1 GOVERNMENT OF NATIONAL CAPITAL TERRITORY OF DELHI BHAGWAN MAHAVIR HOSPITAL H-4/5, GURU HARKISHAN MARG, PITAMPURA DELHI Phone: Fax: LIMITED TENDER DOCUMENT The Medical Superintendent, Bhagwan Mahavir Hospital, Delhi 34 invites sealed quotations for the supply of Defibrillator (01 Nos) as per Annexure I, for use in B M Hospital,Govt. of NCT of Delhi,PitamPura,Delhi-34. Cost of tender: Nil Start Date & time of Issue of Tenders : from 9.00am Last date & time for downloading the tender: upto 01.00pm Last date & time for submitting the tender: upto 2.00pm Date & time of opening of prequalification : at 2.30pm Date & time of opening of technical bid : Shall be intimated later Date & Time of opening of Price Bid : Shall be intimated later EMD : Rs. 10,000/- (Rs. Ten thousand only) Validity of Tender : Single Time Procurement TERMS & CONDITIONS The tender should be typewritten, free from cutting erasing and over typing. All rows & columns on prescribed format should be filled and not left blank, may be struck as not applicable. Each paper of the tender should be serially numbered and duly signed by the bidder with the rubber stamp of the firm on each page. The tender shall be evaluated in 03 parts: (i) (ii) (iii) Pre-qualification Bid : The pre-qualification bids shall be opened first and evaluated. Technical Bid : Technical Bids shall be opened and evaluated by technical committee. Price Bid : The Price Bids of the items / equipments quoted by the bidders that are technically approved by the committee shall be opened thereafter. Each bid to be submitted in separate sealed envelopes superscribed as Prequalification Bid Technical Bid and Price Bid. All the envelopes should be put in another envelope superscribed as Quotation for DEFIBRILLATOR bid and sealed with lakhh (sealing wax). Name of the firm with complete address, ID and contact number should be written on the envelop. The quotation should reach on or before upto 2.00pm in the tender box at room number 133, BM Hospital Pitampura, Delhi

2 Documents to be submitted in Pre qualification Bid : (I) EMD of Rs. 10,000/- in the Shape of FDR valid for a period of 06 months in the name of Medical Superintendent, Bhagwan Mahavir Hospital, GNCTD, Pitampura, Delhi. (II) Undertaking on the Non Judicial Stamp Paper of Rs. 100/- as per format given in Annexure IV. (III) Attested photocopy of latest DVAT registration issued by the Department of Trades & Taxes, Delhi Government along with copy of returns of last one year. (IV) Attested copy of PAN Card issued by Income Tax Department. (V) Self Attested copy of Performance Certificate for dealing in medical equipments issued from any government hospital / Medical Institution to testify the proper dealing and performance duly signed by the competent authority. The certificate must be issued within last 02 years (VI) Self Declaration / Authorization letter to sign the tender document. In case of partnership firm, the authority letter must be signed by all the partners. Documents to be submitted in Technical Bid (I) Technical Bid as per Annexure I (II) Catelogue to be submitted with the bid. (III) In addition to submission of Catalogue, the firm will have to demonstrate the Defibrillator before the technical evaluation committee on the date as informed by Chairperson Committee / Purchase department through telephonic message / . Non presentation of demo may lead to rejection of the item. Documents to be submitted in Price Bid:- (I) Price Bid as per annexure II. L1 will be calculated only on the basis of Annexure II. (II) The rates of AMC &CMC for a period of 03 years (after expiry of warrantee period) must be quoted in Annexure III. The annexure III shall be submitted in the price bid cover which is mandatory. (VII) General Terms and Conditions 1. The rates should be quoted in Indian Rupees in figure as well as in words. 2. Sales Tax/DVAT etc. wherever applicable, should be indicated separately in percentage. 3. No increase in quoted price and change in quality of product will be allowed during the validity of tender. 4. Bidder will quote firm rates. Rates should be according to a unit e.g. cost per piece/unit, not in any other form. 5. The rates quoted should not be higher than that approval in any other govt. hospital / institute for the same item. 6. In case, the rates of any approved item(s) are lower due to any reason, the bidder will have to intimate the same to the authority and will charge the lowered rates with immediate effect. (VIII) DELIVERY OF THE SUPPLIES/STORES: - 1. Delivery of stores shall be F.O.R Bhagwan Mahavir Hospital, Govt. of NCT of Delhi, Pitampura, Delhi 34. Hospital is not liable for payments on account of freight/taxes, which are to be paid inclusively by the suppliers. 2. The firm will be bound to supply the items within 30 days of issued of supply order and install the same within 07 days from the date of supply. 3. The Medical Superintendent, Bhagwan Mahavir Hospital, Delhi reserves the right to extend the period of delivery subject to imposition of a penalty of 0.5% of the value of order per working day maximum upto 10% for delayed supply. Maximum delay of only 30 days is admissible otherwise supply order will be treated as canceled without any further reference.

