ABC Pumping Company Header Form Number This allows you to create a unique identifier for your form number. ABC I Hot/Cold Work Permit Supervisor Request In this section you will provide all pertinent information in order to complete the hot or cold work operation. NA I Hot/Cold Permit Check Off Check off either Hot Work or Cold Work Permit. I Start Date Print the date work is to start. (Note: Permit is only good for a maximum of 24 hours). October 23, 2015 I Start Time Print the start time of the permit. Use military time I Permit Expiration Date & Time Print the date and time that the permit expires, which will be 24 hours after the Start Date & Time. October 24, 2015, 0800 I Ship or Vessel Fill in the vessel name. USS GUNSTON HALL I Trade Fill in the trade associated with the work to be performed. Painter, welder, etc. I Compartment Name Identify the name of the compartment and add it into the form. Supply Berthing, Crews Galley, Void, Fuel Tank, etc. Page 1
2 I Compartment Type Check the appropriate box for the type of compartment. I Compartment # Add the compartment number E I Ships Contract # Input the contract number I Marine Chemist Certificate /Shipyard Competent Person (SCP) Required in Space or Adjacent Space or System? Does the space have posted the Marine Chemist Certificate (MCC) /Shipyard Competent Person (SCP) log as required in Space or Adjacent Space or System? Yes or No I Ship s Item # Input the work item number I Safety/CMC/SCP Instructions / Gas Free Scope & Log Read and comply with the instructions written on the CMC/SCP Gas Free Scope & Log prior to the start of the scope of work. Establish local negative ventilation six inches away from heat source and vent to the weather. I Fire Watch Count Input the number of fire watches required for the work identified. Be sure to have each Fire Watch listed in.c of the form. 3 I Lockout/Tags-plus Required? Identify whether or not Lockout/Tags-plus is required for the work. Yes or No I Adjacent/Affected Spaces, Name & Compartment Numbers Identify and add all adjacent and affected spaces for the hot work including the space names and compartment numbers E, T, T I Scope of Work Print a description of the scope of work to be performed Welding deck foundation and studs on the bulkhead. I Current Gas Free Status Check all appropriate boxes that apply to the space. I Enter with Restrictions List all restrictions for conducting work within the space(s) identified. No welding allowed from frame port side. Page 2
3 I Type of Work Check the appropriate boxes for the type of work to be performed. If the type of work that you are performing is not listed, then you will need to check the box listed as other and detail that work below that check box. I Supervisor s Badge # Print Supervisor s company badge number A. Hot Work Operator (HWO)/Each Fire Watch (FW)/Permit Authorizing Individual (PAI) Shift Condition Verification 1. Compliant fire extinguisher(s) [seal, pin, tag, & charged]. 2. Work areas clear of combustibles (35FT) & clear of flammable liquids (50 FT). In this section the Hot Work Operator, each Fire Watch and the Permit Authorized Individual will verify the condition of the space prior to the start of hot work. a. The HWO shall inspect extinguisher for an intact tamper seal, fully attached to pin, and verify the PMS Tag is current and check the SAT box or N/A if conducting cold work. b. PAI will place their initials that they have verified the condition has not changed in the appropriate shift block. a. The HWO shall verify all combustibles have been removed from the area (35ft/50ft for flammables). Once verified, the HWO shall initial in the appropriate block for the shift or N/A if conducting cold work. b. The PAI will verify that conditions are safe to start hot work and all flammables and combustibles have been removed from the area and place their initials in the block for the appropriate shift. 4 3. The SCP log has been updated daily or as often as necessary to maintain atmospheric conditions. a. The HWO shall read and verify the SCP log has been updated daily or as often as necessary to maintain atmospheric conditions. Upon verification the HWO will initial in the block for the shift. b. The PAI shall read and verify the SCP log has been updated daily or as often as necessary to maintain atmospheric conditions. The PAI will then initial in the block for the appropriate shift. 4. Marine Chemist Certificate (MCC)/Gas Free Log/Gas Free Work Scope specified, read & verified by each HWO & Fire Watch a. The HWO & each Fire Watch shall read and verify all conditions are in place IAW the MCC/Gas Free Log/Gas Free Work Scope. Upon verification the HWO will initial in the block for the shift. b. The PAI shall read and verify all conditions are in place IAW the MCC/Gas Free Log/Gas Free Work Scope. They will then initial the block for the appropriate shift. 