3 TERMS OF REFERENCE FOR THE CONSULTING FIRMS FOR HIV RISK AND VULNERABILITY ASSESSMENT OF PRISONERS IN NEPAL 1. Background Save the Children, Principal Recipient (PR) of Global Fund grants for HIV, TB and Malaria in Nepal is working in partnership with Ministry of Health and its respective entities in combating three diseases HIV, TB and Malaria. Save the Children is currently working in partnership with the National Centre for AIDS and STD Control (NCASC), the National Tuberculosis Centre (NTC) and the Epidemiology and Disease Control Division (EDCD), of the Ministry of Health, in implementing the priorities of the National Strategies Plans of HIV, TB and Malaria respectively and to ensure that these strategic plan are aligned with the The Sustainable Development Goals, which include with a bold commitment to end these epidemics by The reports of department of prison management of Nepal mentions that total prison population of Nepal (including pre-trial detainees and remand prisoners) as of sept 2017 is 18,550 (Male and Female-1292). The total number of establishments (institutions/prisons) in Nepal until 2014 is 74. The official capacity of prison system in Nepal is 10,608 (June 2017). The prisons in Nepal are overpopulated and the occupancy level as of June 2017 is 178%1. Nepal has recently, successfully submitted HIV, TB and Malaria funding applications to the Global Fund for the period of March 2018 to March 2021 and is currently in the process of grant making. The application submitted to Global Fund through the CCM Nepal has some resources allocated for migrants through the TB and Malaria grants but there is no resource allocated in the HIV grant. Due to unavailability of resources the minimum resources allocated in TB and Malaria will also not be sufficient to meet the goals envisioned by National Strategic Plans. To explore the possible options for a coordinated effort to tackle the problem of migrants across the three grants, the CCM in Nepal with the support of the PR, is planning to do an assessment of the current situation of migrants and services gap with relation to HIV, TB and Malaria services. 1 Department of Prison Management (
4 Similarly, the CCM in Nepal is also planning to do a situation assessment of prison and HIV related interventions for and among prisoners in Nepal. Save the Children will be recruiting a consulting firm for the completion of this task. 2. Objectives The overall objectives of this assessment are to:. Assess the current situation of prisons and risk and vulnerability assessment in regard to HIV and TB in prisons of Nepal. Examination of the possible entry points and structural elements that will support the implementation of an impactful HIV programme in prison settings. Explore possibility of integration of TB and Malaria services with HIV in prison settings. 3. Deliverables and Timeline Deliverables A final report with the current situation of prisons in Nepal, gap in the HIV relates services for prison inmates and recommendation for providing services in prisons of Nepal. Trip reports in case of any trips made outside the Kathmandu valley. The level of effort for both assessments is 45 working days within January to March 15th, Methodology This assessment will be using both quantitative and qualitative methods for data collection and analysis. However, major tasks are detailed out below: Conduct desk review of national and international documents, strategies and SoPs on Prison Management, HIV related interventions in prison settings; Operational guidelines for Prisons, review the achievements and constraints for program interventions in Prisons as per the program reports available at the country level as of now. Conduct key informant interview with key stakeholders (Director- Department of Prison Management, Director-National Centre for AIDS and STD control, office incharge of prisons, health service providers in Prisons, district public health officer (DHO/DPHO), CCM coordinator and Civil society
5 representatives) and explore the existing bottlenecks and opportunities for providing HIV related programs in prisons; Visit the central jail and at least one prison in each of the provinces and conduct key informant interview and FGD with beneficiaries and get their opinion and suggestion for HIV program interventions in prisons of Nepal; Conduct key informant interview with stakeholders implementing OST program and NSEP program for PWID and get their opinion on need of providing Harm Reduction program in prisons of Nepal; Conduct key informant interview with key stakeholders implementing HIV program for different key population (FSW, MSM, TG, MSW and Migrants) and get their opinion on need of providing HIV relates services in prisons of Nepal; Analyze the current trend of incarceration among drug users and other key populations of Nepal; Explore the availability and use of services in prison by the prison inmates from the key population; Analyze the barriers of accessing different services in prisons and suggest approaches to overcome those barriers; Analyze the existing service modalities and recommend for integrated services models for prison inmates; Share the findings and recommendations of the assessment to the NCASC, department of prison management, CCM and other stakeholders for their comments and finalize the report by incorporating their comments. 5. The remaining key populations (except FSW) will be covered by the Global Fund. Likewise, as stated in the Sustainable Development Goals, the National TB Strategic Plan envision a TB free Nepal and has set up a goal of reducing the morbidity; mortality and transmission of tuberculosis until it is no longer a public health problem. The migrant population remains one of the key populations for TB program to achieve its ambitious goal of eliminating TB. Due to mobility, migrants are at higher risk of acquiring and transmitting TB and also it is difficult to retain those who are on TB treatment.
