USE Do you approve the removal of approximately 1.42 acres of land from the City's Environmentally Sensitive Lands Conservation District so that a Dog Park, financed by Sanibel Dog Club, Inc., can be located thereon, such land, which will also remain as an emergency burn site, generally described as located within the Sanibel Gardens Subdivision adjacent to Island Inn Road extending from approximately.25 miles to approximately.33 miles west of Tarpon Bay Road? 12 6/14/2013 4:55 PM 12 COUNTY CHARTER 2008 LEE COUNTY AMENDMENT 1 County Charter Revision relating to the Non-Partisan Election of the Supervisor of Elections. County Charter Amendment creating a new Section 3.2, "Non-Partisan Elections"; changing the Office of Supervisor of Elections from partisan to non-partisan; provides if three (3) or more candidates qualify for the Supervisor of Elections, the names of all candidates shall be placed on the Primary Ballot with no party affiliation. If no candidate receives a majority of the vote, the two (2) candidates receiving the highest number of votes will be placed on the General Election ballot. COUNTY CHARTER AMENDMENT LEE COUNTY Revising Article IV., Section 4.1 of the County Charter establishing an independent Charter Review Commission. County Charter Amendment creating an independent Charter Review Commission appointed by the Board of County Commissioners to meet every eight (8) years after the general election of 2008 to propose any Charter amendments. Proposed amendments receiving an affirmative vote of threefifths (3/5) approval of the entire membership of the Commission shall be submitted to the County Commission. Proposed amendments receiving four-fifths (4/5) approval of the entire membership of Commission shall be placed directly onto the next General Election ballot. NON-BINDING REFERENDUM ON THE INCORPORATION OF LEHIGH ACRES Do you favor the unincorporated community of Lehigh Acres being incorporated as a municipality under the Laws of the State of Florida? 2008 Also see 1996 LEHIGH ACRES (voters residing within the boundaries consistent with the Lehigh Acres Fire Control and Rescue District) PASSED PASSED FAILED

13 6/14/2013 4:55 PM 13 CHARTER AMENDMENT ONE RESIDENCY REQUIREMENT 2008 CITY OF BONITA SPRINGS PASSED Section 13 (b) and (d) of the Charter shall be amended to require a residency requirement of six months immediately before holding the office of Council member or Mayor in the District and the City, respectively. NO. 1 CHARTER AMENDMENT FOR FOUR YEAR COUNCIL TERMS, ELECTIONS IN NOVEMBER OF ODD YEARS AND TRANSITION 2008 TOWN OF FORT MYERS BEACH Shall Section 5 of the Town Charter relating to council member elections be amended to provide for four year terms of office and to change council elections to November of odd years, with terms for Council seats 1 and 2 elected in March, 2007 to end in November 2011 and terms for Council seats 3, 4, and 5 elected in November, 2005 to end in November 2009 for transition purposes? NO. 2 CHARTER AMENDMENT FOR ELECTION BY PLURALITY VOTE AND ELIMINATION OF QUALIFICATION FOR SPECIFIC COUNCIL SEAT 2008 TOWN OF FORT MYERS BEACH FAILED PASSED Should the Town Charter be amended to eliminate the requirement of candidates to seek election to a specific council seat and to provide that eligible candidates receiving the greatest number of votes fill the available seats without requiring a majority vote?

14 6/14/2013 4:55 PM 14 CITY CHARTER AMENDMENT ABOLISHING TERM LIMITS FOR THE MAYOR AND CITY COUNCIL MEMBERS 2007 CITY OF CAPE CORAL FAILED This proposal amends the Cape Coral City Charter by deleting language that limits the number of terms for the Mayor and City Council members. If this proposal is adopted, there will no longer be term limits for the Mayor and City Council members. CITY CHARTER AMENDMENT RELATING TO REQUIREMENTS FOR INITIATIVE AND REFERENDUM PETITIONS This proposal amends the Cape Coral City Charter to decrease the number of signatures required for an initiative or referendum petition from fifteen percent (15%) to five percent (5%) of the total number of qualified electors registered to vote at the last regular City election. CITY CHARTER AMENDMENT RELATING TO THE LEASE OF CITY-OWNED RESIDENTIAL PROPERTY This proposal amends the Cape Coral City Charter to provide that leases of City-owned residential property of 15,000 square feet or less for terms of twenty-four months or less shall not require an ordinance CITY OF CAPE CORAL FAILED CITY OF CAPE CORAL PASSED

