Australian Government Department of Immigration and Border Protection Visa Office Australian Consulate-General Hong Kong GUIDE TO COMPLETION OF FORM 80 FOR SIGNIFICANT INVESTOR VISA APPLICANTS The Form 80 Personal particulars for assessment including character assessment is a form used for collection of personal details of Significant Investor Visa applicants. The information gathered is used for assessment of each application. All questions must be answered and genuine, accurate information must be provided as the answers given may affect the outcome of the application. The form must be provided within the timeframe given for response to avoid delays in the processing of the application. If the applicant is unable to answer a question, please provide an explanation next to the space provided or at Part T of the Form. Please also give further details at Part T or attach a separate page if there is insufficient space. Below provides clarification on how to answer some questions which relate to the specific information we require, but that we have frequently found to be missing in many responses. Please note that all questions on the Form 80 must be answered, not just those outlined in this document. The Form 80, 04/14 edition, is the one referred to. Question 2 Do you ever use a Chinese Commercial Code Number for your names shown in the passport/travel document at Question 1? Chinese Commercial Code for applicant s name (if the applicant has a Chinese name) If this cannot be provided, the applicant must provide their name in Chinese characters. Ensure the full details are provided as stipulated for Question 11. Please ensure details of all the passports the applicant used during his/her travel detailed at question 19 are provided. Please specify if exact information cannot be provided due to the passports having been lost or discarded. However, please still give as much information as possible. At Part T of the form, you should then indicate whether the applicant has used those passports for travel and the countries that he/she has travelled to. Simply stating that the passport is lost is not acceptable you will be asked further questions and the application will be delayed. Question 15 Do you currently have, or have you ever had, any national identity documents or numbers (including birth registration numbers, social security cards etc)? Please ensure you provide the name of the identification document (e.g. identity card ) in the first column, then given the actual number of the document in the column on the right. Question 16 Do you have any email addresses? : Include all personal, student and work email addresses. Do not include migration agent email addresses. If there are gaps, you must provide an explanation at Part T Additional information. All addresses of the places where the applicant has lived during the last 10 years are required, including current address. Please clarify the usage of each residence if the applicant has more than one current residential address. Include details such as whether one may be a holiday house and the other may be his/her usual residence. Please also advise how frequently the applicant stays at each place. Ensure the addresses are listed chronologically and it is very important that there are NO time gaps between addresses. Question 19 Have you travelled to any country other than Australia in the last 10 years? : If you are applying for a refugee/humanitarian visa, you must provide all visits to any country other than Australia for the last 30 years. Include: work or study outside your usual country of residence; holiday/leisure trips; business; military deployment; visits back to your own country. All records of international travel during the past ten years are required. If exact information cannot be provided, please specify reason for such, e.g. due to the passports having been lost or discarded. Please still provide as much information as possible, such as the countries that the applicant travelled to. For example Europe or North America is not acceptable and multiple countries on one trip is not acceptable, this is because the arrival and departure dates from each country must be indicated. 5

Question 20 Give details of all employment and unemployment since birth. Employment includes: all paid employment; self-employment/family business; work experience/internships; unpaid employment/volunteer work. Unemployment includes: from date of birth up until first employment; all gaps/breaks between employment; all gaps between education. If you are currently retired write CURRENTLY RETIRED in Name of business field and provide details in the Your occupation and duties field. If you have never worked write your date of birth in the field and NEVER WORKED in the Name of business/company field and provide details in the Your occupation and duties field. : Provide your employment and unemployment history with no gaps in the timeline. Question 21 Give details of all education and qualifications since birth Education/qualification includes: primary, middle and high school; college/vocational schools; university; research/thesis; specialist training; skill /trade qualifications. ALL education details are required, including courses completed, withdrawn or currently studying. Please note that NO TIME GAPS are acceptable between periods. Family members include nieces, nephews, cousins, in-laws, grandparents etc. who are travelling with you Please indicate if the applicant has no siblings. Please give details for applicant s natural children, adoptive children and step-children. Information for children who are not included in the visa application is also required. Specify if there are no children. The following information is required for the applicant s partner, parents, siblings, children and other family who are travelling.
