How to use this checklist 1. Before you sign your visa application form, use this checklist to ensure that you have included all necessary documentation to support your claims. 2. When you are satisfied that you have correctly completed your visa application form and all of the necessary documentation is included, you should sign and date your visa application form. 3. Include this completed checklist, when you lodge your visa application form and all necessary documentation. Do not provide original documents unless requested. You should provide certified copies of original documentation. Documents not in English must be accompanied by accredited English translations. Complete this checklist and attach all documentation in the order provided below. The checklist should be placed to the front of your application. Necessary Visa Application Requirements A completed and signed Form 47SP - Application for migration to Australia by a partner IMPORTANT: Please provide a complete detailed residential address in 47SP. The form must be signed in all the relevant places and each person included in the form, who is aged 18 years or over and migrating with you, has signed the Australian values statement. A completed and signed Form 40SP - Sponsorship for a partner to migrate to Australia This form should be completed and signed by your sponsor. A completed and signed Form 54 Family composition A completed and signed Form 80 - Personal particulars for character assessment by applicant and all migrating dependents 16 years or older. Applicants must list full details of all addresses that they have lived at for 12 months or more during the last ten (10) years, whether in Australia or overseas, and provide full details of all employment and / or education since leaving school. A completed and signed Form 47A - Details of child or other dependent family member aged 18 years or over whether or not they are migrating with you Payment of the Australian Government Visa Application Charge. Advice on how to pay this charge can be found at www.vfsglobal.com/australia/cambodia/ OR if the Visa Application Charge has been paid at an office of the Department of Immigration and Citizenship in Australia, a copy of the receipt must be submitted. Two recent passport-size photographs (45mm x 35mm) of the sponsor. (Please ensure that photographs are not digitally enhanced.) These should be of the head and shoulders only, and should show the person facing the camera and against a plain background. Whichever is applicable, a completed and signed: Form 956 - Advice by a migration agent/exempt person of providing immigration assistance or Form 956A - Appointment or withdrawal of Authorized Recipient Evidence that your sponsor is aged 18 years or older and is an Australian citizen, Australian permanent resident or eligible New Zealand citizen, such as: Certified copy of birth certificate. Australian passport or foreign passport containing evidence of permanent residence Australian citizenship certificate. Evidence that your sponsor usually resides in Australia. For New Zealand citizens; evidence of length of residence in Australia and of continuing links with Australia. See: Sponsor Eligibility If there is a child under 18 years of age included in the application, the sponsor must provide the following: An original AFP National Police Check, if the sponsor has lived in for more than 12 months or more (cumulatively and continuously) in Australia since turning 16 years of age. Original police certificates from each country in which the sponsor an applicant, or anyone includes in the application has lived in for more than 12 months or more (cumulatively and continuously) in the last 10 years since turning 16 years of age. You will be issued with a National Police Check certificate which will have complete disclosure in the heading if your AFP police check application is successful. All members of your immediate family, including dependent family members who do not intend to migrate, must meet the character requirement. All applicants and non-migration dependents over the age of 16 must provide a police clearance certificate from each country where they have lived for a total of 12 months or more in the last 10 years, these 12 months are calculated cumulatively, not consecutively. If you live in Phnom Penh, you must provide a police certificate from the Ministry of Justice. If you live outside of Phnom Penh, you must provide a police certificate from the chief of Police of the local police authority. Police clearances are required for all applicants aged 16 years of age or more. Further information on character requirements are available http://www.border.gov.au/trav/visa/char Certified copies of valid passports or travel documents including any amendment pages of all people included in the application covering the entire period from when the applicant and sponsor first met until now. This should also include the bio pages and expiry pages. If you have not yet obtained a passport, the Department will request it at a later date if required. Documents to prove your identity a certified copy of your birth registration showing both parents names. If you do not have a birth certificate and are unable to get one, you must provide a certified copy of the identification pages the following documents: passport family book showing both parents names identification document issued by the government document issued by a court that verifies your identity. If you are unable to provide one of these documents, you must provide other acceptable evidence that you are who you claim to be. If a child under 18 years is coming to Australia without one or both parents or legal guardians, a consent form signed by non-accompanying parent or legal guardian. Evidence must be provided to confirm family relationship. A birth certificate of the child should be provided. Bio data pages of the passport ( or if available, Cambodian ID card ) of any parent or guardian not accompanying the child should be provided. Evidence of child s enrolment in school and/or a copy of parent s marriage certificate ( if applicable ) Contact details including full name, date of birth, address and phone numbers should be provided of the non-accompanying parent or guardian. Evidence that there is no impediment to your intended marriage to your fiancé i.e. You are both free to marry (If either you and/or your fiancé have been previously married or a previous spouse has died, a certified copy of the divorce decree absolute or the death certificate of the deceased spouse).if your ex-spouse has passed away, please provide a selection of paragraphs of the person s funeral and 7 day and 100 day death ceremonies if available. If you, your fiancé or any other migrating dependents included in your application has changed his or her name (for example by marriage or deed poll), a certified copy of evidence of the name change. Certified copies of documents (evidence of dependency) to verify custody and access arrangements, for children under 18 years of age unless it is the child of both you and your fiancé. See: Evidence of dependency: If any migrating child included in your application is adopted, a certified copy of the adoption papers. Evidence that you and your fiancé have met face-to-face as adults, and are personally known to each other. MAY2016 Page 3 of 5

Evidence that you intend to marry your fiancé (for example, a certified copy of the Notice of Intended Marriage (NOIM) OR a letter (on letterhead) from an authorized Marriage Celebrant in Australia confirming the names, date and place of marriage to take place in Australia and that a NOIM in respect of the parties has been lodged with the celebrant See: Information on getting married in Australia. Note: The visa condition is that the applicant and the sponsor need to get married within nine months of being granted the visa Evidence that you and your fiancé genuinely intend to live in a married relationship. See: Evidence of a genuine and continuing relationship: Written statements from you and from your fiancé detailing the history of your relationship (for example when and how you met, when you became engaged, joint activities, significant events in the relationship) and your future plans as husband and wife. Form 888 Statutory declaration by a supporting witness relating to a partner visa application. At least two statutory declarations, preferably from individuals who are Australian citizens or permanent residents and who have personal knowledge of your partner relationship (such as a relative and a friend) and support your claim that the relationship is genuine and continuing. Evidence of your and any migrating dependents current activities regarding work/study (if applicants are studying, evidence from the school or college including the contact telephone number/s). If there are migrating dependents 18 years of age or over, evidence that they are financially dependent on their sponsor for the basic needs of food, shelter and clothing, and how long this support has been provided. Evidence may include bank statement, money transfers, rent receipts etc. Evidence that your relationship in genuine and going. This includes information and evidence that demonstrates commitment to each other, that you have financial ties, your relationship is recognized by friends, family and community and that you have made plans in relation to your house life together.