8 The Malaria program is committed for malaria elimination by The migrant population is also a priority population for achieving the Malaria elimination targets. Nepal shares an open border with India and most of the Indian bordering districts have high Malaria prevalence. It will be difficult for Nepal to eliminate Malaria in the stipulated time, until and unless the program screens and targets the migrants that freely move between the porous borders of India and Nepal. Migrants are a priority population across all three diseases. The destination cities of Nepali migrants in India are mostly those cities with higher risk of HIV, TB and Malaria. Associated factors like poverty, longer working hours with minimum rest, poor access to health-care services, multiple people sharing a room in poorly ventilated areas, poor sanitation, and a lack of balanced diets make migrants more vulnerable to tuberculosis. Being away from their spouses for a long period of time keeps them at risk of HIV as they are likely to practice unsafe sexual behaviors. Similarly, living in areas with poor sanitation in hot weathers of Mumbai, Delhi and Punjab keeps migrants at higher risk of Malaria as well. Nepal has recently, successfully submitted HIV, TB and Malaria funding applications to the Global Fund for the period of March 2018 to March 2021 and is currently in the process of grant making. Deliverables and Timeline Deliverables Inception report including work plan of the study including timeline, milestone, limitations and challenges; Final versions of the assessment tools; Final and cleaned data sets; Summary reports, final PowerPoint slides and all the meeting notes; Final assessment reports outlining the findings of the assessment with the current situation of migrants, available services and gaps for the migrants with regards to HIV, TB and Malaria services; Trip reports in case of travel outside Kathmandu valley. The level of effort for both assessments is 45 working days within January to Mar 15st, Methodology This assessment will be using both quantitative and qualitative methods for data collection and analysis. However, major tasks are detailed out below: Conduct desk review of currently available national and international documents, strategies, SoPs and guidelines on HIV, TB and Malaria and review what these documents recommend about providing services for migrants; so that the recommendations could later feed into the service package for migrants during programming. Explore and access currently available services for migrants in relation to HIV, TB and Malaria; find out the need and gaps for providing HIV, TB and Malaria related services to migrants and provide recommendations for providing integrated HIV, TB and Malaria services to migrants; Prepare a list for Key Informant and prepare data collection tools for the assessment in consultation and collaboration with all three disease stakeholders. Conduct key informant interview with key stakeholders (Program Managers of each disease entities, technical specialists, district public health officer (DHO/DPHO), CCM coordinator and Civil society representatives) and explore
10 the existing bottlenecks and opportunities for providing integrated services for migrants in relation to HIV, TB and Malaria; Visit at least two districts bordering with India, conduct one on one interview with some randomly selected migrants in the transit??and get their opinion and suggestion for integrated HIV, TB and Malaria services during the transit; Conduct key informant interview with stakeholders implementing HIV, TB and Malaria services in the district and get their opinion on need of providing integrated HIV, TB and Malaria services for migrants; Analyse the current trend of migration and destination cities of migrants in India (through secondary data review); Explore the availability and use of services by migrants in their destination country (through secondary data review); Analyze the barriers for accessing different kinds of services by migrants in relation to HIV, TB and Malaria in source, transit and destinations (through secondary data review) Analyze the existing service modalities for HIV, TB and Malaria and recommend for integrated service models for migrants; The recommendations should include a proposed definition of a standard package and a delivery model(s) for HIV/TB and malaria integrated individual-centred services for migrants and their families. This will then feed into the follow up dialogue to agree on a standard package and service delivery Share the findings and recommendations of the assessment to the NCASC, NTC, EDCD, CCM and other stakeholders for their comments and finalize the report by incorporating their comments. 4. 12 TERMS OF REFERENCE Study on susceptibility status of malaria vectors to insecticides currently in use for vector control 1. Background Malaria remains a public health priority in Nepal, where approximately % (In high risk 0.60%; Moderate risk- 4.83% and Low risk 85.30%) of the population is at risk of malaria. Total confirmed malaria cases declined by approximately 91% over the last decade (12,750 in 2002 vs 1128 in 2016), with no reports of malaria deaths since It is to be noted that a declining trend throughout the last five years in confirmed malaria cases is matched by a corresponding decline in number of clinical malaria cases. Nepal has achieved and exceeded the Millennium Development Goals (MDGs) and the 2010 Roll Back Malaria (RBM) malaria targets, and achieved universal coverage of malaria control interventions. Significant progress has been made in controlling malaria transmission due to a change from sulfadoxine-pyrimethamine (SP) to Artemether/lumefantrine (AL) therapy, indoor residual spraying (IRS) in high-endemic foci, distribution of long-lasting insecticide-treated nets (LLIN) in high-endemic areas, and enabling factors such as strategic partnerships, socioeconomic development and free health service delivery from government health institutions. Vector control is the major component of the strategy for malaria elimination which aims to prevent parasite transmission through interventions targeting adult malaria vectors. Successful eradication of malaria in different parts of the world was achieved mainly by chemical insecticides for vector mosquito control. Generally, two types of vector control methods, namely indoor residual spraying (IRS) and long-lasting insecticide-treated nets (LLINs) are used for controlling malaria transmission. The World Health Organization currently approves twelve insecticides in four different classes for use in IRS programs. Because of its human safety and insecticidal properties, pyrethroids are the only insecticide class used for treated nets. Pyrethroids are also now the cheapest, safest and, in the absence of resistance, the most effective insecticides used for IRS. As a result of this, vector resistances to these insecticides have been recorded globally. In Nepal, insecticide for IRS is used among the four available classes of WHOPES-approved insecticides (i.e. organochlorine (DDT); organophosphates (primiphos-methyl); carbamates (bendiocarb); and pyrethroids (alphacypermethrin, deltamethrin, and lambdacyhalthrin). Since 1990, DDT is no longer used in our country the only insecticides that have been used are the pyrethroids. Likewise, WHOPES approved Pyrethroids impregnated bed net are currently in used.
13 Overuse of insecticides, whether in LLIN, indoor residual spraying, or in agricultural use, can select for resistance in mosquito vector species. Recently, there are only a few reports on the susceptibility status of the mosquitoes against various insecticides and thus more emphasis on these studies should be given. Again the appropriate use of the insecticides like rationale use with rotation of insecticides and insecticide combinations can be an effective strategy to combat this insecticide resistance. Therefore, WHOPES approved LLINs/IRS recommended for vector control purposes, their performance should be monitored in the field under various ecological settings to both investigate the insecticide susceptibility status of the malaria vectors, as well as to assess the potency of an insecticide deposits to adult mosquitoes and to evaluate the condition of LLINs distributed in the malaria endemic districts (High and moderate risks areas) of the country, in order to determine their potential efficacy after various intervals of typical use in the community. 2. Objectives The objective of this consultancy is 1. To collect information on susceptibility status of malaria vectors on insecticides (alphacypermethrin, deltamethrin, and lambdacyhalthrin) currently in use / to be used in the future (Bendiocarb) by conducting susceptibility test in different geoecological areas (Plain cultivated Terai, Inner Terai, Forest, forest fringe, Hill and Hill upper river valley) representing all provinces of the country 2. To provide appropriate recommendation based on findings of the study. 3. Deliverables and Timeline The consultant (Entomologist and his/her team) will work collaboratively with EDCD /DCS and EDCD/SCI/PMU to conduct the study and present a report of their findings including a manuscript within 35 days of signing the contract. The consultant will deliver the following documents to DoHS/EDCD and Save the Children: Preliminary report (progress report) of each site Test Record forms of all sites A detailed analysis sheet of all sites A final report of the study
14 4. Methodology The consultant (Entomologist) will: 1. In first 2 days develop SOP, forms and format, prepare detailed work plan and collect reagents and materials required for study and orient team members about the study. Susceptibility test kit and insecticide impregnated papers will be provided by SCI 2. Carrying out of the field visit for 30 days and conduct susceptibility test as per WHO protocol in different sites (Different geo-ecological areas representing all provinces) and number of days spend in each sites will depend upon the availability of vector species but not more than 5 days excluding movement days. 3. After completion of study in a site a preliminary report (progress report) will be submitted to EDCD/SCI 4. In last 3 days collected data will be compile, analyze and finally a final report with recommendations will be prepared and submitted 5. The report writing will entirely be a desk work. 5. Consultant Specification A consultant is desired with at least a Master's degree (In Science preferably in Entomology or Ph. D in Entomology) with prior experience in conducting research including stringent data analytical skills and writing reports and manuscripts in English language. The consultant will be selected through a competitive and transparent process as per the rules and regulations of SCI. Relevant experience criteria shall include: Background in medical entomology At least 10 years of experience in entomological investigations including susceptibility tests, Bioassay tests, fauna (mosquito/sand fly) studies etc. Strong ability to analyse quantitative data and skills to portray/display analysed information skilful Excellent report writing skills in English language and prior experience in making national level reports Has time to complete the task within stipulated time. Has good in computer typing and text formatting And study team members should have