5. Qualified Fire Watch assigned (must have proof of qualification.) a. The HWO shall verify that each Fire Watch has on their person a current Fire Watch Certification Card. Once verified, the HWO will initial in the block for the appropriate shift. b. The PAI shall verify that each Fire Watch has on their person a current Fire Watch Certification Card. Once verified, the HWO will initial in the block for the appropriate shift. 6. HWO ensures Fire Watch has immediate access, and established communications. a. The HWO shall establish communications with each Fire Watch. Upon verification the HWO will initial in the block for the appropriate shift. b. The PAI shall verify that communications have been established and the each Watch has immediate access. Upon verification, the PAI will initial in the block for the appropriate shift. 7. Ensure proper working ventilation established in all required areas. a. The HWO shall ensure negative portable ventilation is rigged and established in all areas. b. The PAI will verify that conditions have not change and negative ventilation is rigged as required. 8. Paint removed a minimum of 4 inches from hot work on all affected surfaces. a. The HWO shall ensure paint has been removed a minimum of 4 inches from the hot work on all affected surfaces to include the backside of bulkheads. b. The PAI shall verify paint has been removed a minimum of 4 inches from the hot work on all affected surfaces to include the backside of bulkheads. Page 4
5 9. Lagging removed a minimum of 12 inches from the area of hot work & remaining lagging is covered. a. The HWO shall ensure lagging has been removed a minimum of 12 inches from the area of hot work & all remaining lagging is covered. b. The PAI shall verify lagging has been removed a minimum of 12 inches from the area of hot work & all remaining lagging is covered. 10. No hot work within 50 feet of a dust collector, fuel cell or flammable liquid storage locker. a. The HWO shall ensure no hot work is to take place within 50 feet of a dust collector, fuel cell or flammable liquid storage locker. b. The PAI shall verify no hot work is to take place within 50 feet of a dust collector, fuel cell or flammable liquid storage locker. 11. HWO verifies Lockout/Tagsplus (if required). a. The HWO shall ensure that Lock Out/Tags-plus is in place, as required. b. The PAI shall verify that Lock Out/Tags-plus is in place, as required. B Each FW must verify HWO.A conditions and fill in the rows below. PAI must validate and authorize PRIOR to work start. Fire Watch must print their Shift Number (list as: 1st, 2nd, or 3rd), first and last name, Company Name, Badge Number, time and qualification date. Sign: Place signature of Fire Watch on the same line. Note: PAI must validate and authorize PRIOR to work start C After validating.a conditions, HWO must fill in the rows below. PAI must validate and authorize PRIOR to work start. HWO Print: Shift Number (list as: 1st, 2nd, or 3rd), first and last name, Company Name, Badge Number, time and any comments. Sign: Place signature of HWO on the same line. Note: Fire Watch and PAI validate and authorize PRIOR to work start. D PAI must fill in the rows below to authorize the hot work. PAI Print: Shift Number (list as: 1st, 2nd, or 3rd), first and last name, Badge Number time and any comments. Sign: Place signature of PAI on the same line. I PAI Surveillance The PAI must survey the hot work area at a minimum of one per shift while work is in progress to verify that conditions have not changed. PAI Print: Shift Number (list as: 1st, 2nd, or 3rd), Time, First and Last name, Badge Number, Comments. Sign: Place signature of PAI on the same line. Page 5
6 IV V Hot Work Supervisor (HWS) End of Shift Inspection Ships Force Notification Acknowledgement Hot Work Supervisor signs first and last name, prints date, time and any comments at the end of shift. This signature is verification all hot work is complete, housekeeping has been accomplished and a 30 minutes cool down has taken place. This section is completed by Ship s Force. The ship s Fire Marshall or their representative will sign in this block. This notice is effective for 24 hours unless a shorter period of time is specified. Loss of gas free status in the location(s) above cancels this Hot/Cold Work Permit. An inspection of the area 30 minutes after completion of the hot work is the final action required to close this permit. VI Permit Closure HWO or PAI will sign off on the permit to close it out. They will then remove it and turn it in following local company policies and procedures for records retention. Page 6