15 6/14/2013 4:55 PM 15 CITY CHARTER AMENDMENT RELATING TO CITY BUDGET LINE ITEM VETO POWER FOR THE MAYOR AND POWER OF CITY COUNCIL TO OVERRIDE SUCH VETO 2007 CITY OF CAPE CORAL PASSED This proposal amends the Cape Coral City Charter to provide the Mayor with the power to veto line items in the City Budget, subject to the right of the City Council to override such veto with a two-thirds vote of the City Council. The chief purpose of this proposal is to provide the Mayor with a means to exercise control over the expenditure of City funds. CITY CHARTER AMENDMENT RELATING TO THE ANNUAL SALARIES FOR THE MAYOR AND CITY COUNCIL This proposal amends the Cape Coral City Charter to provide that the Mayor shall receive an annual salary that is equivalent to $ per week; Council members shall each receive an annual salary that is equivalent to $ per week; and that salaries of the Mayor and City Council shall be adjusted annually based on the Consumer Price Index. CITY CHARTER AMENDMENTS DELETING CERTAIN OBSOLETE PROVISIONS AND CLARIFYING OTHER PROVISIONS This proposal amends the Cape Coral City Charter by deleting certain obsolete provisions and by adding or amending language to clarify other provisions CITY OF CAPE CORAL FAILED CITY OF CAPE CORAL PASSED

16 6/14/2013 4:55 PM 16 SANIBEL CHARTER AMENDMENT REFERENDUM ARTICLE III, SECTION 3.07(c)(2) "FILLING OF VACANCIES" 2007 CITY OF SANIBEL PASSED Shall Section 3.07(c)(2) of the City Charter (as codified) be amended to require that a special election to fill a Council vacancy be scheduled within 120 to 150 days after the occurrence of the vacancy, rather than the currently required 60 to 90 days, in order to provide the Supervisor of Elections sufficient time to schedule, prepare for and hold such special election? APPROVAL OF GENERAL OBLIGATION BONDS TO FINANCE A PUBLIC SAFETY FACILITY CITY OF CAPE CORAL FAILED Shall the City of Cape Coral be authorized to issue general obligation bonds maturing not later than 20 years from their issuance in a principal amount not exceeding $110,000,000, bearing interest at market rates, to finance the acquisition, construction and equipping of a public safety facility, primarily for police, fire/rescue and emergency management and services, which bonds shall be payable from ad valorem property taxes without limit on all taxable property within the City? ACQUISITION OF FINANCING IN EXCESS OF 36 MONTHS Is the Town Council authorized to obtain financing or extend the existing financing in excess of 36 months in connection with the refinancing of the purchase of the water utility operated by the Town of Fort Myers Beach Public Works Services, Inc. and for the purpose of providing for capital improvements necessary in connection with the operation and maintenance thereof? 2007 TOWN OF FORT MYERS BEACH PASSED

SANIBEL CHARTER AMENDMENT REFERENDUM ARTICLE III, SECTION 3.17 CLARIFICATION OF THE TERM "NEW VEHICULAR TRAVEL LANES" Shall the Sanibel Charter (as codified) be amended to clarify that a "new vehicular travel lane" shall constitute a major change to Highway 867 (Causeway Blvd., Periwinkle Way, Palm Ridge Road, and Sanibel-Captiva Road) when there is an additional vehicular travel lane, and that "new vehicular travel lane" does not mean the redesign and relocation of an existing vehicular travel lane within City right-of-way?
