3 PHILADELPHIA LATINO VOTER ID WORKGROUP RESOURCE LIST Below is a list of resources in the Philadelphia area to help Latinos get the documents that they need to get a valid ID for voting in Pennsylvania. For more information refer to the PA Voter ID Coalition's Education Guides which can be found at: This list of resources for Latinos starts with the contact info for the elected officials whose districts have the highest concentration of Hispanics in the city. Elected officials have a constituent service staff with vast experience helping people get official documents. I. ELECTED OFFICIALS IN PHILADELPHIA AREA A. Community based organizations that help people get their Puerto Rican Birth Certificate Congreso de Latinos Unidos Congreso has been helping people get Puerto Rican birth certificates since An appointment is not needed. Individuals must visit Congreso with a valid ID and a $5 money order, unless they are more than 60 years old. Congreso is open Monday through Friday from 8 a.m. to 5 p.m. and is located at: Congreso de Latinos Unidos 216 West Somerset Street Philadelphia, PA, Casa Del Carmen Casa Del Carmen provides assistance with birth certificates on Mondays and Tuesdays from 8 a.m. to 11:30 a.m. A valid ID is required as well as a $5 money order (people aged 60 or older are exempt) and $5 in cash. Reese Street Philadelphia, PA, Puerto Rico Federal Affairs Administration (Phone Inquiries Only - Office in New York City) Reyes Rodriquez at the Puerto Rico Federal Affairs Administration has vast experience and is available by phone or to answer questions about getting a birth certificate from Puerto Rico. Individuals without the valid ID necessary can speak to her for guidance on establishing identity for the purpose of obtaining a birth certificate from Puerto Rico. 5 Office hours 8:00 a.m. to 4:00 p.m. Monday through Friday, excluding State holidays. For walk-in services, please allow four days processing time for standard requests. If you were born in Pennsylvania, you will not need an official copy of your birth certificate. You can fill out form HD01564F, a request for certification of birth record for voter ID purposes only. You will need to take the form to PennDOT to apply for a letter certifying your birth record. When you receive that letter, you will have to return to PennDOT to apply for your ID. III. SOCIAL SECURITY CARD A. Direct Access to the Social Security Administration to replace your Social Security Card You must provide current, unexpired evidence of identity in your legal name to apply for a replacement Social Security Card. MARRIAGE/DIVORCE A. Direct access to Philadelphia's Marriage License Bureau at the Office of the register of Wills to get a marriage certificate from Philadelphia A Marriage Record is a certified or exemplified copy of a Marriage Application or Duplicate Portion of the Marriage License. To obtain a Marriage Record, please send the Philadelphia Marriage License Bureau a letter containing 1) the names that the parties used when they applied for a license, 2) the date of the marriage, 3) a self addressed stamped envelope. In addition, you must send in two checks. The first check for $15.00, made out to The Clerk of Orphan s Court. This is to pay for the research. The second check is to be for either $20.00 if you are ordering a Certified Copy or $40.00 if you are ordering an Exemplified Copy. Marriage License Bureau Room 415 City Hall Philadelphia, PA

How to get a copy of a Divorce Decree Requests for certified copies of divorce decrees in Philadelphia can be made at the Clerk of Family Court at 1113 Chestnut St or in Room 269 in City Hall The fee is $41.58 and all requests from 1972 to the present can be completed within 5 minutes. Older requests will take about a week to be filled. For other counties in PA you need to contact the courthouse in the county where the marriage license was issued or divorce decree was granted. For a list of PA County Courthouses visit: certificates/14126 V. NATURALIZATION DOCUMENTS AND PASSPORTS A. Replacing Lost Naturalization Documents To replace a lost certificate of citizenship or certificate of naturalization, you must file Form N- 565 with the US Citizenship and Immigration Services. Call or visit: The form is free, but filing a completed form costs $345. It can take 3-6 months, or even longer if you became a US citizen many years ago. Applying for a US Passport To apply for a US passport for the first time, you must fill out Form DS-11 and apply in person at an official Passport Agency or Acceptance Facility. 7 C. Renewing a US Passport To renew an expired US passport, you must file Form DS-82 with the US Department of State. You can apply by mail or in person. The cost is $110. D. Replacing a lost or stolen US passport To replace a lost or stolen US passport, you must file Form DS-11 and Form DS-64 with the US Department of State. You can apply by mail or in person. The cost is $110. To access the US Passport related forms mentioned above visit: VI. PENNSYLVANIA STATE VOTER ID A. Department of State ID card through PennDOT for voters who cannot get another form of ID that can be used for voting. Voters who lack the verification needed to obtain a secure Pennsylvania Photo ID can visit a PennDOT Driver License Center and obtain a new, Department of State voter identification card for free. Persons who want the new Department of State Voter ID need to provide their Name, Address, Social Security number, Proof of residency in County, and previous name and/or address if the applicant's name changed in the past 12 months. The applicant must fill out a Department of State application form AND sign an oath/affirmation that they are a registered voter AND have no other form of identification that can be used for voting. While the person waits, staff at the Driver License Center will contact the Department of State to validate that the person is registered to vote. Once verified, the person will receive the free Voter ID, which will be good for 10 years. For more information and for copies of the application form and the oath/affirmation form visit VII.
