Circulator is not required to be a resident of this state but otherwise must be qualified to register to vote in this state and, if not a resident of this state, shall register as a circulator with the secretary of state. 3. Circulator s actual residence address or, if no street address, a description of residence location shall be included on the petition. 5 Nomination (Signature) Petition Printing Instructions The Nomination (Signature) Petition must be Double-Sided The second page (instructions for circulator) must be printed on the reverse side of the nomination petition. Instructions for printing the PDF document: NOTE: The following instructions are for illustrative purposes only and reflect how the document would be printed using a duplex printing, capable printer. Individual computer and printer configurations vary and it is ultimately the candidate s or circulator s responsibility to ensure that the petition is printed according to A.R.S ) Click print icon from the Adobe Product, not your browser. A window similar to the one pictured below should appear. 2) Ensure that the page is printing Actual Size. 3) Click the Print on both sides of paper option. 4) Under Orientation select Landscape 5) When you have printed the petition, make sure that it complies with the requirements found in A.R.S (verify that line spacing is one-half of an inch).
6 Three (3) things to check once you have printed the form: 1. The form is printed on both sides (front and back) 2. The form is printed on 8 ½ x 11 paper 3. The spacing of the signature lines is one-half of an inch. Arizona Revised Statutes Form of petitions; registration of circulators A. The nomination petitions shall be in substantially the following form: 1. Petitions shall be on paper eleven inches wide and eight and one-half inches long. 2. Petitions shall be headed by a caption stating the purpose of the petition, followed by the body of the petition stating the intent of the petitioners. 3. There shall be ten lines spaced one-half of an inch apart and consecutively numbered one through ten. 4. The signature portion of the petition shall be divided into columns headed by the following titles: (a) Signature. (b) Printed name. (c) Actual residence address, description of place of residence or Arizona post office box address, city or town. (d) Date of signing. 5. A photograph of the candidate may appear on the nomination petition. B. The following shall appear on the petition: Instructions for Circulators 1. All petitions shall be signed by circulator. 2. Circulator is not required to be a resident of this state but otherwise must be qualified to register to vote in this state and, if not a resident of this state, shall register as a circulator with the secretary of state. 3. Circulator's name shall be typed or printed under the circulator's signature. 4. Circulator's actual residence address or, if no street address, a description of residence location shall be included on the petition. C. The secretary of state shall prepare sample nomination petition forms and distribute such forms to all election officers. D. Circulators who are not residents of this state must be registered as circulators with the secretary of state before circulating petitions. The secretary of state shall provide for a method of receiving service of process for those petition circulators who register pursuant to this subsection. The secretary of state shall establish in the instructions and procedures manual issued pursuant to section a procedure for registering circulators and receiving service of process.
7 GOVERNING BOARD CANDIDATE STATEMENT NOVEMBER 8, 2018 GENERAL ELECTION (A.R.S [F]) The candidate shall submit a statement to the Apache County School Superintendent. The candidate statement shall be typewritten or electronically submitted and shall contain the following items: 1. A recent photograph of the candidate. 2. A statement not to exceed five hundred words. 3. A disclosure of any relationships by affinity, by consanguinity or by law to the third degree that exist between the candidate and any current governing board members or other candidates for election to the same governing board. The statements will be posted to the Apache County School Superintendent s website as described below: 1. The statements will be posted verbatim as they are received unless a candidate requests in writing that typographical errors be corrected. 2. All statements and photographs will be posted on the Apache County School Superintendent s website alphabetically by school district and by candidate s last name. 3. Candidate statements may be submitted beginning July 6, 2018 through September 20, Photographs and statements must be submitted together. Photographs should be no larger than 2 x3. 5. All candidate statements and photographs will be posted by September 26, For candidates that do not submit a statement, the website will state, No Response Submitted. 7.
