Late registration fees are an additional $100 per registrant. **New Initiation/Tom Thumb players registered to Calmar Minor Hockey will be given a discounted registration price of $145.00

Please register your child\children in their respective division. If you are unsure about participating in the upcoming season, register early to avoid the LATE REGISTRATION FEE of $100.00. A full refund, without penalty, will be returned to a member if registration is cancelled by October 1, 2016. After October 1, $65.00 is non refundable, the remaining refund will be pro rated from start of season. No refunds will be allowed after December 31st. After May 31, 2016 it will be a first come, first serve basis, providing all registrations fees and requirements are met. NO spots will be held for players due to late registration. NO player(s) shall be released from CMHA until all spots on a team are full. You must register first with CMHA before trying out for any team other than one within Calmar Minor Hockey. *Garment Bags are mandatory from Novice to Midget; the cost is $20 each. Please select on your online registration if you have not previously purchased a CMHA garment bag. You will not be given your game jerseys for the season without providing the garment bag. Once you have completed your online registration, please mail or bring to the Annual General Meeting (Monday, May 9, 2016 at the Calmar Curling Rink 630pm), the following items to complete your registration: Ø Hockey Fees and garment bag payment (Garment bags for every player Novice through Midget) Ø 3 Undated Fundraising Cheques of $100.00 each per family Ø 2 Undated Jersey Deposit Cheques of $100.00 each for every player Novice through Midget Note if jerseys are not returned by March 31, 2017 the jersey deposit cheques will be cashed. Hockey fees, if paying by credit card online can be paid 50% at registration and 50% on September 15, or you can select to pay the full amount now at registration, which will appear at the very end of the credit process. If you are selecting the cheque option you may drop off at the AGM or make arrangements with the treasurer if you would like to pay all, 2 payments or monthly payments deposited from registration to September 15. Please note all fees and commitment cheques (both fundraising and jersey deposits) must be in by September 15, 2016.

PLAYER MOVEMENT: ANY MEMBER THAT HAS NOT REGISTERED WITH CMHA BEFORE AUGUST 1, 2016 WILL NOT GET ANY HOCKEY ALBERTA PERMISSION FORMS SIGNED FROM THAT POINT FORWARD. OVER-AGED PLAYERS: Please contact the registrar of your intentions for an over-aged player. Additional forms will need to be submitted to 1660 and Hockey Alberta for approval. A letter from your previous coach will be required or if for health reasons, a letter from your physician. Previous season s hockey stats will also be required. Please submit this request by September 1, 2016 as this process can take up to November 15, 2016 for approval. CMHA FUNDRAISING REQUIREMENTS: Each family is required to participate to the best of their ability in 3 Association fundraisers (family also includes blended). The board will determine these fundraisers before the season gets underway. If you cannot participate in the fundraisers one of 3 $100 cheques will be cashed per fundraiser after board discussion. Included in the 3 fundraisers is your team tournament. In this case your shifts worked is your contribution (shifts include; raffle table, 50/50, time box). These shifts are based on a per child basis on each team. If you are unable to attend then it is your responsibility to inform the team manager and have someone there to fill your shifts. As for Team fundraising it has been decided that it is based on a per child basis on each team. If this is an issue for your family please advise the board. With out the support of the community and members we cannot continue to offer our athletes the best experience they deserve, we appreciate each family for their past and future support of CMHA.

**Coaches Applications will be available at the AGM as well as on our website. All coach applications must be handed in by August 15, 2016 with COMPLETED criminal record check c\w vulnerability sector check that can be acquired at your local police station. Please contact our registrar for your request letter. ELECTIONS: Nominations will be accepted for ALL positions for the Board of Directors. Elections will take place during the AGM. Please realize that these positions require dedication and that all elected shall attend the monthly meetings. President, Vice President, Secretary, Treasurer, Registrar for Player and Coaches, Fundraising and Licensing Coordinator, Ice Coordinator, Equipment and Apparel Coordinator, Referee Coordinator, and Web and Communications Coordinator are all positions on the Calmar Minor Hockey Board.