3 4. The store should be supplied and installed strictly in accordance with the supply order. 5. Supplier must ensure that every challan is to be submitted in the concerned store alongwith the supplies. The number and date of delivery challan must be indicated on the bill. 6. Triplicate bills duly, pre receipted on appropriate revenue stamp affixed be submitted in the name of the Medical Superintendent in respective stores. 7. The bill should be in printed form having printed bill number, VAT/CST/TIN Number as well as D.L. No. wherever applicable. 8. In case it is a computer-generated bill, it must have the seal of the firm affixed on it. GUARANTEE/WARRANTY: - The bidder will have to certify and guarantee that their equipment would continue to conform to the description and quality during the warranty period, which shall be two years from the date of installation for including spares. For the equipment, the firm shall quote for AMC & CMC charges for the next 3 years after expiry of the comprehensive warranty period of 2 years in the Annexure III. In case of non working of equipment during the warrantee period (02 years), cumulative downtime should not be more than 5%. Security Deposit (10% of the total cost of the equipment) is liable to be forfeited if the cumulative downtown of the machine remains more than 5%. PERFORMANCE SECURITY DEPOSIT The bidder would submit a performance security deposit in the shape of FDR for 10% of the total cost (exclusive of taxes) of the equipment for the period of warranty plus 2 months indemnifying the hospital against all losses incurred by the hospital during the warranty/maintenance period. This has to be submitted after satisfactory installation along with the bills. The Security deposit shall be released after expiry of the tenders /or after the completion of comprehensive warranty period subject to successful fulfillment of terms and conditions, on receipt of requisite No dues certificate from the concerned departments/authorities. Security Deposit is liable to be forfeited if the tenderer withdraws or impairs or derogates the bid in any respect. (IX) PAYMENTS: - Payments will be made only after the receipt and installation of the items in good condition as per specification against the supply order in the hospital stores duly approved on satisfactory inspection against the pre-receipted bills in triplicate. (X) DISPUTES AND ARBITRATION: - All disputes or differences arising during the execution of the contract shall be resolved by the mutual discussion failing which the matter will be referred to the Secretary (Law, Justice & Legislative Affairs), Govt. of NCT of Delhi or his nominee for arbitration whose decision shall be blinding on the contracting parties. 7 ANNEXURE IV UNDERTAKING (to be executed on Rs. 100/-Nonjudicial Stamp Paper) Group Name.. Due on. The undersigned certify that I have gone through the terms and conditions mentioned in the tender document and undertake to comply with them. I have no objection for any of the content of the tender document and I undertake not to submit any complaint/ representation against the tender document after submission date and time of the tender. The rates quoted by me/us are valid and binding on me/us for acceptance till the validity of tender. 2. I/We undersigned hereby bind myself/ourselves to Lt. Governor Delhi to supply the various items for BM hospital, Delhi, or on behalf of the President of India during the period under contract. 3. The articles shall be of the best quality and of the kind as per the requirement of the institution. The decision of the Medical Superintendent, Bhagwan Mahavir Hospital, Delhi, (herein after called the said officer) as regard to the quality and kind of article shall be final and binding on me. 4. Bid security deposited by me viz Rs. 10,000/- in the form of FDR in the name of the Medical Superintendent, BM Hospital, Delhi as attached herewith and shall remain in the custody of the Medical Superintendent. 5. Should the said officer deem it necessary to change any article on being found of inferior quality, it shall be replaced by me/us free of cost in time to prevent inconvenience. 6. I/We hereby undertake to supply the items during the validity of tender as per directions given in supply order within stipulated period positively. 7. If I/We fail to supply the stores in stipulated period the hospital has full power to compound or forfeit the Bid Security/EMD/security deposit. 8. I/We declare that no legal/financial irregularities are pending against the proprietor/partner of the tendering firm or manufacturer. 9. I/We undertake that the rates quoted by me when approved and selected by the Medical Superintendent, BM Hospital, Delhi will be valid for single time procurement from the date of finalization of rate contract or till extended as mutually agreed upon. I undertake to supply the equipment/stores within 30 days. I undertake to supply the order within stipulated period and if fail to supply order during the stipulated period the necessary action can be taken by the Medical Superintendent, BM Hospital, Delhi.

8 10. I/We undertake that if the rates of any items are lowered due to any reason, I will charge the lower rates. 11. I/We undertake that the items supplied are as per sample/catalogue/technical literature description. 12. I/We undertake that the quoted rates are not higher than that approved in any other Govt. institutions in Delhi for the same items. 13. I / we do hereby undertake that no CBI inquiry/criminal proceeding/black listing is pending against me / my firm. I undertake that I will not submit any irrelevant documents with the tender and in doing so I will not have any objection if my tender is rejected on that ground. 14. I/we do hereby confirm that the prices/rates quoted are fixed and are at per with the prices quoted by me/us to any other Govt. of India/Govt. of NCT Delhi Hospitals/Medical Institutions. I/we also offer to supply the stores at the prices and rates not exceeding those mentioned in the price bid. 15. I/we do accept/agree for the all clauses including payment terms and conditions of this tender enquiry.
