2013 Policies CONFERENCE REGISTRATON: The conference registration fee is $150.00 which includes two (2) luncheon tickets and one (1) banquet ticket. Additional luncheon tickets are $35.00 each and additional banquet tickets are $50.00 each. Any additional tickets must be ordered at the time of registration. Conference registration forms are enclosed, the forms should be completed and mailed with checks or money orders payable to: Midwest Regional Conference and forward to The, PO Box 11839, Louisville, Kentucky 40251-0839, Attn: Jonathon Bryant at jabmm1@aol.com, or (502) 553-4665. Deadline March 4, 2013. There will be NO ONSITE REGISTRATION. HOTEL REGISTRATION The Galt House Hotel will serve as the Conference Headquarters located at 104 North Fourth Street, Louisville, Kentucky 40202. The hotel rates are: Double Queen $109.00 per night plus tax, Executive Suite $129.00 plus tax to contact the hotel directly call (502)589-5200 or call toll free 1(800)843-4258. When calling for registration ask for the block listed under Midwest Regional Conference. Parking is $7.00 per night for registered hotel guest. The deadline for hotel registration is February 25 th 2013. Reservation request received after the deadline will be accepted on a space and rate availability basis; if the group rate is not available after the cutoff date, the prevailing rates will apply for any confirmed reservations thereafter. Cancellation Policy Cancellation of full registration at no charge will be accepted until March 4, 2013. Any cancellations after March 4, 2013 will be assessed a fee of $120.00 per registered cancellation.

PHOTOGRAPHY The committee has contracted with a photographer to provide coverage of the entire conference. Time slots will be designated for the tradition group photos as in the past; the photographer will be available on site to make arrangements with members who wishes to purchase individual pictures. SOUVENIR JOURNAL Preparation for the 2013 Annual Midwest Regional Conference Souvenir Journal is in progress. The Planning Committee invites your Jurisdiction to participate in the journal by purchasing advertising space or a Patron Ad. Dear Vendor: The, and Prince Hall Grand Chapter, Jurisdiction of Kentucky O.E.S., P.H.A., will host the 41 st Annual Midwest Regional Grand Masters and Masons, and the Grand Chapter and Eastern Stars. The conference will convene March 21 st, 22 nd, 23 rd, 24 th, in the year 2013, at the Galt House Hotel Louisville, Kentucky 104 North Fourth Street. We are accepting applications from vendors who are interested in participating in this endeavor; we have limited space designated within the hotel. The fee for table reservation is $75.00 per table and $25.00 for each additional table. All applications must be submitted no later than March 1 2013; no refunds will be issued after March 4 2013. Registered vendors will have access to set up display booth on Thursday evening, March 21 st, 2013.