DUTIES OF THE EXECUTIVE: President Ø The President shall preside at all meetings of the Association. Ø Provide leadership to the Association that will result in the achievement to the objectives of the Association. Ø Exercise general supervision over the business of the Association. Ø Delegate tasks and responsibilities to the Board Members as the need arises. Ø Provide for the orientation of new Board members at the first meeting after elections. Ø Initiate appropriate workshops or other in-service training for Board Members and general membership that will improve the functioning of the Association. Ø Represent the Association in the community and with other communities or organizations. Ø Attend all HA zone meetings or ensure one member of association is in attendance. Ø Shall be an ex office member of all committees. Vice President Ø The Vice-President shall act in accordance with the duties of the President in the absence of the President. Ø Act as CMHA representative within league, attend all league meetings, deliver league communications to CMHA board and membership Ø Sit on the discipline committee as required from time to time. Ø Perform such duties as directed by the President of the Board. Ø Chair committees to review the rules and regulations of the Association and recommend changes as required to the Annual General Meeting or Special Board Meeting for such purpose. Immediate Past President Ø Review the bylaws and recommend changes as required at the Annual General Meeting. Ø Act as Presidents advisor on request.

Secretary Ø The Secretary shall keep minutes of all meetings of the Executive, Board and a list of the general membership of the Association. Ø Send notices of meeting as required by the bylaws. Ø Receive and respond to correspondence as required. Ø Secure meeting facilities for board meetings and other special events Ø Shall have custody of the minutes, records and files pertaining to the Association. Treasurer Ø The Treasurer shall maintain all financial records as required by the Association. Ø Present the records for audit and present the audited financial statements upon completion to the board. Ø Receive and deposit all funds of the Association in the bank designated by the Board. Ø Submit bills for approval of payment to the Board as required. Ø Prepare cheques in payment of accounts for signature and disbursement. Ø Perform such other duties as directed by the Board. Ø If the Treasurer resigns before the term of office expires, the financial books shall be audited prior to take over by the new Treasurer. Registrar Ø Maintain and administer registration process via board approved methods (website) Ø Collect required documents for coaches application process Ø Complete Hockey Canada & Hockey Alberta registration requirements within HA timelines Ø Ensure proper documentation is completed for inbound and outbound player movement as required by HA. Ø Attend annual registrars meeting as provided by HA for updates and communication of information back to CMHA.

Fundraising and Licensing Coordinator Ø Plan and coordinate fundraising efforts of the association, including casino s, oilers 50/50 and other events as deemed appropriate by CMHA board Ø Complete all ALGC applications for approved fundraising activities of CMHA & teams Ø Reports results as required to AGLC for all fundraising Ø Act as liaison for tournament supplies as required form time to time Ice Coordinator Ø Schedule and maintain ice requirements for CMHA. Ø Liaise with town of Calmar recreation coordinator as required to secure CMHA ice requirements, resolve deficiencies or issues with the arena, staff, equipment etc. Ø Work with team managers to secure required ice and schedule changes as required from time to time. Ø Work with all parties to ensure CMHA ice utilization is maximized Equipment and Apparel Coordinator Ø Maintain & inventory all CMHA equipment (jersey s, goalie equipment, pucks and bags, first aid supplies, nets, etc) Ø Determine CMHA requirement and timing for replacement of aged equipment, including sourcing supplies and pricing Ø Work with team management for supply and collection of team equipment each season. Ø Coordinate and administer CMHA approved apparel needs with teams and suppliers.

Referee Coordinator Ø Coordinate and schedule applicable referees for all CMHA games (exhibition, league and provincial) Ø Work with Referee coordinators form various associations to obtain required officials as needed. Ø Report monthly to the treasurer a list of officials used with in the month (Names, address, game date, level, system used) Ø Be listed with North Zone Referee council as CMHA representative. Deliver communications as required from time to time to CMHA and officials. Ø Work with CMHA board to determine recruitment & training requirements to foster local officials and growth of local officials Web and Communications Coordinator Ø This position will involve updating, managing and designing CMHA s website and face book page. Therefore the volunteer will need to be able to commit to attending monthly board meetings, and any other training that may be required. Ø Work closely with the registrar to ensure that the RAMP program is linked properly with the website. Ø Includes uploading game schedules for all teams, black out dates, tournament information etc. Ø Duties will also include close communication with team managers to assist them in contacting the teams and members. Ø Relationship between web/communication director and other executive members of the association will be crucial. Our goal is to utilize the website as a main communication tool.
