CHAPTER 11 Jointly Administered Under CASE NO wlh SUPPLEMENTAL AFFIDAVIT OF SERVICE I, Alvaro Salas, Jr., depose and say: 1. I am employed by Kurtzman Carson Consultants LLC ( KCC, the claims, noticing and balloting agent for the Debtors in the above-captioned cases. I submit this affidavit in connection with the service of solicitation materials (the Solicitation Packages for the First Amended Joint Plan of Liquidation [Docket No. 934]. I am over the age of 18 and not a party to this action. Except as otherwise noted, I could and would testify to the following based upon my personal knowledge. 2. On August 6, 2015, the Court entered the Order Granting Application for Authority to Retain Kurtzman Carson Consultants LLC as Claims, Noticing, and Balloting Agent [Docket No. 38]. 3. Consistent with its retention as claims, noticing and balloting agent, KCC is charged with, among other things, the duty of printing and distributing Solicitation Packages to creditors and other interested parties pursuant to the instructions set forth in the Order Granting Joint Motion For an Order Approving Solicitation Procedures and Form of Solicitation Materials [Docket No. 936] (the Solicitation Procedures Order as entered by the Court on June 8, 2018 and the Order and Notice Approving Debtors' Disclosure Statement to Accompany First Amended Joint Plan of Liquidation and Fixing Time for Filing Acceptances or Rejections of Amended Plan, and Notice of Confirmation Hearing, and Notice of the Time Fixed for Filing Objections to Confirmation of the Amended Plan [Docket No. 938] (the Disclosure Statement Order, as entered by the Court on June 8, 2018.

The various Solicitation Packages distributed to creditors and other interested parties by KCC consisted of one or more of the following solicitation materials (as described in more detail in paragraphs 5 through 8 below: a. Class 2 Secured Claims Ballot (the Class 2 Ballot (substantially in the form attached as Exhibit D to the Solicitation Procedures Order; b. Class 3 Unsecured Claims Ballot (the Class 3 Ballot (substantially in the form attached as Exhibit D to the Solicitation Procedures Order; c. Class 4 Convenience Claims Ballot (the Class 4 Ballot (substantially in the form attached as Exhibit D to the Solicitation Procedures Order; d. Solicitation Letter from the Debtors (the Solicitation Letter (substantially in the form attached as Exhibit B to the Solicitation Procedures Order; e. Disclosure Statement to Accompany First Amended Joint Plan of Liquidation (without First Amended Joint Plan of Liquidation [Docket No. 935] (the Disclosure Statement ; f. Disclosure Statement Order; g. Instructions for Obtaining Copy of Plan (the Plan Notice (substantially in the form attached as Exhibit A to the Solicitation Procedures Order; h. Notice of Non-Voting Status (the Notice of Non-Voting Status (substantially in the form attached as Exhibit C to the Solicitation Procedures Order; or i. Pre-addressed, postage pre-paid return envelope (the Return Envelope. 5. On July 11, 2018, at my direction and under my supervision, employees of KCC caused the Class 3 Ballot, Solicitation Letter, Disclosure Statement, Disclosure Statement Order, Plan Notice and Return Envelope to be served via Overnight Mail on the service list attached hereto as Exhibit A.
