Henley Affinity Consulting Group, LLC Columbus, Ohio Mr. Henley received both his B.S. and his B.A. from The Ohio State University and his J.D. from The Ohio State University Michael E. Moritz College of Law. His professional memberships include the American Bar Association (Law Practice Management Section), Ohio State Bar Association (Law Office Automation and Technology Committee), Columbus Bar Association (Law 2010 Committee), and Worthington Estate Planning Council. Mr. Henley is an attorney, a legal technologist since 1990, and a founding member of HMU Consulting, Inc. Skeeles Ohio State Bar Association Columbus, Ohio Ms. Skeeles received her undergraduate degree from The Ohio State University and her J.D. from Capital University Law School. She is a member of the Ohio State Bar Association and the Ohio Bar. Ms. Skeeles joined the Ohio State Bar Association in 2007 in the position of Member Services Consultant. As part of her responsibilities at the OSBA, she trains the Ohio legal community on Casemaker, increases awareness and enrollment in OSBA CLE programs, markets the benefits of the OSBA to members, and sells advertising in OSBA publications. Prior to joining the OSBA, Ms. Skeeles worked in the construction industry for over 15 years in sales and marketing and served as corporate counsel for two construction companies. Outside of work, she is a member of the United States Equestrian Federation and Arabian Horse Association. Ms. Skeeles has shown Arabian and Half-Arabian horses at the Regional and National level for over 20 years. OhioDocs TM is a new service from the OSBA that has harnessed this technology by creating an Ohio-specific library of thousands of legal documents that have been automated using HotDocs TM (which is included with the set). Practice areas include domestic relations (child support worksheets, uniform domestic relations forms, pleadings and county forms), probate (estate administration, wrongful death, guardianship, adoption, conservatorship, name change, minor settlement and protective services), real estate (deeds), corporate (all Secretary of State forms including business entity, mergers, consolidations, conversions, name registrations, trademark/service mark, business & real estate trusts and UCC). Also included are the Ohio Advance Directives forms and many administrative forms including fee agreements, engagement letters, confidentiality agreements, and many others. The OhioDocs program launched as a subscription-based program in February 2011 with Phase 1 a set of documents that were selected by an advisory committee of OSBA members. Over the coming months, the OSBA will continue working with its committees and sections to expand the OhioDocs library with both new and updated documents. If you are a subscriber, then you will get the additional forms and documents added to your set at no additional charge. It s also important to note that you must be a member of the OSBA to subscribe. How Document Assembly Works: Document assembly technology overcomes the problems inherent with standard word processing, such as slowness and formatting problems. Programs such as HotDocs TM the industry standard in document assembly allow you to get a customized document with all the blanks filled in, all the pronouns and verbs grammatically in place, all the paragraphs properly placed and numbered everything complete and correct, in a fraction of the time it would take you using a word processor alone. * This link will take you to OhioDocs 1.1

6 OhioDocs templates ask on-screen questions, then automatically create customized documents based on the answers given. This means that you are able to create consistent, accurate documents more quickly than ever before. You can now spend more time serving your clients specific needs and less time researching and drafting documents from scratch. That leads your practice to ultimately be more productive and more profitable. OhioDocs Costs: Your annual subscription fee is based on number of attorneys in the firm and allows you to access the entire library of automated legal documents. The library comes bundled with HotDocs Player which allows you to use all the templates without having to pay for a full version of HotDocs. As a subscriber, you are also entitled to any and all updates and new form releases. Subscription rates start at $195 for a solo attorney. More details on the pricing can be found below. OhioDocs renews annually on April 1. As such, subscriptions are prorated quarterly (depending upon when you sign up): April 1 to June 31: full price July 1 to Sept. 30: 25 percent off Oct. 1 to Dec. 31: 50 percent off Jan. 1 to March 31: 75 percent off As mentioned previously, subscriptions must be renewed annually prior to April 1, and subscribers will receive courtesy notices to renew beginning 15 days in advance of the termination date. Subscriptions that have not been renewed by April 1 will be terminated and the document library will cease to work.
